Dear Mr. Mark Petermann:
Terracon Consultants, Inc. (Terracon) is pleased to submit this monitoring well and water supply
well abandonment letter report for the above referenced property. This report has been prepared in
general accordance with North Carolina Department of Environmental Quality (NCDEQ),
Underground Storage Tank (UST) Section Assessment Guidelines for UST Releases and
Corrective Action Guidelines for UST Releases,dated January 19, 2021, and Task Authorization
No. 10 approved on December 16, 2022.
The Plain View Grocery site, located at 5515 Plain View Highway in Plainview, North Carolina (site)
(Figure 1), is comprised of 0.74 acres and consists of a commercial rural retail/storage building, a
shed/outbuilding, and a mix of gravel parking and grassy land. The surrounding properties are
primarily developed as residential with some vacant grassy land. A release was discovered in
March 1988 assumed to originate from a suspected release at the Plain View Grocery property.
In November 2022, Terracon oversaw the installation of a municipal water line that now services the
mobile home park located downgradient from the site. Following the installation of the water line,
the downgradient water supply well PW-4 that once serviced the mobile home park was converted
to an irrigation only well.
At the direction of NCDEQ, Terracon oversaw the abandonment of the on-site water supply well
PW-3, water supply well PW-2, and on-site monitoring wells MW-3r, MW-7, MW-8, MW-9, MW-10,
MW-11, MW-12, MW-13, MW-14, MW-15, MW-16, and MW-17. The wells were abandoned on
January 11, 2023 and March 22, 2023, by a North Carolina-licensed well driller (SR&R
Environmental, Inc.) using neat cement and concrete grout. An updated site diagram is provided in
Figure 2. A copy of the well abandonment records and photographs are provided in Appendix A.
