1. BACKGROUND INFORMATION
The Former Bazemore Grocery site is approximately 1.1 acres in size and is located at 730/732
Mt. Olive Road, in Republican, Bertie County, North Carolina (Figure 1). According to the Bertie
County Online GIS Database, the Parcel Identification Number (PIN) is 5875342404 (Figure 2).
The property is currently developed with an abandoned residential building. The site previously
operated two 2,000-gallon USTs, one 1,500-gallon UST, and one septic tank.
2. FIELD ACTIVITIES
Prior to conducting field activities, ECS contacted the public utility locator service (North
Carolina One-Call Center) and contracted with a private utility locator to mark utilities and the
UST prior to performing excavation activities.
On August 14 and 15, 2017, ECS observed the removal of two 2,000-gallon USTs, one 1,500-
gallon UST, and one septic tank UST by Zebra. ECS observed rust and holes on the two 2,000-
gallon USTs, one 1,500-gallon UST and detected a petroleum odor from the surrounding soils.
The USTs was transported to an approved facility for disposal. The excavation extent measured
approximately thirty (30) feet in length, twenty (20) feet in width, and six (6) feet in depth.
Groundwater was encountered at approximately 6 ft bgs therefore base samples were not
collected. Zebra removed and disposed of approximately 1,937 gallons of contaminated fluid
from the USTs and approximately 98.88 tons of petroleum-impacted soil from the excavation.
Site photographs documenting field activities are included in Appendix A. A copy of Zebras
Certificate of Disposal, Tank Disposal Certificate, and Non-Hazardous Materials Manifest are
included in Appendix B.
ECS collected a total of sixteen (16) soil samples along the sidewalls of the septic tank
excavation (SW-1 through SW-4) and UST excavation (SW-5 through SW-16). The locations of
the soil samples are shown on Figure 3. The soil samples were collected at approximately 4 feet
below the ground surface (ft bgs). ECS screened the soil samples using a Photoionization
Detector (PID) to detect volatile organic vapors. VOCs were detected in soil samples SW-7
through SW-16 ranging from 1.2 (parts per million) ppm to 25.0 ppm. VOCs were not detected
near the eastern edge of excavation, closest to monitoring well MW-20, in soil samples SW-5
and SW-6; therefore, the excavation was not advanced to the east but rather the northwest of
the site.
The soil samples were appropriately packaged and submitted to Pace Analytical Laboratory to
be analyzed for TPH-GRO using EPA Method 5035/8015. ECS maintained proper Chain-of-
Custody (COC) procedures throughout sample collection and transportation process.
ECS was contracted to abandon, reinstall, and collect groundwater samples from monitoring
MW-20 and collect groundwater samples from monitoring wells MW-2 and MWD-7. ECS was
unable to locate monitoring wells MW-2 and MWD-7. Monitoring well MW-20 was located. On
August 15, 2017, ECS measured approximately two (2) feet of free-product in monitoring well
MW-20 and was unable to collect the groundwater sample. Additionally, a sorbent sock was
previously installed in monitoring well MW-20. ECS removed and disposed of the sorbent sock.
No monitoring wells were installed during field activities as no monitoring wells were presumably
destroyed during tank removal, soil exaction, and soil removal tasks.

3. LABORATORY ANALYSIS
Laboratory analysis detected TPH-GRO at a concentration above the North Carolina
Department of Environmental Quality (NCDEQ) UST Section’s Action Level of 50 mg/kg in soil
samples SW-14 and SW-16.
Laboratory analysis detected TPH-GRO at concentrations above the laboratory reporting limit
but below the NCDEQ UST Section’s Action Level of 50 mg/kg in oil samples SW-8, SW-9, SW-
11, and SW-13.
Laboratory analysis did not report TPH-GRO at concentrations above the laboratory reporting
limit in soil samples SW-1 through SW-7, SW-10, SW-12 or SW-15.
A summary of the laboratory analytical results is included in Table 1. The laboratory analytical
report and COC are included in Appendix C.
4. CONCLUSIONS AND RECOMMENDATIONS
Based on the field observation and laboratory analytical results of the soil samples collected
during this assessment, ECS concludes the following:
On August 14 and 15, 2017, ECS observed the removal of two 2,000-gallon USTs, one
1,500-gallon UST, and one septic tank UST by Zebra. ECS observed rust and holes on
the two 2,000-gallon USTs, one 1,500-gallon UST and detected a petroleum odor from
the surrounding soils;
On August 15, 2017, ECS measured approximately two feet of free-product in
monitoring well MW-20;
Laboratory analysis reported TPH-GRO at concentrations above the NCDEQ UST
Section’s Action Level in the soil samples SW-14 and SW-16; therefore, laboratory
analysis of soil samples collected from the three USTs’ basin confirms a release most
likely occurred;
Laboratory analysis reported TPH-GRO at concentrations above its laboratory reporting
limits but below the NCDEQ UST Section’s Action Level of 50 mg/kg in the soil samples
SW-8, SW-9, SW-11, and SW-13;
Laboratory analysis did not report TPH-GRO at concentrations above the laboratory
reporting limit in soil samples SW-1 through SW-7, SW-10, SW-12 or SW-15; and
ECS recommends conducting an Aggressive Fluid Vapor Recovery (AFVR) event on
monitoring well MW-20 to remove existing free-product. Upon completion of the AFVR
event, ECS recommends installing an O-SOX remedial device in monitoring well MW-20.

5. QUALIFICATIONS OF REPORT
The activities and evaluative approaches used in this assessment are consistent with those
normally employed in UST closure assessments of this type. Our evaluation of site conditions
has been based on our understanding of the site project information and the data obtained
during our field activities. Based on the field observation and laboratory analytical
