On March 4, 2015, on behalf of Carolina Hosiery Mills Inc., Mineral Springs Environmental P.C. (MSE) submitted a North Carolina Brownfields Application for the site. A letter of eligibility was issued for entry into the North Carolina Brownfields Program on May 27, 2015. The site currently has one open environmental incident according to North Carolina Department of Environmental Quality, Division of Waste Management Inactive Sites Section files. • Incident # NONCD0001259 has been assigned by the Inactive Sites Section due to a release of tetrachloroethene and trichloroethene in the 1980s. Significant investigation activities have been ongoing since the 1990s. In 1992, a Phase I Environmental Site Audit (ESA) was performed at the site by Trigon Engineering Consultants, Inc (Trigon). The audit indicated that stained soils were present in and around an empty drum storage area, the chemical storage building, former compressor area and near the heating and air conditioning equipment. These areas are located on the western side of the building. Subsequently, Trigon conducted a Phase II ESA which consisted of collecting soil samples adjacent to the above mentioned areas. Tetrachloroethene and other chlorinated solvents were detected at concentrations in excess of the North Carolina Cleanup Standards. Additional soil and groundwater investigation activities were performed by Environmental Investigations (EI) in 1999. The results revealed the bulk of the groundwater impact is to the west of the building. In 2002 EI installed an air sparge and soil vapor extraction groundwater remediation system at the site. The system operated short time and was cut-off due to large quantities of groundwater entering the system. Numerous assessments have been performed at the site. The assessment activities have been documented in reports and submitted to the appropriate state agency. The work tasks have involved the sampling and closure of drinking water wells and the evaluation of soil vapor intrusion at various residences in the area. Table 1 summarizes the ownership history and operations performed at the site. Historical operations performed at the site consisted of casket manufacturing by Heritage Caskets from 1984 to 1992. Of note the chlorinated solvents identified in 1992 were attributed to the casket operation. American Multimedia International occupied the site from 1992 until 2013 and manufactured compact disk, cassette tapes and DVDs. The site remained vacant from 2013 to 2015. Since 2015 the site has been used for the storage of textile products by Carolina Hosiery Mills Inc. Current the owner has no plans for building expansion and will continue to use the building for storage purposes and office space. MINERAL SPRINGS ENVIRONMENTAL PC Indoor Air Sampling and Receptor Survey Report March 8, 2022 Former AMI Site Page 2 The subject site is comprised of two parcels of land totaling 17.63 acres. One parcel is approximately 7.15 acres and is occupied with an approximately 123,000 square foot building and a small wooden structure located to the west of the building. The large building is currently being used for the storage of yarn products. A large portion of the building is not being used. A section of the building to the east is comprised of office space with carpeted and tile flooring and sheet rock walls. Drop down tiles are located in the office area. The remainder of the building exhibits concrete floor and concrete walls or metal siding. In some areas of the warehouse vinyl tile floors are present. Two electric freight elevators are present in the building. The exterior of the building is comprised of glass, concrete and metal siding. The building is surrounded by an asphalt parking lot with minor landscaped areas. Several transformers and air conditioning/heating units are located adjacent to the building. Metal plates and lids were identified in the parking lot to the rear of the building. These items appear to be associated with ongoing assessment activities and a former remediation system. The small building also located to the rear of the building was locked at the time of the site visit. However, some items such as a fork lift were observed in the building. The building appears to be connected to the city water and sewer system. The second parcel of land totals approximately 10.5 acres and is primarily fallowed. A building with tanks is located on this property and most likely contains remediation equipment. Piping from the remediation system is most likely under ground. Several monitoring wells are also located on the property. 