2.0 EXECUTIVE SUMMARY
ATC Associates of North Carolina, P.C. (ATC) has completed a Phase I Limited Site Assessment
(LSA) for the Sheltontown Grocery site. This report documents soil sampling, monitoring well
installation activities, groundwater sampling, and a receptor survey performed between July 2014
and August 2014. The LSA was performed based on a June 24, 2014 Task Authorization (TA-1) by
the Trust Fund Branch of the Underground Storage Tank (UST) Section, Division of Waste
Management, North Carolina Department of Environment and Natural Resources (NCDENR). The
work is being performed as part of the State Lead Contract #N14004i between NCDENR and ATC.
The former Sheltontown Grocery is located at 3170 Westfield Road in Mt. Airy, Surry County,
North Carolina. The site is developed with one single-story building that is currently used as a
convenience store and retail petroleum facility. Twenty-four water supply wells (SW-1 through
SW-24) are also located within 1,000 feet of the site. One potable water supply well is located on-
site. Municipal water is not available to the site and vicinity.
On July 10, 2014, ATC personnel mobilized to the site to collect soil samples from the source area
and to install monitoring well MW-1. On August 14, 2014, a groundwater sample was collected
from MW-1. Water supply well samples SW-1, SW-3, SW-16, SW-17, and SW-22 were also
collected.
Based on topography at the site, it appears groundwater flow is to the southwest toward the
nearest surface water body. Soil, groundwater, and water supply well analytical results do not
indicate petroleum concentrations above laboratory detection limits. Based on the results of this
LSA, the site is a candidate for site closure.

3.0 SITE DESCRIPTION
3.1 Site Characterization
The site covers an approximate 0.851-acre parcel of land and is currently developed with Jacks
Grocery which is a convenience store and retail gasoline facility. It should be noted the property
address was 3120 Westfield Road at the time of the incident release; however, the site address is
currently 3170 Westfield Road. The former UST system associated with Incident #14744 consisted
of two 4,000-gallon gasoline USTs and one 2,000-gallon gasoline UST. The UST system was
installed in May 1976 and was removed from the site in June 1994. Currently, on-site is a
compartmentalized UST with a 4,000-gallon compartment and a 6,000-gallon compartment, both
for gasoline storage. Former and current UST system owner and operator information is included as
Table 1.
The site is bounded to the north, southeast, and west by residences and to the east and southwest by
undeveloped vacant lots. Twenty four water supply wells (SW-1 through SW-24) are located
within 1,000 feet of the site. Water supply well ownership information is included in Table 2. A
site topographic map is included as Figure 1 and a site map depicting pertinent site features is
included as Figure 2. A Potential Receptor Map showing water supply well locations is included
as Figure 3. Current site photographs are included in Appendix A.
3.2 Site Background
On June 24, 1994, the UST system associated with Incident # 14744 was removed from the site.
The system consisted of two 4,000-gallon gasoline USTs and one 2,000-gallon gasoline USTs.
Following system removal, soil samples were collected from beneath the tanks, product lines,
and dispensers. Laboratory results for the soil samples indicated total petroleum hydrocarbon
(TPH) concentrations ranging from 472 milligrams per kilogram (mg/kg) to 26,161 mg/kg
beneath the USTs and TPH concentrations ranging 15,900 mg/kg to 20,154 mg/kg beneath the
product lines and dispensers. A Site Investigation Report for Permanent Closure of UST & Site
Check Report documenting the UST closure was submitted to NCDENR in July 1994. It should

be noted ATC did not have access to this report and acquired this information from the site
background section in the Receptor Survey and Water Supply Well Sampling Report submitted
on October 30, 2006.
Based on the October 2006 receptor survey, 24 water supply wells were identified within 1,000
feet of the site. Samples were collected from seven water supply wells (SW-1, SW-2, SW-3,
SW-4, SW-5, SW-16, and SW-19). Based on laboratory results, SW-3 indicated petroleum
compounds above laboratory detection limits but below 2L Standards. Water supply well SW-4
indicated benzene, methyl tert-butyl ether (MTBE) and naphthalene above 2L Standards. It
should be noted the sample was collected from the influent portion of the point-of-entry system
for SW-4. Water supply well SW-4 formerly supplied water to the residence at 3156 Westfield
Road; however, based on an interview with Ms. Sheila Cox, the property owner of 3156
Westfield Road, water supply well SW-1 became the source of water for the residence in August
2009.
4.0 RISK CHARACTERIZATION
In accordance with 15A NCAC 2L .0115, a completed Limited Site Assessment Risk
Classification and Land Use Form provided by the NCDENR is included in Appendix B.
Answers and explanations included on the form are based on information obtained by ATC
during the LSA activities. Based on information collected during this LSA, the site appears to
meet the criteria for “high” risk classification.
5.0 RECEPTOR INFORMATION
5.1 Water Supply Wells
ATC performed a survey to identify water supply wells within 1,000 feet of the site on July 10,
2014. The survey was performed by visually observing for the presence of water supply wells
and when possible, contacting area residents to confirm water usage. A water supply well
questionnaire form was distributed to properties within 500 feet of the site. Based on the

