B. Site History and Background
The Kash & Karry #11 site is located at 4115 Old Tar Road in Winterville, North
Carolina. The site is currently owned by Wayne G. Hardee and Charles R. Hardee
ETAL of Greenville, North Carolina. A gasoline station/convenience store is located
onsite and currently does not distribute gasoline as the USTs are in temporary closure.
A release occurred at the site on August 30, 2010 and the amount of product released
is unknown. The location of the UST basin is illustrated on Figure 2. For detailed
information about the UST system refer to Table 1 UST/AST System and Other Release
Information and Table - 2 UST/AST Owner/Operator and Other Responsible Party
Information.
C. Receptor Information
A site reconnaissance of the surrounding area was conducted by Blue Ridge personnel
on January 21, 2015. In addition, Blue Ridge passed out water supply well (WSWs)
questionnaires to all property occupants located within 500 feet of the subject site. Two
WSWs (WSW-1 & WSW-2) were identified during the site reconnaissance conducted.
WSW-1 is located approximately 349 feet from the source area at the New Beginnings
Community Church. The well system was found to be empty at the time of the
reconnaissance and appeared to be connected to public water. WSW-2 is located 751
feet from the source area at the Plant and See Nursery. The location of the WSWs is
illustrated on Figure 3. Of the two WSWs identified, WSW-1 was found to be inactive,
while WSW-2 was noted to be currently in-service primarily for irrigation usage.
During the site reconnaissance Blue Ridge observed water meters at almost all
residences. Public water is available to properties located within a 1,500 foot radius of
the site. Underground utilities on or adjacent to the subject property include
underground water, gas, electric, fiber optic and phone lines. Adjacent property owners
are noted on Table-3 Adjacent Property Owners. A Limited Site Assessment Risk
Classification and Land Use Form is included as Appendix B.
D. Site Geology/Hydrogeology
According to soil descriptions from the installation of the LSA monitoring wells MW-1
through MW-4, the soils underlying the subject site consist predominately of grey to
sandy clay to brown clayey sand. Please refer to Appendix C for additional lithological
descriptions. Two unnamed drainage features which flow into Fork Swamp are located
approximately 1,073 feet south/southwest and 1,205 feet northeast of the source area.
The measured depths to groundwater in monitor wells MW-1 through MW-4 on January
22, 2015 ranged between 2.21 feet bgl at MW-2 to 5.03 feet bgl at MW-4. A

groundwater contour map depicting aquifer flow is enclosed as Figure 5. Based on the
groundwater elevations obtained on January 22, 2015 groundwater flow is calculated to
the east.
E. Sampling Results
Limited Site Assessment Activities
On October 28, 2014 Blue Ridge installed monitoring well MW-1 adjacent to the UST
basin. The well was completed to 12 feet below grade level. A soil sample was collected
at 1 foot bgl during the installation of MW-1. The soil sample was laboratory analyzed
according to EPA Method 8260 and MADEP Method VPH. Laboratory results of the soil
sample collected at 1 foot indicated detectable contaminant concentrations of Benzene
(0.29 mg/kg) and MTBE (0.44 mg/kg) above the soil-to-water Maximum Soil
Contaminant Concentration (MSCC). Soil analytical results are summarized in Table-5
Select Summary of Soil Analytical Laboratory Results. Please refer to Figure 5 for soil
analytical results at site specific locations.
On October 28, 2014 Blue Ridge gauged and sampled monitoring well MW-1. A
groundwater sample was obtained and laboratory analyzed according to EPA Method
6200B, EPA Method 3030C and MADEP Method VPH. The laboratory results of the
groundwater sample collected on October 28, 2014 from MW-1 revealed detectable
concentrations of Benzene (20,000 ug/L), Toluene (39,000 ug/L), Ethylbenzene (5,400
ug/L), Xylenes (23,600 ug/L), MTBE (1,600 ug/L), Naphthalene (920 ug/L), n-
Propylbenzene (530 ug/L), 1,3,5-Trimethylbenzene (980 ug/L) and 1,2,4-
Trimethylbenzene (3,700 ug/L) above the North Carolina Groundwater Quality
Standards (GWQS). In addition the reported concentration of Benzene exceeded the
Gross Contamination Level (GCL). Because the groundwater results of MW-1 were
above the North Carolina Gross Contaminate Levels a Phase II LSA was completed.
On January 21, 2015, Blue Ridge mobilized back to the site and installed monitoring
wells MW-2, 3 and 4. On January 22, 2015 Blue Ridge gauged and sampled monitoring
wells MW-2 through MW-4. A groundwater sample was obtained and laboratory
analyzed according to EPA Method 6200B and MADEP Method VPH. The laboratory
results of the groundwater samples collected on January 22, 2015 from MW-2 through
MW-4 revealed concentrations of Benzene, MTBE and Naphthalene that exceeded the
North Carolina GWQS from the groundwater sample collected from MW-4. All other
remaining constituents analyzed were below the GWQS in the groundwater samples
collected on January 22, 2015. Groundwater analytical results are summarized in Table-
6 Summary of Groundwater Sampling Results. Please refer to Figure 7 for groundwater
analytical results at site specific locations. A copy of the laboratory analytical data is

