EXECUTIVE SUMMARY Jerry’s Place, the subject property located at 983 US Highway 21, is situated on a 1.58-acre parcel (Surry County Parcel No. 495301186476) in State Road, North Carolina. The facility reportedly operated four 4,000-gallon gasoline underground storage tanks (USTs) which were removed from the site in 1992. At the direction of the NCDEQ, Terracon mobilized to the site on April 29, 2021 to sample five monitoring wells (MW-2, MW-3/RW-3, MW-4/RW-4, MW-5, and RW-5) and one water supply well (WSW-5) for analysis by EPA Method 6200. Based on information obtained during the completion of these assessment activities, Terracon provides the following findings and conclusions: ◼ Several petroleum constituents were detected at concentrations exceeding the established 15A NCAC 2L Groundwater Quality Standards (2L Standards) but below the established NCDEQ Gross Contamination Limits (GCLs) in groundwater samples collected from wells MW-2/RW-2, MW-3/RW-3, and RW-5. ◼ Multiple petroleum constituents were also detected in groundwater samples from wells MW-2/RW-2, MW-3/RW-3, MW-4/RW-4 and RW-5 at concentrations exceeding laboratory reporting limits but below the 2L Standards. ◼ Target compounds were not detected at concentrations exceeding the laboratory method Reporting Detection Limits (RDLs) in samples collected from wells MW-5 and WSW-5. ◼ Measurable thicknesses of light non-aqueous phase liquids (LNAPL) were not observed in the monitoring wells gauged. ◼ Terracon replaced the sorbent socks in wells MW-2, MW-3/RW-3 and RW-5 with oxygen releasing substrate ORS sleeves (O-Sox) and will return to the site in 3 months to replace the O-Sox again. Based on the data presented in this Annual Monitoring Report, Terracon recommends the following activities be completed at the site: ◼ Replace O-Sox in on-site monitoring wells on a quarterly basis. ◼ Sample on-site wells at approximate 6-months intervals to evaluate O-Sox performance and groundwater conditions over time. ◼ Update receptor survey. ◼ Collect geochemical and geophysical data to model the environmental transport and fate of residual petroleum contamination over time to project future impacts and risks. C. SITE HISTORY AND CHARACTERIZATION This Annual Monitoring Report was prepared in response to a request by Mr. Herbert Berger, P.G., Incident Manager with the NCDEQ Division of Waste Management, Trust Fund Group. The report was prepared in general accordance with 15A NCAC 2L .0115 regulations and the NCDEQ UST Section Corrective Action Guidelines dated January 19, 2021, under the supervision of Mr. Mark I. Miller, a North Carolina registered licensed geologist (North Carolina License No. 1715). The purpose of this Annual Monitoring Report is to present the current status of the groundwater quality at the Jerrys Place site located at 983 US Highway 21 in State Road, Surry County, North Carolina (Exhibit 1). Terracon completed this report to summarize the findings of groundwater sampling activities performed on April 29, 2021. C.1 Site Setting Jerry’s Place, the subject property located at 983 US Highway 21, is situated on a 1.58-acre parcel (Surry County Parcel No. 495301186476) in State Road, North Carolina (Exhibit 1). The facility reportedly operated four 4,000-gallon gasoline USTs which were removed from the site in 1992. The site is currently occupied by commercial businesses (Exhibit 2). C.2 Release Summary According to prior reports, four 4,000-gallon gasoline USTs were removed from the site in 1992. During closure activities, LNAPL was observed flowing into a test pit that was excavated to a depth of approximately 19 feet below ground surface (bgs). Approximately 1,029 tons of petroleum contaminated soil was subsequently excavated during the UST closure activities. Annual Monitoring Report Jerry’s Place - Incident No. 9547 ■ State Road, NC May 19, 2021 ■ Terracon Project No. 75207118 Responsive ■ Resourceful ■ Reliable 4 C.3 Previous Investigations According to information included in the incident folder, four 4,000-gallon gasoline USTs were removed from the site in 1992. Eight monitoring wells (MW-1 through MW-8) and two recovery wells (RW-5 and RW-6) were installed between 1993 and 1994. On-site monitoring wells were sampled in 1993, 2009, 2010, 2011, 2014, 2015, 2018, and 2019. The contaminant concentrations appear to have been decreasing since 1993. A previous consultant conducted a free product/vapor recovery event on July 29, 1996. A mobile multiphase extraction (MMPE) event was later conducted in 2001. According to a Groundwater Monitoring Report completed by a previous consultant and dated January 7, 2020, methyl tert-butyl ether (MTBE) has been detected in the on-site water supply well (WSW-5) since 2009 at a concentration below its 2L Standard. C.4 Water Supply Wells According to a Groundwater Monitoring Report prepared by Engineering Consulting Services (ECS) and dated September 16, 2014, 16 water supply wells were identified within 1,000 feet of the site. A copy of the ECS water supply well location map is presented as Exhibit 3, and Table 1 presents information on the water supply wells located within approximately 500 feet of the site. Water supply well WSW-5 is located on the site, approximately 90 feet southwest and cross-gradient from the source area. Groundwater samples from well WSW-5 have not contained