The Tank Closure Report is attached. The well has not been sampled. The report states that no wsws were located
within 150'. 1 checked the Mecklenburg County Well Information System shows a well on the property (details

below). Mid Atlantic visited the site again and located the well. They are looking to abandon it to reduce the
site risk to low and file a Notice of Residual Petroleum to achieve closure.

I'm looking at a permit request for abandoning a water supply well at the subject site. According to Laserfiche this
address has a UST incident 48527. Do you know if the on -site water supply well has been sampled due to the UST
release? If so, are those sampling results available? Additionally, what is the current status of the incident?
