2.0 EXECUTIVE SUMMARY
ATC Associates of North Carolina, P.C. (ATC) has completed a Groundwater Monitoring Report for the
Jeff’s Grocery site. This report documents groundwater sampling activities conducted at the site in
February 2023. The activities were performed based on a December 21, 2022, Task Authorization (TA-03)
by the Trust Fund Branch of the Underground Storage Tank (UST) Section, Division of Waste Management,
North Carolina Department of Environmental Quality (NCDEQ). The work is being performed as part of
State Lead Contract #N122019A between NCDEQ and ATC.
The former Jeff’s Grocery site is located at 2996 NC Highway 268 in Pinnacle, Stokes County, North
Carolina. The site is developed with one single-story brick building and is currently occupied by Cornerstone
Market, a convenience store. Three monitoring wells are located at the site. Eight potable water supply
wells are located within 1,000 feet of the site, one of which is located on the site property. Municipal water
is not available to the site or vicinity.
On February 14, 2023, ATC personnel mobilized to the site collect groundwater samples from monitoring
wells MW-1, MW-2, and MW-3 and on-site water supply well WSW-1. Laboratory results indicate methyl
tert-butyl ether (MTBE) above the North Carolina Groundwater Quality Standard (2L Standard), but below
the Gross Contaminant Level (GCL) in MW-1. No other contaminants of concern were detected above 2L
Standards. The sample collected from water supply well WSW-1 yielded a concentration of
trichloroethylene (TCE) which exceeded the laboratory method detection limit but did not exceed the
laboratory reported detection limit or 2L Standard. Based on the sampling results collected to date, the
plume is limited to the area around the former dispenser island and MW-1.
The site is not a candidate for closure due to the presence of potable water supply wells within 1,000 feet
of the site, most notably WSW-1 located on the source property. ATC recommends groundwater and water
supply well sampling on an annual basis. If funding is available, ATC recommends delineating the horizontal
and vertical extent of the groundwater plume.
3.0 SITE DESCRIPTION
3.1 SITE CHARACTERIZATION
The Jeff’s Grocery site formerly operated as a convenience store and petroleum retail facility. Currently,
the site is occupied by Cornerstone Market, a convenience store. The former UST system consisted of two
3,000-gallon gasoline USTs located at the southwestern corner of the onsite building. The USTs were in
operation from 1984 until March 2008 when they were removed from the site. Three monitoring wells (MW-
1 through MW-3) are associated with the site and one potable water supply well (WSW-1) is located on the
site property.
Properties surrounding the site are used for residential and agricultural purposes or are undeveloped. A
site topographic map is included as Figure 1. Eight water supply wells are located within 1,000 feet of the
site. Water supply well ownership information is included in Table 1. A site map depicting monitoring well
locations and other pertinent site features is included as Figure 2. A potential receptor map is included as
Figure 3. Site photographs are included as Appendix A.
3.2 SITE BACKGROUND
In August 2007, ECS Carolina, LLP, submitted a Report of Environmental Services documenting soil and
groundwater assessment activities performed at the site at the request of property owner, Mr. Chad Lynch.
During the assessment, soil borings GP-1 through GP-10 were advanced around the UST system and a
groundwater sample was collected directly downgradient of the USTs. Soil samples collected from borings
GP-7 through GP-10 contained levels of total petroleum hydrocarbons (TPH) gasoline range organics
(GRO) and/or diesel range organics (DRO) above the NCDEQ Action Level. The groundwater sample did
not contain petroleum compounds above laboratory detection limit.

On March 26, 2008, Geological & Environmental Services submitted a UST Closure Report for the site.
During the closure, two 3,000-gallon gasoline USTs were removed from the site. Soil samples collected
from beneath the USTs and dispenser did not contain petroleum hydrocarbons above the respective
standards; however, the soil directly beneath the product lines indicated impact. Over excavation was
performed in the vicinity of the product lines. Following over excavation activities, samples were collected
from beneath the product lines at approximately 8 feet below ground surface (bgs) and contained soil
impacts above Soil-to-Groundwater Maximum Soil Contaminant Concentrations (MSCCs). Approximately
50 tons of soil was removed from the site during the UST Closure.
On April 29, 2011, Terraquest Environmental Consultants, P.C. submitted a Receptor Survey and Water
Supply Well Sampling Report. Based on the results of the receptor survey, nine active water supply wells
were located within 1,000 feet of the site. In November 2019, the owner of WSW-2 previously indicated
there is no water supply well at this location; however, he does plan on installing a water supply well in the
future. Water supply wells WSW-1 through WSW-6 were sampled in March and April 2011. Lead was
detected in water supply wells WSW-5 and WSW-6 above the 2L Standard. Based on a 2-step, first draw
method of sampling, it was determined the source of lead for WSW-5 was from the home supply line while
the source of lead for WSW-6 was the groundwater. No other compounds were detected in the samples.
