NC Engineering Corporation F-1333 4905 Professional Court, Raleigh, North Carolina 27609 (919) 876-9799 April 19, 2022 Mr. Mark Bowles Baker Residential mbowles@bakerresidential.com Report of Observations Jewett Property Apex, North Carolina Our Project Number 121-22-108042 Gentlemen: As requested, a representative of NV5 Engineers and Consultants, Inc. (fka TerraTech Engineers, Inc.) was present at the above referenced site on February 16 and February 22, 2022 to document the potential release of petroleum products, to observe the pumping of suspected petroleum products from a non- commercial and non-regulated above ground storage tank (AST) and an underground storage tank (UST) located adjacent to each other, and to observe the excavation of visibly stained soils from beneath the AST. We were present onsite during the removal of an AST and a UST located at the northeast corner of the existing home at the above referenced property on February 16, 2022. Based on our previous Report of Phase I Environmental Site Assessment dated December 30, 2021, stained soils were observed beneath the AST, and vertical pipes indicating the potential presence of an UST located adjacent to, and to the southeast of, the AST were documented. The AST showed no visible signs of rusting or corrosion of the tank. Upon removal of the UST located adjacent to the AST, we observed the release of suspected petroleum products from the bottom of the tank. We observed the UST that was removed and observed rusting holes in the bottom of the tank. Since tax records show that the residential structure was built in 1943, the AST and UST were likely used to store heating oil. The dimensions of the AST and UST are approximately 38 and 42 inches in diameter, and 60 and 92 inches in length, respectively. The approximate locations of the AST and UST are shown on Figure 1 included in the Appendix. Our representative also observed the pumping of a relatively small volume of suspected heating oil from the AST and UST on February 16, 2022. The suspected heating oil was pumped by Oaks Grading & PES into their storage tank and removed from the site. The tanks were removed from the site by Oaks Grading & PES and transported to their shop to be cleaned and broken down for recycling. Based on the approximate dimensions of the AST and UST, they indicated that the tanks were likely 300 and 550-gallon tanks, respectively. Our consultant, Rapid Environmental Diagnostics, LLC (REDLAB), was present onsite on February 16 and February 22, 2022 to perform testing on potentially contaminated soils removed from beneath the AST, and from the excavation after the UST was removed. The REDLAB results are included in the appendix. We observed and performed onsite testing on the contaminated soils as they were removed from beneath the AST and UST. These soils were immediately placed in dump trucks and transported to Triangle Area Earth Corporation soil farm in Zebulon, NC for disposal. Based on the soil disposal manifests we received from Triangle Area Earth Corporation, which are included in the appendix, approximately 120 cubic yards of contaminated material was removed from the excavation. Contaminated soils beneath the AST and UST extended to a depth of approximately 13 feet below the existing ground surface elevation. Groundwater Page 2 was not encountered during removal of the contaminated soils. We observed and performed onsite testing on the contaminated soils removed from the excavation until non-contaminated samples from each sidewall and the bottom of the excavation were obtained and transported to PACE Analytical for UST closure testing. The test results for the UST closure testing obtained from PACE which are included in the attached Appendix indicated that the no targeted compounds were detected. Plan view and cross-sectional views of the excavation and sampling locations are provided as Figures 3 through 6 in the appendix. Based on field measurements taken while onsite, previously used and/or inactive water supply wells are located approximately 140 feet to the west, and 85 feet to the north of the removed AST and UST. Although the presence of water supply wells was observed, as previously mentioned, no ground water was encountered during the excavation of the contaminated soils. In addition, we were informed by Mr. Mark Bowles with Baker Residential that there are plans to abandon both wells, and that municipal water supply will be utilized when development of the property takes place. The approximate locations of the referenced water supply wells are shown on Figure 2 included in the Appendix. We recommend that all wells be filled in with concrete and closed in accordance with the Wake County Health Department requirements. Concrete should be pumped or tremied into the well to an elevation approximately 5 feet below the finished subgrade. Then, excavate and remove the well casing to a depth 5 feet below finished subgrade and backfill with well compacted ABC stone meeting the NCDOT Standard Specifications for gradation. The crushed stone should be placed in 6-inch loose lifts and compacted with "wacker-packers" or other suitable equipment to at least 95 percent of the standard Proctor (ASTM D-698) maximum dry density. The excavation should be backfilled to return to the originally planned finished subgrade. Based on the results of our testing, and our visual observations during removal of the tanks and contaminated soils, we recommend that a notice of “No Further Action” be provided for the site with no land use restrictions with regards to the removed AST and UST.
