On behalf of the North Carolina Department of the Environmental Quality (NCDEQ) Underground
Storage Tank (UST) Section, Terracon Consultants, Inc. (Terracon) is providing this letter and the
attached agreement for water supply well abandonment. NCDEQ respectfully requests your
cooperation to abandon the water supply well located on the property. The request is related to the
petroleum release from the former UST system located at 5515 Plain View Highway (Former
Plainview Grocery) in Plain View, NC.
NCDEQ would like to eliminate the use or potential for use of groundwater in close proximity to the
release site as a measure to ensure public health by abandoning the water supply well located at
Country Manor Lane Property.Activities related to the abandoning the water supply well will be
borne by the NC State Trust Fund.
Please fill out the attached water supply well abandonment agreement and return it to Terracon at
your earliest convenience. Please email the attached form to [e-mail] or mail it to
2401 Brentwood Road Suite 107, Raleigh, NC 27604. Terracon, or the state, will hire a licensed
driller to complete the water supply well abandonment.

Dunn, Sampson County, NC give the NC State Trust Fund permission to abandon the water
supply well located at ____________________________. I understand that the cost of the water
supply well abandonment will be borne by the NC State Trust Fund. This activity will be done
according to the state’s well abandonment guidelines.