On June 18, 2015, ATC submitted a Letter Report documenting a water supply well sampling event. During
the event, samples were collected from water supply wells WSW-1, WSW-3, WSW-5, and WSW-6.
Chloromethane was detected in water supply well WSW-6 at a concentration of 0.36 micrograms per liter
(g/L), which is below the laboratory reporting limit as well as the 2L Standard. No other compounds were
detected is the water supply well samples. Water supply well information is included in Table 1 and historical
water supply well data is included as Table 2. ATC recommended completion of a soil assessment around
historical soil borings GP-7, GP-8, GP-9, and GP-10 to determine current soil impacts at the site.
On October 8, 2015, ATC submitted a Soil Assessment Report documenting the collection of four soil
samples (SB-1 through SB-4). The soil borings were placed in the vicinity of former soil borings GP-7
through GP-10 to determine current soil concentrations at the site. Based on the laboratory results, it
appears compounds are attenuating in the soil. ATC recommended installing one monitoring well in the
vicinity of the former product lines to determine impacts to groundwater.
On June 3, 2016, ATC submitted a Groundwater Monitoring Report documenting the installation and
sampling of monitoring well MW-1. The sample collected from MW-1 indicated MTBE above the 2L
Standard. ATC recommended installing two additional monitoring wells to determine groundwater flow
direction, placement of O-SOX in monitoring well MW-1, and performing a water supply well sampling event.
On August 27, 2016, ATC submitted a Water Supply Well Sampling Letter Report documenting the
sampling of water supply wells WSW-1, WSW-3, WSW-4, and WSW-5. No compounds were detected. The
report also documented the placement of O-SOX in monitoring well MW-1. ATC recommended sampling
monitoring well MW-1 in January 2017.
On May 11, 2017, ATC submitted a Groundwater Sampling Letter Report documenting the sampling of
monitoring well MW-1 and water supply well WSW-1. Monitoring well MW-1 indicated MTBE and
naphthalene above the 2L Standards. No compounds were detected in water supply well WSW-1. ATC
recommended placement of O-SOX in monitoring well MW-1 with sampling of MW-1 three months later.
On May 14, 2018, ATC submitted a Groundwater Sampling Letter Report documenting the sampling of
water supply wells WSW-1, WSW-3, WSW-4, and WSW-5. Monitoring well MW-1 indicated MTBE above
the 2L Standard. No compounds were detected in water supply wells WSW-1, WSW-3, WSW-4, or WSW-
5. ATC recommended placement of O-SOX in monitoring well MW-1 with sampling of MW-1 three months
later.
On June 25, 2019, ATC submitted a Groundwater Monitoring Report documenting the sampling of water
supply wells WSW-1, WSW-3, WSW-4, and WSW-5. Monitoring well MW-1 indicated MTBE above the 2L

Standard. No compounds were detected in the water supply wells. ATC recommended installing and
sampling two additional monitoring wells at the site to delineate groundwater impact and to determine
groundwater flow direction.
On February 7, 2020, ATC submitted a Groundwater Monitoring Report documenting the installation of
monitoring wells MW-2 and MW-3 and the sampling of monitoring wells MW-1, MW-2, and MW-3 and water
supply wells WSW-1, WSW-3, WSW-4, and WSW-5. Monitoring well MW-1 indicated MTBE above the
corresponding 2L Standard. Monitoring wells MW-2 and MW-3 did not contain compounds exceeding the
2L Standards. No compounds were detected in the water supply wells.
On January 28, 2021, ATC submitted a Groundwater Monitoring Report documenting the sampling of
monitoring wells MW-1, MW-2, and MW-3 and water supply wells WSW-1, WSW-3, and WSW-5. Monitoring
well MW-1 indicated MTBE above the 2L Standard. No compounds were detected in water supply wells
WSW-1, WSW-3, or WSW-5. ATC recommended groundwater and water supply well sampling on an
annual basis.
On February 19, 2022, ATC submitted a Groundwater Monitoring Report documenting the sampling of
monitoring wells MW-1, MW-2, and MW-3 and water supply well WSW-1. Monitoring well MW-1 indicated
MTBE above the 2L Standard. No compounds were detected in water supply well WSW-1. ATC
recommended groundwater and water supply well sampling on an annual basis.