1.2 Initial Indoor Air and Groundwater Sampling Programs On April 1, 2016 MSE collected 10 indoor air quality samples (IA-1 thru IA-10) to determine if concentrations of volatile organic compounds were present in the building. Indoor air samples were collected in accordance with procedures provided in the most recent version of the North Carolina Division of Waste Management Vapor Intrusion Guidance document. The samples were collected in various areas of the building to obtain a thorough representation of the air quality inside the building. The sample locations are shown on Drawing 2. None of the 10 indoor air samples indicated the presence of compounds at concentrations above the IHSB Screening Values. Chloroform, chloromethane dichlorodifluoromethane and trichlorofluoromethane were detected and are associated with refrigerants and also found as a part of the byproducts of the chlorination of drinking water was also detected at concentrations above their respective IHSB Screening Values in Sample 3. Tetrachloroethene (PCE) was detected in sample location IA-1 at a concentration well below IHSB Screening Values. Several other compounds including 2 propanol, 2-butanone, actenone were also detected but at concentrations below the IHSB Screening Values. On April 1, 2016 steps were also taken to collect groundwater samplings from six onsite monitoring wells. The six wells sampled were monitoring wells, MW-1, MW-7, MW-8, MW-9, MW-10 and MW-13 (see Drawing 2). The collected samples will then be transported to ENCO located in Cary, North Carolina for analysis of volatile organic compounds according to EPA SW- 846 Method 8260. The groundwater sample obtained from monitoring well MW-7 did not contain compounds above the laboratory practical quantitation limit (LPQL). Several chlorinated compounds were detected in the groundwater samples obtained from monitoring wells MW-1, MW-8, MW-9, MW-10 and MW-13 at concentrations above the North Carolina Groundwater Quality Standard (NCGWQS). The primary compounds detected were PCE, trichloroethene (TCE) and cis 1,2 dichloroethene (cis 1,2 DCE). PCE was detected at concentrations ranging from a low of 16 micrograms per liter (ug/1) in monitoring well MW-8 to a high of 21,000ug/l in monitoring well MW-13. TCE was detected at concentrations ranging from a low of 3.6ug/I in MINERAL SPRINGS ENVIRONMENTAL PC Indoor Air Sampling and Receptor Survey Report March 8, 2022 Former AMI Site Page 3 monitoring well MW-8 to a high of 2,100ug/I in monitoring well MW-10. Cis 1,2 DCE was detected at concentrations ranging from a low of 3.1 ug/I in monitoring well MW-8 to a high of 730ug/I in monitoring well MW-1. Vinyl chloride was detected in one monitoring well MW-1 at a concentration of 180ug/I which is above the NCGWQS. Based on a thorough review of previously prepared documents mercury impact has not been assessed in the soil or groundwater. Based on the operations previously performed at the site, mercury vapor intrusion does not appear to be a concern. A second indoor air sampling program was requested by Sharon Eckard with the North Carolina Brownfields Program on September 14, 2021 during a site visit. 2.0 INDOOR AIR SAMPLING PROGRAM 2.1 Purpose Mineral Springs Environmental PC (MSE) was contracted to perform an updated indoor air assessment of the potential for solvent vapors to be present in the building. This assessment consisted of collecting air samples for laboratory analysis at various locations within the building using summa canisters. 2.2 INDOOR AND OUTSIDE AIR SAMPLES The scope of work is based on a site visit conducted between Sharon Eckard with the North Carolina Brownfields Program and representatives for the owners of the AMI facility on September 14, 2021. During this visit, the approximate locations of the air sample locations were selected. The primary operations performed inside the facility consisted of the storage of textiles products. A small office area is located in the southeastern corner and occupied by a company that provides truck training activities. On October 31, 2021, MSE collect up to 11 indoor air quality samples (IA2-1 thru IA2-11) to determine if concentrations of volatile organic compounds are present in the building. Three duplicate air samples were collected using a duplicate manifold at the following locations: IA2- 2D, IA2-7D and IA2-10D. In addition to the above, an ambient background air sample was collected outside and on the property approximately 200 feet north of the onsite