IV. Inspector Comments
V. PHOTOGRAPHS
Photo 1: VIMS barrier spread out, and taping begun within future tenant space.
Site inspection was performed to observe sub-slab soil gas sampling that was to be
performed following 24-hour IA sampling that terminated that morning. Upon arrival,
walked through the former mill and new construction apartment building prior to Excel
personnel returning with equipment to perform sub-slab soil gas sampling. Noticed that
the VIMS barrier had been spread out in tenant space where concrete and soil with little
gravel were previously present (Photo 1). I pulled aside VIMS barrier and observed that
these issues were still present (Photos 2 & 3). Sent email to Aaron Long with Excel
Engineering letting him know what I had found prior to VIMS barrier being taped up.
Aaron & Camden from Excel arrived on-site approximately 12:45 and I showed them
material under barrier I had pointed out during previous Site Inspections (there should be
4-inches of washed gravel), and Aaron indicated that he had told superintendent that this
needed to be addressed. Excel personnel exposed sub-slab sampling point in former mill
building and began setting up sampling equipment and shroud for helium leak check. It
was not until after leak check had begun that I realized the Summa Canister was not also
being leak checked which I brought to their attention as the entire sampling train should
be tested. Due to the quick/connect setup of the Summa cannister and sampling
assembly (no valve), it was not possible to test the current setup without the sampling
process beginning. After discussing, it was determined that sub-slab testing would be
performed at a later date once the correct sampling equipment had been obtained.

Photo 4: Materials stored in room off hallway observed shortly after IA sampling completed.
Acetone can is half full and did not have a lid on it.
Photo 5: Did not notice until the helium check had begun that the Summa cannister was not
present below the shroud. With the fittings provided on the Summa cannister (quick
disconnect), there was no way to leak check the entire sampling train. As a result, sub-slab soil
gas sampling was postponed until the correct sampling equipment was obtained.