detected concentrations of petroleum constituents since 2018. Well WSW-6 is located approximately 350 feet north and upgradient of the source area of the release and has not contained detected concentrations of petroleum constituents since at least 2009. Wells WSW-3 and WSW-4 are located approximately 550 to 600 feet southwest and cross-gradient relative to the source area of the release. The wells previously contained detected concentrations of petroleum products including MTBE; however, petroleum products have not been detected in groundwater samples from the wells since at least 2009. Municipal water is not available to the site and surrounding properties. C.5 Surface Water Based on review of the USGS Topographic Map, the closest body of water is an unnamed intermittent stream located approximately 225 feet east of the site. Surface water intakes are not known to be located within 1,500 feet of the source area of the release. Nearby surface water bodies are shown on Exhibit 1. D. ASSESSMENT INFORMATION D.1 Sampling Activities Terracon mobilized to the site on April 29, 2021 to collect groundwater samples from the site monitoring wells (MW-2/RW-2, MW-3/RW-3, MW-4/RW-4, MW-5 and RW-5) and one water supply well (WSW-5) located on the site. The approximate locations of the wells are depicted on Exhibit 2 and copies of Terracon’s field notes are included in Appendix A. D.3 Groundwater Sampling Activities Prior to sample collection, Terracon gauged the groundwater elevations in the monitoring wells using a decontaminated electronic sounder. The approximate locations of the wells are shown on Exhibit 2 and monitoring well construction details and groundwater elevation measurements are provided in Table 2. Groundwater flow was estimated, based on groundwater elevation data, to be directed to the southeast. Groundwater elevations and groundwater elevation contours are depicted on Exhibit 4. The monitoring wells were purged with a peristaltic pump using low flow sampling techniques (i.e., <200 milliliters per minute) prior to sampling. The following parameters were periodically measured during purging: pH, temperature, oxidation reduction potential (ORP), dissolved oxygen (DO), and conductivity. Purging was considered complete once the field parameters stabilized. The groundwater samples were then collected into laboratory-supplied glassware, labeled and stored in an ice-packed cooler. The samples, along with chain-of-custody documentation, were delivered to Pace National in Mt. Juliet, Tennessee under a standard turnaround time for analysis of volatile organic compounds (VOCs) by EPA Method 6200B including xylenes, MTBE, EDB, and di-isopropyl ether (IPE). Copies of field notes including groundwater parameters are provided in Appendix B. Terracon also collected one field blank sample by collecting de-ionized water used in the equipment decontamination process into a laboratory-supplied bottle. The field blank was submitted to the laboratory along with the groundwater samples for analysis of VOCs using EPA Method 6200B. D.4 Analytical Results Based on the laboratory analytical report, several petroleum constituents were identified at concentrations exceeding their respective 2L Standards in groundwater samples collected from monitoring wells MW-2/RW-2, MW-3/RW-3 and RW-5. ◼ Benzene [510 µg/L], naphthalene [194 µg/L], n-propylbenzene [97.8 µg/L], 1,2,4- trimethylbenzene [639 µg/L], and total xylenes [1716 µg/L] were detected in the groundwater sample from well MW-2/RW-2 at concentrations exceeding their respective 2L Standards but below their respective GCLs. ◼ Naphthalene [207 µg/L], n-propylbenzene [130 µg/L], and 1,2,4-trimethylbenzene [478 µg/L] were detected in the groundwater sample from well MW-3/RW-3 at concentrations exceeding their respective 2L Standards but below their respective GCLs. Annual Monitoring Report Jerry’s Place - Incident No. 9547 ■ State Road, NC May 19, 2021 ■ Terracon Project No. 75207118 Responsive ■ Resourceful ■ Reliable 6 ◼ Naphthalene was detected in the groundwater sample from well RW-5 at a concentration of 58.5 µg/L which is above its 2L Standard but below its GCL. ◼ Multiple petroleum constituents were also detected in groundwater samples from wells MW-2/RW-2, MW-3/RW-3, MW-4/RW-4 and RW-5 at concentrations exceeding their laboratory method RDLs limits but below their 2L Standards. ◼ The field blank did not contain VOCs at concentrations exceeding their laboratory method RDLs. Summaries of the groundwater analytical results are included on Table 2 and Exhibit 5. The historical groundwater analytical results summaries are in Table 4. A copy of the laboratory analytical report is provided in Appendix B. E. CONCLUSIONS AND RECOMMENDATIONS Based on information obtained during the completion of these assessment activities, Terracon provides the following findings and conclusions: ◼ Several petroleum constituents were detected at concentrations exceeding their respective 2L Standards but below their respective GCLs in groundwater samples from wells MW-2/RW-2, MW-3/RW-3, and RW-5. ◼ Multiple petroleum constituents were also detected in groundwater samples from wells MW-2/RW-2, MW-3/RW-3, MW-4/RW-4 and RW-5 at concentrations exceeding their laboratory method RDLs limits but below their 2L Standards. ◼ No target compounds were detected in samples collected