These activities have been conducted following the discovery of petroleum impacted soil during the UST closure assessment at the subject site. The soil remediation consisted of the excavation and disposal of approximately 426 cubic yards of contaminated soil and confirmatory sampling of in-situ soils. All activities were conducted in accordance with North Carolina Department of Environmental Quality (NC DEQ) guidelines and the requirements of 15A NCAC 2N. Site History 1. Introduction Mid-State Petroleum Leasing, Inc. owned an UST system at 1675 US Highway 601 in Mocksville, NC, which was referred to as Cohen’s Citgo (Pop Shoppe #233). The subject property was operated as a fueling station by Mid-State Petroleum Leasing, Inc. until 2022. The former tanks were removed from this subject property on January 31, 2023. A release was discovered on November 24, 2008 when the middle gasoline UST failed and has been an open incident (#37355) since that time. Following removal of the tanks, product piping, and dispensers, contaminated soils were detected in the native soils beneath the former UST system. Figure 1 illustrates the location of this facility on the Mocksville Quadrangle U.S.G.S. Topographic Map. Figure 2 illustrates the site layout of this facility and the location of the remedial excavation. After removal of the gasoline USTs, the contaminated soil was accessible for the first time since the release discovery in 2008. Paragon Environmental Consultants, Inc. was contracted by Mid-State Petroleum Leasing, Inc. to perform soil remediation activities and confirmatory soil sampling and laboratory analyses in the impacted areas of the former UST system at this project site. Contained herein is a summary of activities which were completed at the project site. 2. Scope of Services This Initial Abatement Action Report contains documentation concerning the following activities which have been conducted at this facility: o Excavation of contaminated soils and backfilling of the excavation performed by KBS Earthworks, Inc. of Julian, NC o Transportation of contaminated soil by KBS Earthworks, Inc. and disposal of the excavated material at AES in Thomasville, NC o Collection of four soil samples from the vertical limits of the remedial excavation performed by Paragon Environmental o Laboratory analyses of soil samples conducted by Waypoint Analytical, LLC of Charlotte, NC Page - 2 C. Site Investigation 1. Excavation and Soil Sampling On March 8, March 9, and March 14, 2023, KBS Earthworks, Inc. and Paragon personnel mobilized to the subject site and attempted to remove the petroleum impacted soil which had been confirmed by the UST closure assessment. The impacted soils in the excavation area were excavated to a depth at or beneath the shallow groundwater table. In order to verify site conditions, four soil samples were collected from the bottom of the remedial pit. Appendix A contains a geologic log of excavation for the soil remediation activities at this facility. All of the soil samples collected from the remedial excavation at Cohen’s Citgo (Pop Shoppe #233) were analyzed by EPA Method 8260 and MADEP methods for Volatile Petroleum Hydrocarbons (VPH). The in-situ soil samples from the remedial excavation were collected with the excavator bucket and were immediately placed into laboratory supplied glassware and placed on ice for transportation to the analytical laboratory utilizing EPA approved chain-of-custody procedures. According to the laboratory analytical results for the final in-situ soil samples, sample E, which was located in the eastern corner of the remedial excavation, indicated four Method 8260 compounds and three carbon fraction classes in excess of the Residential Standards. Benzene, Ethylbenzene, Naphthalene, and 1,2,4-Trimethylbenzene were reported by Method 8260 at levels above the Residential Standards in this sample. C5-C8 Aliphatics, C9-C18 Aliphatics, and C9-C22 Aromatics were also shown at concentrations which exceed the Residential Standards in the E soil sample. Soil sample W, obtained from the western corner of the remedial excavation, showed four Method 8260 compounds and two carbon fraction classes in excess of the Residential Standards. Benzene, Ethylbenzene, Naphthalene, and 1,2,4-Trimethylbenzene were reported by Method 8260 at levels above the Residential Standards in this sample. C5-C8 Aliphatics and C9-C22 Aromatics were also shown at concentrations which exceed the Residential Standards in the W soil sample. Sample N, collected from the northern corner of the remedial excavation, indicated Benzene, Ethylbenzene, Naphthalene, and 1,2,4-Trimethylbenzene at levels above the Residential