F. Conclusions
• Two WSWs (WSW-1 & WSW-2) were identified during the site reconnaissance
conducted. WSW-1 is inactive and located approximately 349 feet from the
source area. WSW-2 is currently in-service primarily for irrigation usage and is
located 751 feet from the source area. After discussions with the NC DENR
Project Manager, it was decided not to sample the nursery well at this time.
• Public water is available to all occupants located within a 1,500 foot radius of the
subject site.
• Two unnamed drainage features which flow into Fork Swamp are located
approximately 1,073 feet south/southwest and 1,205 feet northeast of the source
area.
• The source area is not located within a designated well head protection area.
• A soil sample collected at 1 ft. indicated a violation of the soil-to-water MSCC for
Benzene (0.29 mg/kg) and MTBE (0.44 mg/kg).
• Laboratory results of the groundwater sample collected from MW-1 revealed
detectable concentrations of Benzene, Toluene, Ethylbenzene, Xylenes, MTBE,
Naphthalene, n-Propylbenzene, 1,3,5-Trimethylbenzene and 1,2,4-
Trimethylbenzene that exceeded the GWQS.
• A reported concentration of Benzene (20,000 ug/L) in MW-1 exceeded the GCL.
• Laboratory results of the groundwater sample collected from MW-4 revealed
detectable concentrations of Benzene, MTBE and Naphthalene that exceeded
the GWQS.
• A reported concentration of Benzene (20,000 ug/L) in MW-1 exceeded the GCL.
• Based on the information presented in this report, it appears additional
assessment may be warranted. Furthermore it is recommended that the site be
ranked “Intermediate Risk” due to the concentration of Benzene reported in MW-
1.

Drilling and Field Activities Protocol
Pre-Drilling Protocol
Blue Ridge will contact a commercial utilities location service a minimum of 48 hours (per location service
requirements) prior to mobilization of the drill crew to the project site. It will be the responsibility of the utilities
location service to locate and delineate (by marking the ground surface), to the extent possible, buried or
underground utilities which may be encountered during or impeded by drilling activities. This should include,
but may not be limited to: electrical power services, private and/or municipal water and sewer services,
telecommunication services, natural gas services, and/or other public or municipal utilities. Borehole locations
will not be made closer than 36" from surface delineated utilities. If closer proximity to delineated utilities is
required, bore hole advancement will be conducted by hand auger boring protocol.
Prior to commencement of drilling, the augers, down hole equipment and associated tools will be cleaned in a
designated decontamination control area using a steam cleaner or high pressure washer. Bulk soil material
which may remain inside or outside of the augers or tools will be removed. Augers will also be pressure
washed between each successive bore hole to remove any soil material from the preceding boring.
Before raising the drill rig mast, Blue Ridge drilling personnel will observe over-head conditions for the
presence of aerial mounted utilities, tree limbs, canopies, awnings, building overhangs or other structural
impedances to normal drill rig operations.
Drill Protocol and Techniques
Bore holes utilized in sub-surface intrusive investigations will be advanced using a trailer mounted Simco 2400
SK-1 drill rig equipped with 4.5 inch outside diameter solid stem augers or 3.25 inch hollow stem augers.
Where warranted, shallow boring hole advancement may require utilization of a steel hand auger equipped
with a 3" inside diameter barrel.
Soil Sampling Collection and Analysis
The sample preparation area at each bore hole location will be located as reasonably possible away from
running motors or where discharged vapors, exhaust and/or fumes may come in contact with the sample.
Towels, field meters, sample jars and other equipment will be placed such that they do not come into contact