returned water supply well information forms and the visual reconnaissance, there are twenty-
four water supply wells within 1,000 feet of the site, twenty-two which are used for drinking, one
which is inactive, and one with an unknown use. The nearest active water supply well is located
on-site approximately 35 feet southeast of the source area. Water supply well information is
included in Table 2. A Potential Receptor Map is included as Figure 3. The returned Water
Supply Well Information forms are included in Appendix C.
5.2 Public Water Supplies
ATC contacted the City of Mt. Airy Utility Department and spoke with property owners in the area
to identify public water sources in the area. Municipal water is not provided to the site or
surrounding properties.
5.3 Surface Water
ATC reviewed a USGS Quadrangle Topographic Map to identify surface water bodies in the site
vicinity. In addition, ATC conducted a visual reconnaissance for surface water bodies in the
area. Based on this evaluation, the nearest surface water body is an unnamed tributary of Seed
Cane Creek located approximately 330 feet southwest of the site. The surface water body is
shown on Figure 1. This unnamed tributary does not have a classification per the NCDENR
Division of Water Quality; however, Seed Cane Creek is classified as a Class C surface water body.
Therefore, ATC conservatively assumes the unnamed tributary would share the Class C
classification.
5.4 Wellhead Protection Areas
According the NCDENR Division of Water Resources Wellhead Protection Program, there are
no wellhead protection areas in Surry County.
5.5 Deep Aquifers in the Coastal Plain
The Sheltontown Grocery site is located in the Piedmont Physiographic Region; therefore this
section is not applicable.

5.6 Subsurface Structures
Subsurface lines at the site include water (from on-site well) and lines related to a septic system.
Municipal water and sewer lines do not exist in the vicinity. There are also overhead power and
communication lines at the site. According to Surry County Geographical Information Systems
(GIS), structures at the site and on adjacent properties do not appear to extend beneath the
subsurface (i.e. basements).
5.7 Land Use
According to the Surry County Zoning Department, the site is zoned Rural Business and
surrounding properties are zoned Residential General. The nearest residence is located
approximately 165 feet west of the site.
5.8 Property Owners and Occupants
Residences are located to the north, southeast, and west of the site. Undeveloped and vacant lots are
located to the east and southwest of the site. Adjacent property ownership information is
summarized on Table 3. A map depicting adjacent property occupants is included as Figure 3.
6.0 SITE CHARACTERISTICS
6.1 Regional Geology
According to the Geologic Map of North Carolina (1985) produced by Philip Brown, et al, the
referenced site is located within the Piedmont Physiographic Region of North Carolina. More
specifically, the site is underlain by banded gneiss which is interlayered with calc-silicate rock,
metaconglomerate, amphibolite, sillimanite-mica schist, and granitic rock.
6.2 Site Geology
According to the Soil Survey of Surry County, North Carolina, prepared by the United States
Department of Agriculture Soil Conservation Service, the site lies within the Braddock fine
sandy loam complex and the Woolwine-Fairview-Westfield complex. The Braddox complex is
characterized by 8 to 15 percent slopes, is well drained, has a moderately high to high capacity to

