Laboratory Report 1 INITIAL ABATEMENT ACTION REPORT FOR Buttery and Beanery 1653 Mecklenburg Highway Mooresville, Iredell County, North Carolina ATC Associates of North Carolina, P.C. (ATC) has prepared the following report for the Estate of Linda Holthouser, detailing underground storage tank (UST) closure and initial abatement activities performed at the Buttery and Beanery facility located at 1653 Mecklenburg Highway Mooresville, Iredell County, North Carolina. This report has been prepared pursuant to applicable sections of Title 15A of the North Carolina Administrative Code (NCAC), Chapter 2, Subchapter 2L and in accordance with the State of North Carolina Department of Environmental Quality (NCDEQ), Division of Waste Management (DWM), Underground Storage Tank (UST) Section Guidelines for Site Checks, Tank Closure, and Initial Response and Abatement, effective January 19, 2021. 1.0 SITE HISTORY AND CHARACTERIZATION 1.1 UST OWNER AND OPERATOR INFORMATION Two 6,000-gallon gasoline USTs, one 550-gallon kerosene UST, and two orphan 1,000-gallon gasoline USTs were removed from the referenced facility on November 22 and 23, 2021. The USTs, product lines/piping, and dispensers were owned by The Estate of Linda Holthouser of Mooresville, North Carolina. Figure 2 depicts the site and pertinent structures, including approximate locations of the USTs, product lines/piping, and dispensers. Circle K Store #2723470 June 29, 2021 2 1.3 AST INFORMATION No aboveground storage tanks (ASTs) were identified on the property. 1.4 RELEASE INFORMATION During closure activities, soil samples were collected from the bottom of the UST basin along the sidewalls, along the product lines, and at the dispenser islands. Laboratory analytical results did indicated soil impacted above State Action Levels for Total Petroleum Hydrocarbons (TPH)-Diesel Range Organics (DRO) and Total Petroleum Hydrocarbons (TPH)-Gasoline Range Organics (GRO). A 24-Hour Release and UST Leak Reporting Form was submitted to NCDEQ on December 3, 2021. 1.5 SITE CHARACTERISTICS 1.5.1 Site Status The former Buttery and Beanery facility is vacant. The USTs, product lines/piping, and dispensers have been removed from the facility. Surface cover at the site consists of asphalt, and gravel. 1.5.2 Surrounding Property Use Properties surrounding the subject facility are primarily commercial and residential. The zoning status of the subject facility is commercial. The surrounding area is zoned commercial and residential. 1.5.3 Site Geology/Hydrogeology Based on observations made during UST closure activities, the soil profile consisted of red-brown silty clay. Groundwater was not encountered during the excavation activities, which extended to a depth of approximately 10 feet below ground surface (ft-bgs) in the area of the UST basin. 1.5.4 Receptor Survey During closure activities, ATC did observed evidence of potential water supply wells within 1,500 feet of the source area. The site and surrounding properties appear to be connected to the public water system and private wells. 1.6 INITIAL ABATEMENT ACTIONS Refer to Section 2.3 in regard to information regarding this site. 2.0 SITE CHECK REPORT 2.1 TIGHTNESS TESTING Information regarding tightness testing is not currently available. 2.2 SITE CHECK PROCEDURES Upon closure of the USTs, thirteen soil samples were obtained to document environmental conditions of soil from the UST basin, along the product lines, and beneath each dispenser. Soil samples were submitted for analysis by TPH – GRO and/or DRO, depending on sampling location. Initial Abatement Action Report ATC Associates of North Carolina, P.C. Circle K Store #2723470 June 29, 2021 3 2.3 INITIAL RESPONSE ACTIONS 2.3.1 Notifications A UST-3 Notice of Intent: UST Permanent Closure or Change In-Service was submitted to NCDEQ by ATC personnel on May 24, 2021. A UST-2A Site Investigation Report for Permanent Closure or Change- in-Service of REGISTERED UST was submitted to NCDEQ by ATC personnel on December 16, 2021. A UST-61 24-Hour Release and UST Leak Reporting Form was submitted to NCDEQ by ATC personnel on December 3, 2021. Copies of the referenced correspondence are presented in Appendix A. 2.3.2 Residual Material Residual product was removed from the USTs when they were taken out of service, prior to these removal activities. The interior of the USTs were pressure washed to remove product residue remaining in the tanks. Approximately 390 gallons of rinse water and product residue were generated from pressure washing the interior of the USTs. The rinse water was removed from the USTs via vacuum truck. Copies of the liquid manifest is included as Appendix B. 2.3.3 Source Control Actions Additional source control actions did not appear warranted at the time of site activities. 2.3.4 Contaminant Migration Control Measures Additional contaminant migration control measures were not utilized. 2.3.5 Measures to Identify and Mitigate Hazards ATC field personnel did not observe evidence of hazards during UST closure activities. 2.3.6 Contaminated Soil Storage, Treatment, and/or Disposal During UST removal activities, petroleum-impacted soil was identified in the vicinity of the UST basin. Only soil required to remove the USTs was removed from the excavation. 39.54-tons of impacted soil removed during the UST excavation was disposed of off-site by Zebra Environmental. Additional soil excavation was not completed due to the proximity of the on-site building to the UST bed. 2.3.7 Groundwater Assessment Groundwater was not encountered during the UST removal activities. 2.3.8 Free Product Assessment and Removal Evidence of free product was not observed on the soils in the UST basins; thus, free product removal activities were not warranted. 2.4 SOIL EXCAVATION ACTIVITIES During UST removal activities, clean overburden was stockpiled on-site. Impacted soil was removed from the site. 2.5 SITE INVESTIGATION Refer to Sections 3.0, 4.0, and 5.0 for information on assessment activities. Circle K Store #2723470 June 29, 2021 4 3.0 UST CLOSURE REPORT 3.1 CLOSURE PREPARATION A UST-3 form was filed with the NCDEQ Mooresville Regional Office by ATC personnel on November 2, 2021. Zebra Environmental personnel were responsible for obtaining any permits for the closure of the tanks, contacting the local fire department, and having utilities on the area properly located. 