Consultech Environmental, LLC currently holds North Carolina Corporate Geology license number C-0497. ii 1.0 EXECUTIVE SUMMARY The site consists of one parcel of land totaling approximately 3.80 acres recorded as Parcel Number 748302696605 by the Richmond County Tax Assessor's office. The Site currently operates as the Custom Monument Company and is improved with two structures. One approximate 2,268 square foot office building constructed in 1971 is located in the southeastern portion of the site and one approximate 10,000 square foot workshop and warehouse building constructed in 2005 located in the southwest portion of the site. The workshop / warehouse building is currently utilized for grave monument manufacturing and equipment storage. The site originally operated as a gasoline station from 1971 to approximately 1995, Custom Sandblasting Special Trade Centers from approximately 1995 to 2003, and the Custom Monument Company from approximately 2003 until the present. Prior to 1971, the site existed as undeveloped agricultural land since at least 1938. According to a Preliminary Site Assessment report prepared by Solutions-IES and dated January 6, 2006, the NCDOT (via the Project owner) stated the Project formerly operated as a petroleum filling station and USTs that operated at the site were removed approximately twenty (20) years previous (1980s). The USTs were reportedly located to the east of the on-site office building. In addition, a concrete pad identified to the front (south) of the on-site office building appeared to have been associated with a former pump island. During Preliminary Site Assessment activities, eight soil borings were drilled along US Highway 1 to a depth of 8 feet below ground surface. Soil samples were collected from five of the soil borings at depths of 4 to 6 feet below ground surface for analysis of Total Petroleum Hydrocarbons (TPH) Gasoline Range Organics (GRO) and Diesel Range Organics (DRO) via USEPA Method 8015. No TPH GRO was detected in the samples analyzed. TPH-DRO was detected in two of the samples at concentrations of 7.1 and 38 milligrams per kilogram (mg/kg). TPH-DRO was detected in sample 126-3 at a concentration above the Action Level of 10 mg/kg. On November 29, 2005, a UST Form 61 – 24 Hour Release and UST Leak Reporting Form was submitted to the NCDEQ. In 2010 EnviroAssessments (EA) performed a Phase I Limited Site Assessment (LSA) at the Project. On August 31, 2010 EA personnel drilled one boring in the suspected former UST basin to the east of the on-site office building and installed a groundwater monitoring well (MW-1) in the boring. One groundwater sample and two (2) soil samples (S-1-9 and S-1-19) were submitted from MW-1 for laboratory analysis. No VOCs, semi-VOCs, VPH, or EPH were detected in soil samples S-1-9 at nine and 19 feet bgs. However, several petroleum constituents (naphthalene, n-propylbenzene, 1,2,4-trimethylbenzene, total xylenes, and C9-C12 aliphatics) were detected in the groundwater sample from well MW-1 at concentrations above the NCAC 2L Groundwater Standards. In addition, the naphthalene concentration in MW-1 (110 ug/L) exceeded the 2L Standard of 6 ug/L for naphthalene by a factor of ten (10) which automatically required that LSA-Phase II activities be conducted. EA submitted a Phase I Environmental Site Assessment (ESA) Report with results included in a June 8, 2010 report. On November 12, 2010, EA conducted an Phase II LSA at the site which included the installation of three (3) additional groundwater monitoring wells (MW-2, MW-3 and MW-4). Groundwater samples MW-2, MW-3 and MW-4 were collected from their respective monitoring wells and iii submitted for laboratory analysis. Lead was detected in the groundwater sample from well MW- 4 at a concentration of 66 micrograms per liter (ug/L), which exceeded the NCAC 2L Groundwater Standard of 15 ug/L for lead. No additional petroleum constituents were detected in monitoring wells MW-2, MW-3 or MW-4 at concentrations exceeding their respective NCAC 2L Groundwater Standards. As summarized in the report dated January 10, 2011, EA recommended additional soil and groundwater assessment in the area of the UST basin and the area of the former fuel islands and product pipelines. On March 14, 2011, the site was assigned a high risk classification due to the presence of a water supply well used for drinking water located within 1,000 feet of the source area of the confirmed release. On January 18, 2023, Consultech personnel sampled four shallow monitoring wells (MW-1 through MW-4) at the site. The samples were analyzed for volatile organic compounds (VOCs) by Standard Method 6200B, semi volatile organic compounds (SVOCs) by USEPA Method 625.1, EPH by the MADEP method, VPH by the MADEP method, and lead via USEPA Method 6020B. Lead was detected in the groundwater sample collected from well MW-1 at a concentration (36.4 ug/L) exceeding the NCAC 2L groundwater standard (15 ug/L). No other contaminants were detected in the four groundwater monitoring wells at concentrations exceeding the groundwater standards. No contaminants were detected in the groundwater in the wells at concentrations exceeding the NCDEQ Gross Contaminant Levels. Based on the lack of assessment in the former fuel island area, the high risk rank, and the results of the January 2023 sampling event, Consultech recommends additional assessment in the area of the former dispenser islands and product pipelines and an additional groundwater sampling event to confirm the concentration of lead in the groundwater samples. iv TABLE OF CONTENTS SITE INFORMATION ....................................................................................................... The location of the facility is shown on Figure 1 and a site map is shown in Figure 2. The site is zoned commercial and is operated as a manufacturing and retail store for tombstones. The