with soil or other materials which may contaminate the sampled soils.
Unless specified by the client or by project requirements, soil samples will be collected at approximately 5 foot
intervals. Soil samples will be retrieved according to one of two sampling protocols (dependent upon project or
client requirements): (1) by split barrel sampling protocol or (2) from bulk soil material retrieved from the solid
stem auger following flight advancement.
In an effort to reduce cross contamination of samples, soil samples which are to be submitted for chemical
analysis will only be handled by the geologist using fresh, manufacturer clean, latex examination gloves.
Following retrieval, the soil sample from each interval will be split into roughly equal portions with one portion
subjected to field examination and classification by Blue Ridge geologic personnel and the remaining portion
placed into a glass sample container to await selection for chemical analysis.
Samples which are to be submitted for volatile constituent analysis will be thermally isolated by being placed
into the plastic ice chest.
After the sample is retrieved from the bore hole and placed into the sample container, the project name,
sampling interval, data and time will be noted on the container.
Soil Screening and Photoionization Detector
Soil samples collected from each sampling interval will be subjected to on site screening by head space
analytical method utilizing a Photovac brand portable photoionization detector (PID). Prior to the soil
screening, the PID will be calibrated in the field to 98.9 parts per million (ppm) using isobutylene calibration gas
per manufacturer's standards.
Soil screening will be conducted by placing a spilt portion of each sample collected from the specified sampling
interval into a resealable plastic bag. The bag is then placed aside to allow the head space air above the
sample to equilibrate. Following the anticipated equilibration interval, the probe of the PID is inserted through
the side of the sample bag and the resulting levels of volatile organic vapors recorded. PID levels are recorded
in parts per million (ppm) and may be an indication of the relative concentration of volatile organic compounds
in the soil.
Monitoring Well Construction Protocol
Monitoring well bore hole advancement will be conducted utilizing 3.25 inch inside diameter hollow stem auger.
For typical monitoring well construction, the bore hole will be advanced through the vadose zone (over burden
soil) into the saturated zone to approximately 10 feet below the water table thereby allowing approximately 10
feet of hydrostatic head within the well. The boring will be converted into a groundwater monitoring well by the
emplacement of 2 inch inside diameter schedule 40 PVC pipe (well screen and riser). The well screen will
have a manufacturer machine cut 0.010 inch slot width and will be emplaced so the top of the well screen will
be located above the anticipated seasonal high water table. The annular space surrounding the well screen

will be backfilled utilizing an FX-2 filter sand (45 to 55 mm grain size) to approximately 2 feet above the top of
the well screen. The filter pack will be isolated by the emplacement of approximately 2 feet of bentonite pellets
and the bentonite seal will then be hydrated and allowed to seal. Following the bentonite seal hydration, the
remaining annular space will be backfilled to the surface grade utilizing a neat Portland Type II cement or
similar type grout. Typically the wells will be completed at grade level inside a traffic rated steel vault and the
well sealed using a water-tight locking pressure cap. Where it is possible, the well vault will be raised above
the existing surface grade approximately 1 inch to promote surface water run-off and reduce the potential for
standing water above the well head.
Monitoring Well Development
Blue Ridge will allow an anticipated time interval for the well bore to stabilize following construction (typically an
minimum of 72 hours). The monitoring well will then be developed by hand bailing utilizing a fresh manufacturer
clean polyvinyl bailer. Well development will be conducted until: approximately 5 well volumes of water have
been removed, the water is relatively silt and sediment "free", or until essentially dry.
Well development should be performed to accomplish the following:
1) To remove silt and sediment which may have entered the well during construction activities.
2) To remove mud cake and "smeared material@ on the bore hole walls from drilling.
3) To seat and grade the filter pack.
4) To promote hydraulic conductivity into the well bore.
Monitoring Well Gauging
Static water levels within each monitoring well will be measured utilizing a Slope Indicator Company electronic
Water Level Indicator model #51453. Measurements will be made from the top of the casing (riser) and
recorded to the nearest 0.01 foot.
Blue Ridge will electronically measure for the presence of free product in suspect wells utilizing a Keck
Instruments, Inc. electronic oil/water interface probe model #KIR 89. Where free product is indicated to be
present, the depth to the top of the product layer and the depth to the top of the water will be recorded.
Measurement will be made from the top of the well casing (riser) and recorded to the nearest 0.01 foot.
Monitoring wells will be gauged before performing sample collection events at other times that may be required
and warranted.
The water level indicator and the oil/water interface probe will be cleaned with a solution of laboratory grade
soap (Alconox) and distilled water, rinsed with distilled water, rinsed with methanol alcohol and finally rinsed