This report documents the groundwater and water supply well sampling event on February 14, 2023.
4.0 GROUNDWATER ASSESSMENT ACTIVITIES
4.1 GROUNDWATER LEVELS
On February 14, 2023, groundwater level measurements and samples were collected from monitoring wells
MW-1, MW-2, and MW-3. Monitoring well construction details and groundwater elevation data are
summarized in Table 3. Light non-aqueous phase liquid (LNAPL) was not observed in any of the monitoring
wells that were gauged. Depth to groundwater measurements ranged from 26.85 feet bgs in MW-2 to 32.11
feet bgs in MW-3. A groundwater elevation contour map generated using the February 2023 groundwater
elevation data is included as Figure 4. This figure indicates that the direction of groundwater flow at the
site is to the east, which is consistent with historical data. Field notes from the sampling event are included
in Appendix B.
4.2 GROUNDWATER QUALITY
On February 14, 2023, groundwater samples were collected from monitoring wells MW-1, MW-2, and MW-
3. Samples were collected in laboratory-approved containers, packed in ice and delivered under chain of
custody protocol to Pace Analytical in Mount Juliet, Tennessee (Pace) for analyses. Groundwater samples
were analyzed for volatile organic compounds (VOCs) by Standard Method 6200B.
The analytical results were compared to the 2L Standards and GCLs. MTBE was detected in MW-1 at 53.1
µg/l, exceeding the 2L Standard but below the respective GCL. No compounds were detected above the
2L Standard in wells MW-2 and MW-3. A comparison of the current analytical results (February 14, 2023)
to the results of the previous sampling event (January 12, 2022) indicates a slight increase in MTBE
concentrations in MW-1. A review of all historical data exhibits an overall decreasing trend for MTBE
concentrations in MW-1. Monitoring well analytical results are included in Table 4. Measured field
parameters are summarized in Table 5. A MTBE isoconcentration contour map is included as Figure 5.
The laboratory analytical report and chain-of-custody forms are included in Appendix C.
As part of ATC’s quality assurance and quality control (QA and QC) measures, a data review of the
laboratory report was conducted. During the review, it was concluded that samples were received by the
laboratory in acceptable condition and properly preserved. Samples were analyzed within the correct

holding times, and the chain of custody was properly completed by all parties. ATC collected a field blank
and laboratory analysis of the field blank (FB-1) yielded concentrations of acetone, 2-butanone, and
methylene chloride which exceeded the laboratory detection limit but did not exceed the laboratory reported
detection limit. These compounds are typically associated with laboratory contaminants and are not likely
associated with the release. The laboratory QC report shows an internal blank was analyzed for each
method that was used for the original samples. The laboratory fortified blank (LFB) and laboratory control
sample (LCS) data results were also provided for each analytical method. Refer to the laboratory report in
Appendix C for details regarding QA results for the laboratory samples and any noted exceptions. Overall,
ATC concludes that the laboratory results are usable and any exceptions do not significantly impact the
conclusions presented in this report.
4.3 WATER SUPPLY WELLS
Eight active water supply wells are located within 1,000 feet of the site. Municipal water is not available in
the vicinity of the site. Table 1 presents the water supply well ownership information and Figure 3 depicts
the water supply well locations.
ATC collected a groundwater sample from onsite water supply well WSW-1 during the February 14, 2023,
sampling event. The sample was collected in laboratory-approved containers, packed in ice and delivered
under chain of custody protocol to Pace for analyses. The sample was analyzed for VOCs by Standard
Method 6200B. The sample yielded a concentration of TCE, which exceeded the laboratory method
detection limit but did not exceed the laboratory reported detection limit or the 2L Standard. Concentrations
of TCE have not been historically detected in WSW-1. The water supply well analytical results are
summarized in Table 2. The QA and QC measures and conditions applicable to the monitoring well
sampling also apply to the water supply well sampling. The laboratory analytical report and chain-of-custody
forms are included in Appendix C.
5.0 CONCLUSIONS AND RECOMMENDATIONS
On behalf of the Trust Fund Branch of the UST Section, ATC completed a Groundwater Monitoring Report
documenting the February 2023 groundwater sampling activities at the Jeff’s Grocery property located at
2996 NC Highway 268 in Pinnacle, Stokes County, North Carolina. Groundwater samples were collected
from monitoring wells MW-1, MW-2, and MW-3 and water supply well WSW-1.
Results from the monitoring event indicated MTBE above 2L Standard but below the GCL in monitoring
well MW-1. No compounds were detected above the 2L Standard in wells MW-2 and MW-3. Water supply
well sample WSW-1 contained a concentration of TCE exceeding the laboratory detection limit, but below
the laboratory reporting limit and 2L Standard.