from wells MW-5 or WSW-5. ◼ Measurable thicknesses of LNAPL were not observed in the monitoring wells gauged. ◼ Groundwater flow was estimated, based on groundwater elevation data, to be toward the southeast. Based on the data presented in this Annual Monitoring Report, Terracon recommends the following activities be completed at the site: ◼ Replace O-Sox in on-site monitoring wells on a quarterly basis. ◼ Sample on-site wells at approximate 6-months intervals to evaluate O-Sox performance and groundwater conditions over time. ◼ Update receptor survey. ◼ Collect geochemical and geophysical data from the existing monitor well network to provide input parameters for a groundwater model. ◼ Conduct groundwater modelling to assess environmental transport and fate of residual petroleum contamination over time. This is not intended as a comprehensive explanation, and if you have additional questions please contact your project representative. Results Disclaimer - Information that may be provided by the customer, and contained within this report, include Permit Limits, Project Name, Sample ID, Sample Matrix, Sample Preservation, Field Blanks, Field Spikes, Field Duplicates, On-Site Data, Sampling Collection Dates/Times, and Sampling Location. Results relate to the accuracy of this information provided, and as the samples are received. Abbreviations and Definitions MDL Method Detection Limit. ND Not detected at the Reporting Limit (or MDL where applicable). RDL Reported Detection Limit. Rec.Recovery. RPD Relative Percent Difference. SDG Sample Delivery Group. (S) Surrogate (Surrogate Standard) - Analytes added to every blank, sample, Laboratory Control Sample/Duplicate and Matrix Spike/Duplicate; used to evaluate analytical efficiency by measuring recovery. Surrogates are not expected to be detected in all environmental media. U Not detected at the Reporting Limit (or MDL where applicable). Analyte The name of the particular compound or analysis performed. Some Analyses and Methods will have multiple analytes reported. Dilution If the sample matrix contains an interfering material, the sample preparation volume or weight values differ from the standard, or if concentrations of analytes in the sample are higher than the highest limit of concentration that the laboratory can accurately report, the sample may be diluted for analysis. If a value different than 1 is used in this field, the result reported has already been corrected for this factor. Limits These are the target % recovery ranges or % difference value that the laboratory has historically determined as normal for the method and analyte being reported. Successful QC Sample analysis will target all analytes recovered or duplicated within these ranges. Qualifier This column provides a letter and/or number designation that corresponds to additional information concerning the result reported. If a Qualifier is present, a definition per Qualifier is provided within the Glossary and Definitions page and potentially a discussion of possible implications of the Qualifier in the Case Narrative if applicable. Result The actual analytical final result (corrected for any sample specific characteristics) reported for your sample. If there was no measurable result returned for a specific analyte, the result in this column may state “ND” (Not Detected) or “BDL” (Below Detectable Levels). The information in the results column should always be accompanied by either an MDL (Method Detection Limit) or RDL (Reporting Detection Limit) that defines the lowest value that the laboratory could detect or report for this analyte. Uncertainty (Radiochemistry)Confidence level of 2 sigma. Case Narrative (Cn) A brief discussion about the included sample results, including a discussion of any non-conformances to protocol observed either at sample receipt by the laboratory from the field or during the analytical process. If present, there will be a section in the Case Narrative to discuss the meaning of any data qualifiers used in the report. Quality Control Summary (Qc) This section of the report includes the results of the laboratory quality control analyses required by procedure or analytical methods to assist in evaluating the validity of the results reported for your samples. These analyses are not being performed on your samples typically, but on laboratory generated material. Sample Chain of Custody (Sc) This is the document created in the field when your samples were initially collected. This is used to verify the time and date of collection, the person collecting the samples, and the analyses that the laboratory is requested to perform. This chain of custody also documents all persons (excluding commercial shippers) that have had control or possession of the samples from the time of collection until delivery to the laboratory for analysis. Sample Results (Sr) This section of your report will provide the results of all testing performed on your samples. These results are provided by sample ID and are separated by the analyses performed on each sample. The header line of each analysis section for each sample will provide the name and method number for the analysis reported. Sample Summary (Ss)This section of the Analytical Report defines the specific analyses performed for each sample ID, including the dates and times of preparation and/or analysis.