with deionized water both before the first well is checked and between successive wells. Bailers used to
inspect the water surface will also be cleaned between successive wells using a solution of 5% tri-sodium
phosphate and water.
Surveying Monitoring Well Casing Elevations
To provide information regarding the elevation of the top of each well casing relative to an established data
point (bench mark), Blue Ridge geologic personnel will survey the top of each well casing if necessary. An on
site bench mark will be established with a given elevation of 100.00' and each well casing elevation will be
measured with respect to the bench mark. Measurements will be made from the lowest point of the well riser
and recorded to the nearest 0.01 foot. All water level measurements will be made from this point to provide
consistency in field measurements.
Groundwater Sampling
Whenever possible, prior to conducting the initial sample collection event for any given monitoring well, Blue
Ridge will allow an anticipated time (a minimum of 72 hours) for the well to stabilize following monitoring well
development.
Before conducting each sampling event, an attempt will be made to purge each well of a minimum of three
volumes of water or until the well becomes essentially dry. All wells will then be allowed to recharge in part or
in whole prior to sampling.
Groundwater samples will be collected by Blue Ridge personnel utilizing disposable manufacturer clean
polyvinyl bailers equipped with a bottom ball check valve. An inert string will be used to lower the bailer to the
water surface. The string will be replaced between successive samples. The sample will be collected by
gently lowering the bailer into the water, thereby reducing degasification and/or volatilization of the sample.
After the bailer is lowered into the water, the sample will be allowed to stabilize for approximately three
seconds, allowing the ball valve to close. Care will be taken not to overfill the bailer, allowing inspection of the
water surface upon removal of the bailer from the well. Once the bailer is removed, water samples will be
transferred to glass vials in such a manner as to reduce volatilization. To reduce the potential of sample cross
contamination, Blue Ridge personnel will don fresh disposable latex examination gloves
between each monitoring well sampling point and whenever contact with groundwater is possible.
Transportation of Samples to Laboratory
Before drilling activities are initiated, a plastic ice chest will be prepared for temporary soil and water sample
storage. Preparation will include placing ice packs into the ice chest for thermal isolation in an attempt to
preserve sample integrity. The cooler will be pre-chilled using blue ice, ice packs or similar items to provide
thermal isolation of the sample materials.

The maximum holding time for samples to be tested for volatile organics is 14 days. Therefore, samples will be
delivered to the laboratory and analyzed by the laboratory within this time frame. The laboratory has been
consulted regarding holding times.
If the samples are taken directly to the lab from the drill site, they will be held in the same pre-chilled cooler. If
the samples cannot be taken directly to the lab, they will be taken to Blue Ridge's office, where the sample
containers will be transferred immediately from the cooler to the assigned refrigerator, until such time that they
can be taken to the lab. When the samples are scheduled to be delivered to the lab, they will be transferred
from the refrigerator to a pre-chilled cooler until delivery to the lab has been made.
Blue Ridge Chain of Custody Protocol
For each set of samples collected from the specified project site, Blue Ridge will enter all samples, containers,
and ice chests into standard chain of custody protocol. A chain of custody form will be established for each
sampling event a includes the following information: (1) project name and location, (2) sampling personnel, (3)
project management personnel, (4) sample identification numbers, (5) sample collection date and time, (6)
sample matrix {soil or water}, (7) sample description, (8) preservatives utilized, (9) number of containers, (10)
requested parameters, (11) miscellaneous field or laboratory remarks, (12) and transference records.
The chain of custody form will accompany the samples through each phase of the delivery process.