transmit water, and originates from old colluvium and/or old alluvium derived from igneous
rocks. The Woolwine-Fairview-Westfield complex is characterized by 15 to 25 percent slopes,
is also well drained, has a low to very low capacity to transmit water, and originates from
saprolite derived from schist and/or gneiss.
As part of the Phase I LSA drilling activities, soils were continuously classified using hand auger
cuttings and drilling cuttings. The lithology of the soil samples was described in accordance
with the Unified Soil Classification System. Soils encountered during drilling activities
consisted primarily of brown, orange, and tan silty clay from the ground surface to bedrock, with
was encountered approximately 12 feet below ground surface (bgs). Boring logs are included as
Appendix D. Field notes collected during assessment activities, including drilling, are included
as Appendix E.
6.3 Regional Hydrogeology
The occurrence and movement of groundwater in the Piedmont is within two separate but
interconnected water-bearing zones. A shallow water-bearing zone occurs within the surficial
soils and a deeper zone occurs within the underlying bedrock. Groundwater in the shallow soil
zone occurs in the interstitial pore spaces between the grains comprising the surficial
soils. Groundwater in this zone is typically unconfined. Groundwater movement is generally
lateral from recharge areas to small streams that serve as localized discharge points. The
occurrence and movement of groundwater in the underlying water-bearing zone within the
crystalline bedrock is controlled by secondary joints, fractures and faults within the bedrock. On
a regional scale, the direction of groundwater flow is typically from uplands to major streams
and groundwater sinks.
6.4 Site Hydrogeology
As part of Phase I LSA activities, monitoring well MW-1 was installed on July 10, 2014, to a
depth of 40 feet bgs. Depth to groundwater in the monitoring well was measured at 8.23 feet bgs
on August 14, 2014. It should be noted what although depth to groundwater was measured at a

depth shallower than bedrock, ATC assumes groundwater originated from the bedrock. Depth to
water was measured using an oil water interface probe. Based on topography, it appears
groundwater flow is to the southwest, towards the unnamed tributary of Sand Cane Creek. A
well construction record is included in Appendix D. Monitoring well construction information is
included as Table 4 and groundwater elevation data is included as Table 5.
7.0 SOIL INVESTIGATION
On July 10, 2014, ATC supervised the advancement of soil boring SS as part of LSA activities.
ATC based the location of the soil boring on an interview with the store personnel which
indicated the location of the former UST system. It should be noted that overhead power lines at
the site caused ATC to place the soil boring and monitoring well to the east of the former UST
system (potentially upgradient) as opposed to the west of the system (potentially downgradient).
Prior to drilling activities, ATC Program Manager Mr. Jeff Corson contacted Ms. Linda Blalock
to confirm she approved of the drilling location.
During drilling, groundwater appeared to be encountered at a depth of approximately 40 feet bgs
and bedrock was encountered at 12 feet bgs. Therefore, soil samples were collected in the
unsaturated zone at a depth of 0-5 feet bgs, 5-10 feet bgs, and 10-12 feet bgs. The samples were
placed in laboratory supplied sample containers and shipped in an ice filled cooler under chain of
custody to Contest Analytical Laboratory (Contest) in East Longmeadow, Massachusetts.
Samples from the 0-5 feet and 10-12 feet interval were analyzed for volatile organic compounds
(VOCs) by Environmental Protection Agency (EPA) Method 8260B and volatile petroleum
hydrocarbons (VPH) by the Massachusetts Department of Environmental Protection (MADEP)
method. The sample collected from the 5-10 feet interval was analyzed for MADEP VPH.
The results of the soil analyses indicated acetone and methylene chloride above laboratory
detection limit but below Soil-to-Groundwater Maximum Soil Contaminant Concentrations
(MSCCs). Acetone and methylene chloride are typically considered to be laboratory
contaminants and most likely did not originate from the site. Historically at the site, the
maximum detection of TPH-GRO was 26,161 mg/kg from a sample collected from the beneath