site consists of one parcel of land totaling approximately 3.80 acres recorded as Parcel Number 748302696605 by the Richmond County Tax Assessor's office. The Site currently operates as the Custom Monument Company and is improved with two structures. In addition, the 6 naphthalene concentration in MW-1 (110 ug/L) exceeded the 2L Standard of 6 ug/L for naphthalene by a factor of ten (10) which automatically required that LSA-Phase II activities be conducted. EA submitted a Phase I Environmental Site Assessment (ESA) Report with results included in a June 8, 2010 report. Soil sample locations are shown on Figure 2. Historical groundwater contaminant concentrations are summarized in Table 2, while historical soil contaminant concentrations are contained in Table 3. On November 12, 2010, EA conducted an Phase II LSA at the site which included the installation of three (3) additional groundwater monitoring wells (MW-2, MW-3 and MW-4). Groundwater samples MW-2, MW-3 and MW-4 were collected from their respective monitoring wells and submitted for laboratory analysis. Lead was detected in the groundwater sample from well MW-4 at a concentration of 66 micrograms per liter (ug/L), which exceeded the NCAC 2L Groundwater Standard of 15 ug/L for lead. No additional petroleum constituents were detected in monitoring wells MW-2, MW-3 or MW-4 at concentrations exceeding their respective NCAC 2L Groundwater Standards. As summarized in the report dated January 10, 2011, EA recommended additional soil and groundwater assessment in the area of the UST basin and the area of the former fuel islands and product pipelines. On March 14, 2011, the site was assigned a high risk classification due to the presence of a water supply well used for drinking water located within 1,000 feet of the source area of the confirmed release. 3.0 CURRENT ASSESSMENT ACTIVITIES 3.1 Groundwater Measurements, Sampling and Testing Consultech conducted a groundwater sampling and monitoring event at the John Eric Evans site on January 18, 2023. After opening all wellheads and allowing time for water level equilibration, field personnel measured free-product levels and water levels in each well. The depth to groundwater in the wells was approximately 26 feet below the ground surface. No free product was detected in the wells. This information was used to determine water level elevations and water column thickness as shown in Table 1. Field personnel sampled the four monitoring wells (MW-1 through MW-4) during this event. Prior to purging and sampling, the volume of water in each well was calculated based on the thickness of the water column. Except when a well was purged dry, approximately three well volumes of groundwater were removed from each monitoring well prior to sampling. Groundwater samples were collected from the monitoring wells using disposable polyethylene bailers. The groundwater samples were collected in method-appropriate sampling containers supplied by the laboratory 7 and placed on ice in coolers. All samples were shipped under chain-of-custody to Eurofins Xenco Laboratories, LLC. The samples were analyzed for volatile organic compounds (VOCs) by Standard Method 6200B, semi volatile organic compounds (SVOCs) by USEPA Method 625.1, EPH by the MADEP method, VPH by the MADEP method, and lead via USEPA Method 6010. 4.0 GROUNDWATER SAMPLING RESULTS 4.1 Groundwater Flow Groundwater elevations were determined by gauging the depth-to-water at each monitoring well location. This data was then contoured to create a shallow groundwater potentiometric surface map (Figure 3). The general direction of groundwater flow at the site appears to be to the northwest. 4.2 Laboratory Analysis Results Several VOCs (1,2,4 trimethylbenzene, 1,2,5 trimethylbenzene), VPH compounds, and lead were detected in the groundwater sample collected from well MW-1. No VOCs, SVOCs, EPH compounds, or VPH compounds were detected in the groundwater samples collected from wells MW-2 through MW-4. Historical groundwater analytical results are contained in Table 2. The complete laboratory report is included in Appendix A. 4.3 Extent of Groundwater Contamination No SVOCs, EPH compounds, and VPH compounds were detected in groundwater samples collected from all wells. As shown in Table 2, petroleum hydrocarbon concentrations in groundwater have decreased since the groundwater monitoring event conducted in 2010 during LSA activities. Two VOCs (1,2,4 trimethylbenzene, 1,2,5 trimethylbenzene) and VPH compounds were detected in the sample collected from MW-1, however none of these concentrations exceeded the NCAC 2L Groundwater Quality Standards. Lead was detected above the NCAC 2L Groundwater Quality Standard in the sample collected in MW-1. Lead was not detected in samples collected from wells MW-2, MW-3, and MW-4 at concentrations above the NCAC 2L Groundwater Quality Standard of 15 ug/L. Based on data collected during the January 2023 monitoring event, contaminants are generally isolated to the immediate vicinity of MW-1. Free product was not measured in any site monitoring well. 8 5.0 CONCLUSIONS AND RECOMMENDATIONS Several VOCs (1,2,4 trimethylbenzene, 1,2,5 trimethylbenzene, and VPH compounds) were detected in the groundwater sample collected from well MW-1, however none of these concentrations exceeded the NCAC 2L Groundwater Quality Standards. Lead was detected in the groundwater sample from well MW-1 at a concentration of 36.4 ug/L, above the NCAC 2L groundwater quality standard of 15 ug/L. No VOCs, SVOCs, EPH compounds, or VPH compounds were detected in the groundwater samples collected from all wells above NCAC 2L Groundwater Quality Standards. Based on data collected during the January 2023 monitoring event, petroleum contamination in groundwater appears to generally isolated to the immediate vicinity of well MW-1. Free product was not measured in any site monitoring well.
