Dear Mr. Patel:

On October 11, 2022, the Division of Waste Management (DWM) received a Phase II Limited Site
Assessment Report, prepared and submitted on your behalf. The report summarizes soil and groundwater
assessment activities conducted at the subject site. The DWM thanks you for this report.

After review of this report, the DWM requests that an additional comprehensive groundwater
sampling event, to include sampling of the on-site water supply well and EMW-2, be completed and a
monitoring report submitted. The DWM requests that the next monitoring report be submitted no later than
February 1, 2023. After review of the next monitoring report, additional assessment activities may be
requested.

If State Trust Fund reimbursement is anticipated, please submit a completed pre-approval form to