The site is not a candidate for closure due to the presence of potable water supply wells within 1,000 feet
of the site, most notably WSW-1 located on the source property. ATC recommends groundwater and water

All sample aliquots were received at the correct temperature, in the proper containers, with the
appropriate preservatives, and within method specified holding times, unless qualified or notated within
the report. Where applicable, all MDL (LOD) and RDL (LOQ) values reported for environmental samples
have been corrected for the dilution factor used in the analysis. All Method and Batch Quality Control
are within established criteria except where addressed in this case narrative, a non-conformance form

GLOSSARY OF TERMS
Guide to Reading and Understanding Your Laboratory Report
The information below is designed to better explain the various terms used in your report of analytical results from the Laboratory. This is not
intended as a comprehensive explanation, and if you have additional questions please contact your project representative.
Results Disclaimer - Information that may be provided by the customer, and contained within this report, include Permit Limits, Project Name,
Sample ID, Sample Matrix, Sample Preservation, Field Blanks, Field Spikes, Field Duplicates, On-Site Data, Sampling Collection Dates/Times, and
Sampling Location. Results relate to the accuracy of this information provided, and as the samples are received.
Abbreviations and Definitions
MDL Method Detection Limit.
RDL Reported Detection Limit.
Rec.Recovery.
RPD Relative Percent Difference.
SDG Sample Delivery Group.
(S)
Surrogate (Surrogate Standard) - Analytes added to every blank, sample, Laboratory Control Sample/Duplicate and
Matrix Spike/Duplicate; used to evaluate analytical efficiency by measuring recovery. Surrogates are not expected to be
detected in all environmental media.
U Not detected at the Reporting Limit (or MDL where applicable).
Analyte The name of the particular compound or analysis performed. Some Analyses and Methods will have multiple analytes
reported.
Dilution
If the sample matrix contains an interfering material, the sample preparation volume or weight values differ from the
standard, or if concentrations of analytes in the sample are higher than the highest limit of concentration that the
laboratory can accurately report, the sample may be diluted for analysis. If a value different than 1 is used in this field, the
result reported has already been corrected for this factor.
Limits
These are the target % recovery ranges or % difference value that the laboratory has historically determined as normal
for the method and analyte being reported. Successful QC Sample analysis will target all analytes recovered or
duplicated within these ranges.
Qualifier
This column provides a letter and/or number designation that corresponds to additional information concerning the result
reported. If a Qualifier is present, a definition per Qualifier is provided within the Glossary and Definitions page and
potentially a discussion of possible implications of the Qualifier in the Case Narrative if applicable.
Result
The actual analytical final result (corrected for any sample specific characteristics) reported for your sample. If there was
no measurable result returned for a specific analyte, the result in this column may state “ND” (Not Detected) or “BDL”
(Below Detectable Levels). The information in the results column should always be accompanied by either an MDL
(Method Detection Limit) or RDL (Reporting Detection Limit) that defines the lowest value that the laboratory could detect
or report for this analyte.
Uncertainty
(Radiochemistry)Confidence level of 2 sigma.
Case Narrative (Cn)
A brief discussion about the included sample results, including a discussion of any non-conformances to protocol
observed either at sample receipt by the laboratory from the field or during the analytical process. If present, there will
be a section in the Case Narrative to discuss the meaning of any data qualifiers used in the report.
Quality Control
Summary (Qc)
This section of the report includes the results of the laboratory quality control analyses required by procedure or
analytical methods to assist in evaluating the validity of the results reported for your samples. These analyses are not
being performed on your samples typically, but on laboratory generated material.
Sample Chain of
Custody (Sc)
This is the document created in the field when your samples were initially collected. This is used to verify the time and
date of collection, the person collecting the samples, and the analyses that the laboratory is requested to perform. This
chain of custody also documents all persons (excluding commercial shippers) that have had control or possession of the
samples from the time of collection until delivery to the laboratory for analysis.
Sample Results (Sr)
This section of your report will provide the results of all testing performed on your samples. These results are provided
by sample ID and are separated by the analyses performed on each sample. The header line of each analysis section for
each sample will provide the name and method number for the analysis reported.
Sample Summary (Ss)This section of the Analytical Report defines the specific analyses performed for each sample ID, including the dates and
times of preparation and/or analysis.
Qualifier Description
J The identification of the analyte is acceptable; the reported value is an estimate.
J3 The associated batch QC was outside the established quality control range for precision.
J4 The associated batch QC was outside the established quality control range for accuracy.
