Summary: Two sub-slab vacuum readings were taken from wall mounted access panels
within building 12-15 on each end of the atrium. No vacuum was observed at panel P-14-
1 (Photo 6). None of the VIMS testing apparatus (tubing, vapor barrier wrapped masonry
walls and bottoms of wood columns in atrium, PVC piping, etc.) has been touched since
DEQ’s previous Site Inspection. DEQ personnel inspected active blowers on the roof of
building 12-15 that were accessible, and determined that one of the blowers, extracting
from the vicinity of sub-slab monitoring point P-14-1 was not operating. DEQ personnel
turned the GFI outlet on and off but were unable to restart it.

Summary: Two occupied buildings on the southern half of the property have been
renovated for multi-residential apartments. Based on conversation with construction
worker encountered wile inspecting the vacant building on the northern portion of the
property (resulting from roof work being performed), it is currently in the process of
being converted into residential units of some type (Photos 1-3).
Summary: Soil along both sides of a sidewalk and concrete stairs located on northern side
of building 12-15 has been washed out since prior Site Inspection and mostly covered
portions of sidewalk at lower elevation (Photos 4 & 5). Yellow caution tape has been
installed across the sidewalk to prevent its use, and pieces of coal and other debris are
exposed in both the washed-out soil and in the sidewalls of the intact soil remaining.

Summary: There are maintenance areas, common hallways, residential units, and an
atrium that are at least partially below the surrounding grade in building 12-15 (Photos 6
& 7).
There are soil contact walls around walk-out type basement in vacant building currently
being renovated on northern portion of property (Photos 1-3).

Although the area had caution tape installed to prevent pedestrian traffic, there is a
washed-out soil area north of building 12-15 as mentioned previously (Photos 4 & 5).
This is the general location of the sub-slab/crawl space blower fans that discharge within
the breathing zone which has been noted in prior Site Inspection Reports.
There is a roof mounted fan building 12-15 that has shut down since DEQ’s April/2022
site inspection (Photos 6 & 7), and the risk is unknown for construction workers currently
in the process of remodeling currently vacant building on the northern half of the
property (Photos 1-3).

building 12-15 roof. New roof installation can be seen in the photo.
The Site Inspection was performed at the request of the DEQ project manager to
determine the current condition of the property, and if any additional testing work or test
equipment dismantling has occurred in and adjacent to the atrium portion of building 12-
15 (it has not).
As mentioned previously, at least one blower on building 12-15 was non-operational at
the time of the Site Inspection (there are others on a higher elevation that could not be
checked), there is a washed out area along a concrete sidewalk and stairwell that needs
to be addressed, and there is work beginning on a large masonry vacant building located
on northern portion of property (new roof installed, and contractor meeting scheduled
for later that day).