Standards. Sample S, collected from the southern corner of the remedial excavation, indicated Ethylbenzene, Naphthalene, and 1,2,4-Trimethylbenzene at levels above the Residential Standards. Figure 2 illustrates the area of soil remediation and the locations of the in-situ soil samples collected at this facility. The analytical results for the soil analyses are summarized in Table 1, and a copy of the laboratory analytical report and the chain-of-custody record is included in Appendix B. All samples were obtained in accordance with Paragon’s Standard Operating Procedures which are included as Appendix C. 2. Soil Disposal A total of approximately 426 cubic yards (595.82 tons) of petroleum impacted soils were removed during the site remediation activities on March 8, March 9, and March 14, 2023. This material was transported by KBS Earthworks, Inc. to AES’s facility in Thomasville, NC for treatment and disposal. Copies of the soil disposal manifests for the petroleum impacted material from Cohen’s Citgo (Pop Shoppe #233) are contained as Appendix D. Page - 3 3. Conclusions and Recommendations 3. A Conclusions and Recommendations The soil remediation activities at 1675 US Highway 601 have been completed. From a review of all information gathered during this removal project, Paragon Environmental Consultants, Inc. makes the following conclusions: o Laboratory results for samples from in-situ soils following the excavation activities indicated four Method 8260 compounds and three carbon fraction classes in excess of the Residential Standards. o A total of 595.82 tons of petroleum contaminated soil were removed from this facility on March 8, March 9, and March 14, 2023. This material was transported by KBS Earthworks, Inc. to AES’s facility in Thomasville, NC for disposal. 3. B. Recommendations Based upon a review of all information gathered during the soil contamination project, Paragon makes the following recommendations: o Two 4-inch recovery wells should be installed in the remedial excavation to assess current groundwater conditions and conduct further free product recovery / groundwater remediation at this facility. o A copy of this report should be forwarded to the following address: Winston-Salem Regional Office - UST Section 450 W. Limitations This report has been prepared for the exclusive use of Mid-State Petroleum Leasing, Inc. for the specific application to the referenced site located in Davie County, North Carolina. The assessment was conducted based on the scope of work and level of effort desired by the client. Our findings have been developed in accordance with generally accepted standards in the practice of Initial Abatement Reports in the State of North Carolina, available information, and our professional judgment. No other warranty is expressed or implied. The data presented in this report are indicative of conditions that existed at the precise locations sampled and at the time the samples were collected. Additionally, the data obtained from the samples would be interpreted as meaningful with respect to the parameters indicated in the laboratory reports. No additional information can be logically inferred from this data. Comments: Page 39 of 40 Page 40 of 40 APPENDIX C STANDARD OPERATING PROCEDURES STANDARD OPERATING PROCEDURES PARAGON ENVIRONMENTAL CONSULTANTS, INC. I. SOIL SAMPLE PROCEDURES 1. Collect all samples using disposable Latex gloves. Gloves are not to be re- used. 2. Place samples into laboratory supplied glassware following requirements for specific analysis. 3. Label samples with sample ID, date, time, and job number. Immediately place samples on ice or in refrigerator to be cooled to approximately 4 degrees Celsius. 4. Store all samples on ice or refrigerate until samples are delivered to the laboratory. 5. Complete a chain of custody record for samples to be submitted to laboratory. Sign and date the chain of custody when samples are relinquished in accordance with EPA chain of custody protocol. II. GROUNDWATER SAMPLING 1. Use new disposable bailer and new nylon string to develop well and collect sample. Handle bailer and string with Latex gloves. 2. Develop well by removing 3 well volumes of water. Dispose of water in accordance with NCDEQ guidelines. 3. Following well development obtain samples in laboratory supplied glassware following requirements for specific analysis. 4. Handle, store, and transport samples in same manner as for soil samples. See items I.3, I.4, and I.5 above. III. EQUIPMENT CONTAMINATION 1. Decontaminate augers, split spoons, and other sampling equipment by the following procedure: A. Soap and tap water wash B. Tap water rinse C. Distilled deionized water rinse D.
