(Cedar Rock) is pleased to submit this report describing activitiesand results for the in-place abandonment of the onsite UST located at 108 Nelson Street in PilotMountain, North Carolina. The UST abandonment was performed on April 18, 2016. Included inthis report are project documents consisting of a disposal manifest, and photographic documentation.This report should be kept with other documents pertaining to the residential property.Soil contamination was detected beneath the UST during a site inspection performed on April I 1,2016. Although the State no longer requires further assessment of the contamination, the State doesrequire the filing of a Notice of Residual Petroleum (NRP) prior to conveying the property to anotherparty. The NRP includes land use restrictions prohibiting any disturbance of site soils within aspecified radius of the estimated extent of soil contamination, and prohibits the use or installationof any water supply wells on the property. Once the NRP has been filed, the Department ofEnvironmental Quality (DEQ) will issue a conditional No Further Action (NFA) letter to officiallyclose out the petroleum release incident. Upon receipt that local offrcials and adjacent propertyowners have been notified of the release, the DEQ will reissue the NFA letter with no conditions.Cedar Rock filed the NRP (see attachment) on May 3, 2016 and will complete the PublicNotification requirement once the DEQ issues the conditional NFA letter.Enclosed is an invoice for this work effort. (Cedar Rock) was contracted by Ms. Wanda Porter of Asheboro,North Carolina to abandon in-place an underground storage tank (UST) located at 108 Nelson Streetin Pilot Mountain, North Carolina. The UST was first inspected on April 11,2016. An inactiveUST was observed to be present oriented parallel to and about 3.5 feet off the rear (eastern)foundation wall of the residence and lying partially beneath the rear deck. The northern end of theUST is located approximately 28 feet from the southeastern corner of the residence. The depth tothe top of the UST is approximately 23 inches below land surface (bls). The UST has a length ofabout 70 inches with a diameter of approximately 48 inches and a capacity of about 550 gallons.Both the fill and vent pipes were intact upon inspection. The UST was measured to containapproximately 25 gallons of fluid. White flagging was installed to mark the north end of the UST.Once the orientation of the UST was determined, two soil borings were performed, one at each endof the UST using a3.25 inch diameter stainless steel hand auger. The soil borings were advancedto a depth up to approximately 8.5 feet bls, approximately 2.6fieet below the bottom of the UST.The native soils underlying the UST consisted of a micaceous orange reddish brown sandy silt. Thesoils beneath the north end of the UST appeared to be clean with no observable petroleum odor ordiscoloration. However the soils beneath the south end of the UST exhibited a moderate petroleumodor suggesting significant leakage from the UST has occurred. The incident release was reportedto the North Carolina Department of Environmental Quality on April 12,2016..2.0 PROJECTThe in-place abandonment of the UST was performed on April 18, 2016. The abandonmentconsisted of the hand excavation of the soils overlying a section of the UST by hand excavation toexpose the topside of the tank. A hole approximately l 5 feet in diameter was then cut on the topof the UST to allow for removal of the tank contents, cleaning and visual inspection of the USTinterior, and subsequent filling. The tank was pressure washed of all petroleum sludge.Approximately 65 gallons of fluid (including rinsate) were removed from the UST. The disposalmanifest for the tank contents is located in Appendix A.After the interior of the UST was cleaned, the tank was then entirely filled with an excavatableflowable fill cement slurry. Once filled, the exposed portion of the UST was then covered with sitesoils (see Appendix B - Photographic Documentation). Cedar Rock filed a Notice of ResidualPetroleum on May 3,2016 at the Surry County Register of Deeds Office.
