Roxboro Street, Durham, NC 27704 Incident Number: 5665 Risk Classification: L035E CATLIN Project No. 221073 Dear Ms. Blalock, CATLIN Engineers and Scientists (CATLIN) is pleased to provide you with this letter report documenting field activities performed in general accordance with CATLIN proposal number 221073.02P, prepared February 7, 2023. Services were authorized by the North Carolina Environmental Quality (NCDEQ) Task Authorization (TA) #02, accepted February 8, 2023, and Contract Number N122019D. The approved scope of services for this Task Authorization included: • Sample monitoring well MW-12; • Analyze one groundwater sample per Standard Method (SM) 6200B plus ethylene dibromide (EDB), methyl tert-butyl ether (MTBE) and diisopropyl ether (DIPE); • Photograph the site redevelopment conditions, the approximate historical monitoring well locations, and the proposed monitoring well locations and conditions; and • Prepare a letter report documenting the findings. The Oxford Commons Shopping Center (Oxford Commons) site is located at 3500 N. Roxboro St., Durham, North Carolina (See Figures in Attachment A). Three underground storage tanks (USTs) previously operated at the site. Two 10,000-gallon and one 500-gallon gasoline USTs were removed from the site in 1991. A petroleum release was discovered during tank closure activities. Currently, a Caliber Car Wash facility is being constructed on-site and appears to be largely complete. CATLIN conducted field activities on February 15, 2023 in accordance with the approved TA. Potential hazards were evaluated before beginning field work and documented on an electronic Job Hazard Analysis (JHA) form, provided in Attachment B. On February 15, 2023, CATLIN personnel mobilized to the site to conduct the approved TA #02 field activities. Upon arrival, the well pad for MW-12 was situated at an angle and the surrounding grassy area had been relandscaped. When the cover was removed from the well, CATLIN observed dirt, assorted-size pieces of broken grout, and the well cap/lock placed randomly inside. CATLIN was not able to sample MW-12 based on its condition. Additionally, monitoring well MW-1 was not observed. Based on the approximate former location of MW-1 and the landscaping done in the general area, CATLIN concluded that this well had also been destroyed during site activities. Site photographs showing the destroyed monitoring well MW-12, the approximate former location of MW-1 (assumed destroyed), the approximate former location of monitoring wells MW-10 and MW-10D (formerly abandoned), and the general redevelopment/construction area are included in Attachment C. Recommendations CATLIN recommends abandoning the on-site groundwater monitoring well MW-12 and installing a replacement well. In addition, CATLIN recommends installing additional monitoring wells to replace wells abandoned prior to construction activities.