3.2 CLOSURE PROCEDURES Prior to the closure of the USTs, residual fuel had been pumped out of the tanks when they were taken out of service. Upon removal from the ground, the USTs were noted to be in good condition. The USTs were transported off-site for disposal. A copy of the tank disposal manifest is included in Appendix B. 3.3 CLOSURE SAMPLES Thirteen soil samples were collected from the UST excavation, fuel dispensers, along the product line piping in approximate 10-foot intervals, and at any observed connection points. Soil samples collected from the UST excavations, product line piping, and fuel dispensers were taken directly from the bucket of the excavator. All samples were collected utilizing new disposable nitrile gloves during to reduce the overall risk of cross contamination. Soil samples were placed into sealable Zip-loc® bags with the required volume for laboratory analysis being placed in laboratory provided bottle-ware. The remaining sample was allowed time for vapor stabilization at which point the head-space was screened for the presence of volatile organic vapors (VOCs) using a photoionization detector (PID). Samples T-1-A through T-3-B were collected from the UST basins, from a depth of approximately 6 to 10 ft-bgs. Soil samples D-1-A through D-2-B, L-1 and L-2 were collected from beneath piping or dispensers, from a depth of approximately 2-5 ft-bgs. Soil samples were preserved and packed into approved sample containers, stored in a chilled cooler, and submitted to North Carolina certified Pace Analytical Laboratories, in Huntersville, North Carolina. Soil samples were submitted for analysis by TPH – GRO and/or DRO, depending on sampling location. The locations of the UST system soil samples and select analytical results are provided in Figure 2. Laboratory analytical results did detected the presence of TPH-DRO or TPH-GRO above the NCDEQ Action Level of 50 and 100 milligrams per kilogram (mg/kg) in five of the soil samples collected. Table 3 provides a summary of the laboratory results for the TPH analytical results. A copy of the laboratory report is included in Appendix C. 3.4 SOIL EXCAVATION ACTIVITIES Potentially impacted soil was not excavated from the site due to the proximity of the building at the site. 3.5 SITE INVESTIGATION Additional site investigation activities were not conducted as part of the UST closure. 4.0 OVER EXCAVATION ACTIVITIES Over excavation, activities were not conducted as part of the UST closure. Circle K Store #2723470 June 29, 2021 5 5.0 GROUNDWATER ASSESSMENT ACTIVITIES Groundwater was not encountered during the UST removal activities. 6.0 CONCLUSIONS AND RECOMMENDATIONS ATC prepared an Initial Abatement Action Report for a UST closure Based on the information obtained during UST closure activities at the Buttery and Beanery facility located at 1653 Mecklenburg Highway, Mooresville in Iredell County, North Carolina. ATC offers the following conclusions:  Two 6,000-gallon gasoline USTs, one 550-gallon kerosene UST, two 1,000 gallon orphan gasoline USTs were removed from the referenced facility November 22-23, 2021. ASTs were not identified on the property.  The subject property and surrounding properties are primarily commercial and residential. The site and surrounding properties appear connected to the public water system and also are supplied water by private wells.  Approximately 390 gallons of rinse water and product residue were removed from the USTs via vacuum truck.  Thirteen soil samples were collected from the bottom of the UST basin, below fuel dispensers, and along the product line piping. Soil samples were submitted for laboratory analysis by EPA Method 8015C for TPH-GRO and/or TPH-DRO as applicable.  Groundwater was not encountered during the UST removal activities.  The concentration of TPH-DRO and TPH GRO were detected in soil samples gathered from the site. Based on the information obtained during these activities, additional assessment may be warranted. 7.0 LIMITATIONS This report has been prepared for the exclusive use of The Estate of Linda Holthouser. The opinions included herein are based on information obtained during the study, on our experience in accordance with currently accepted hydrogeological and engineering practices, and relevant regulatory guidelines at this time and location. Other than this, no warranty is implied or intended. If a Professional Engineer (P.E.) or a Licensed Geologist (L.G.) provides supervision for closure or change-in-service site assessment activities and signs and seals all closure reports then at least a five (5) working days’notice is acceptable. Completed UST closure or change-in-service site assessment reports, along with a copy of the UST-2A and/or 2B forms, should be submitted to the appropriate Division of Waste Management (DWM) Regional Office within thirty (30) days following closure activities. The UST-2 form should also be submitted to the Central Office in Raleigh so that the status of the tanks may be changed to permanently closed and your tank fee account can be closed out. Note: Tank fees may be due for unregistered tanks. UST closure and change-in-service site assessments must be completed in accordance with the latest version of the Guidelines for Site Checks, Tank Closure and Initial Response. The guidelines can be obtained at https://deq.nc.gov/about/divisions/waste-management/ust. Note: To close tanks in place you must obtain prior approval from the DWM Regional office located in the region where the facility is located. You must make sure that USTs removed from your property are disposed of properly. When choosing a closure contractor, ask where the tank(s) will be taken for disposal. Usually, USTs are cleaned and cut up for scrap metal. This is dangerous work and must be performed by a qualified company. Tanks disposed of illegally in fields or other dumpsites can leak petroleum products and sludge into the environment.