the former USTs. Soil analytical results are summarized in Table 6. Laboratory analytical
reports are included in Appendix F. Historical soil sample locations are shown of Figure 4.
As part of ATC’s quality assurance and quality control (QA and QC) measures, a data review of
the laboratory report was conducted. During the review, it was concluded that samples were
received by the laboratory in acceptable condition and properly preserved. The samples were
analyzed within the correct holding times, and the chain of custody was properly completed by
all parties. The laboratory QC report shows an internal blank was analyzed for each method that
was used for the original samples. The laboratory fortified blank (LFB) and laboratory control
sample (LCS) data results were also provided for each analytical method. Refer to the laboratory
reports in Appendix F for details regarding QA results for the laboratory samples and any noted
exceptions. ATC concludes that the laboratory results are usable and any exceptions do not
significantly impact the conclusions presented in this report.
8.0 GROUNDWATER CHARACTERIZATION
8.1 Groundwater Quality
Initially, ATC intended to install MW-1 in the soil boring location SS; however, due to the close
proximity of the boring to on-site water supply well SW-1, ATC moved the location of MW-1 in
order to reduce the chance of damaging SW-1 with the force of the air drilling rig. The
monitoring well was installed by North Carolina certified well driller Carolina Soil
Investigations and is located northeast of the former UST pit.
On August 14, 2014, a groundwater sample was collected from monitoring well MW-1, packed in
ice and delivered under chain-of-custody protocol to Contest for analyses. The groundwater
sample was analyzed for VOCs by EPA Method 6200B, MADEP VPH, and lead by method
6020A.
The laboratory analytical results indicated the presence of lead above 2L Standards. No other
compounds were detected above laboratory detection limit. Because no other petroleum
compounds were detected in the groundwater sample, ATC believes the lead detected is naturally

occurring. Groundwater analytical data is summarized in Table 7. The laboratory analytical
report and chain-of-custody forms are included in Appendix F. The QA/QC measures and
conditions applicable to the soil sampling also applies to the groundwater sampling.
8.2 Water Supply Well Sampling
Twenty-four water supply wells (SW-1 through SW-24) are located within 1,000 feet of the site,
twenty-two of which are used for drinking purposes (SW-1 through SW-3, SW-5 through SW-
10, and SW-12 through SW-24), one of which is inactive (SW-4), and one of which the use is
unknown (SW-11B). Historically at the site, the former water supply well influent sample SW-4
contained benzene at 177 micrograms/liter (µg/L), MTBE 527 µg/L, and naphthalene at 23.8
µg/L. It should be noted this well formerly supplied water to 3156 Westfield Road; however,
based on and interview with Ms. Sheila Cox, property owner of 3156 Westfield Road, SW-1 has
been supplying this property water since August 2009.
As part of receptor survey activities, ATC sent water supply well information forms to all
properties whose boundaries were located within 500 feet of the source area. In addition, ATC
sent water supply well access forms to these properties. A total of 17 access agreements were
sent to property owners. Of the 17 agreements, ATC received responses from three property
owners. Owners of water supply wells SW-1 and SW-3 (same property owner), SW-16, and
SW-17 granted access for well sampling. During sampling activities, ATC was able to acquire a
verbal agreement from the property owner of SW-22. Water supply well access agreements are
included in Appendix E.
ATC sampled water supply wells SW-1, SW-3, SW-16, SW-17, and SW-22 on August 14, 2014.
Samples were collected in laboratory-approved containers, packed in ice and delivered under
chain-of-custody protocol to Contest to analyze for VOCs by EPA Method 6200B. Laboratory
results for did not indicate any compound concentrations above laboratory detection limit. Water
supply well analytical results are included in Table 8. Laboratory results are included in
Appendix F. The QA/QC measures and conditions applicable to the soil sampling also applies
to the water supply well sampling.

9.0 CONCLUSIONS AND RECOMMENDATIONS
On behalf of the Trust Fund Branch of the UST Section, ATC has completed a Phase I LSA
documenting assessment activities performed in July 2014 and August 2014 for the Sheltontown
Grocery site located at 3170 Westfield Road (formerly 3120 Westfield Road) in Mt. Airy, Surry
County, North Carolina. Results from LSA did not indicate VOC impact to soil, groundwater,
water supply well samples collected. In addition, ATC performed a receptor survey and
identified twenty-four water supply wells within 1,000 feet of the site, twenty-two of which are
used for drinking purposes, one which is inactive, and one of which the use is unknown. The site
meets the criteria for “high” risk classification.
Soil analytical results indicated acetone and methylene chloride at concentrations above
laboratory detection limits but below Soil-to-Groundwater MSCCs. Acetone and methylene
chloride are typically considered to be laboratory contaminants and most likely do not originate
from the site. Groundwater analytical results do not indicate concentrations exceeding
laboratory analytical results with the exception of lead, which was detected above 2L Standard.
Because no other compounds were detected in the groundwater sample, ATC assumes the lead is
naturally occurring. Finally, water supply well analytical results did not indicate compound
concentrations exceeding laboratory detection limits. Based on the results of this LSA, the site is
a candidate for site closure.