building. The canister was placed in an area upwind of the facility. The sampling locations are shown on Drawing 3. Pictures are included in Appendix A. Indoor air samples were collected in accordance with procedures provided in the most recent version of the North Carolina Department of Environmental Quality (NCDEQ) Brownfields Program Vapor Intrusion Guidance document dated July 2021. Prior to placing the canisters, a thorough inspection was performed of the interior of the building. During the inspection some common and minor cracks were observed in the floor near the northwestern section of the building and the elevator area. As stated an elevator is present along the northern wall. No floor drains are present in the building. In addition, only textile storage is conducted in the building and does not require chemical storage. A small propane fork lift is used inside the building. The forklift was not operated during the indoor air sampling and was not operational for a period of 24 prior to the test. MINERAL SPRINGS ENVIRONMENTAL PC Indoor Air Sampling and Receptor Survey Report March 8, 2022 Former AMI Site Page 4 Six liter individually certified, by the laboratory, summa canisters were used to collect the samples and were deployed for a period of eight hours with regulators pre-set at the laboratory. All pre and post sampling measurements were recorded and are contained in Appendix B and summarized on Table 2. The starting canister pressures ranged from 27 inches of mercury (IA2- 2) to 30 inches of mercury (IA2-1). The ending canister pressures ranged from 5 inches of mercury (IA2-5) to 11.50 inches of mercury (IA2-10D). Each canister was set at a height of approximately five feet above the concrete floor of the building. During the testing all windows and doors remain shut. Limited access was required to place and retrieve the canisters. During the sample collection activities periodic temperature and pressure gauge readings will be obtained within the building and outside the facility. During the sample collection activities periodic temperature readings were obtained within and outside the building. At 13:30 p.m. the temperature inside the building ranged from 69 to 74 degrees Fahrenheit. At 21:40 p.m. the temperature inside the building ranged from 69 to 73 degrees Fahrenheit. The air conditioner was on during the air sampling activities. The weather conditions were moderate during the test. The outside temperature at 13:30 was 72 degrees Fahrenheit and at 22:05 was 56 degrees Fahrenheit. The outside temperature ranged from 70 degrees Fahrenheit to 56 degrees Fahrenheit. The overall weather conditions were party cloudy with some wind. The building conditions, including HVAC conditions, remained the same within 24 hours prior to and during the sampling. Once the summa canisters had been retrieved, each canister was shipped using an overnight carrier following strict chain -of custody procedures. All canister readings, canister and regulator identification numbers, sampling times were recorded on the field log sheet and/or chain -of - custody. The samples were analyzed for volatile organic compounds plus naphthalene according to EPA TO-15 by Eurofins Air Toxics, LLC located in Folsom, California. Analytical results obtained from the indoor and outside air samples are summarized in Appendix C and summarized in Table 3. All results were reported in micrograms per cubic meter and at the lowest possible detection limit. None of the 14 indoor air samples indicated the presence of compounds at concentrations above the IHSB Non -Residential Screening Values. Freon 11/Trichlorofluromethane was detected in all the air samples at concentrations ranging from 1.0 micrograms per cubic meter (ug/m3) to 1.3ug/m3. Ethanol, acetone and 2 propanol were also detected in all 14 air samples. The ethanol concentrations ranged from 5.1ug/m3 to 190ug/m3. The acetone concentrations ranged from 8.3ug/m3 to 160ug/m3. The 2 propanol concentrations ranged from 3.7ug/m3 to 35ug/m3. Several compounds, including 2-butanone, 4-methyl-2- pentanone, chloroform and tetrachloroethene were randomly detected in various air samples. Three compounds, tetrahydofuran, 1,4 dioxane and carbon tetrachloride were detected in one sample location and include IA2-7, IA2-5 and IA2-7D, respectively. Regarding the outside air sample only four compounds were detected in the sample and include: Freon 11/Trichlorofluromethane, ethanol, acetone and 2 propanol. The concentrations detected for each compound were 1.3ug/m3, 2.7ug/m3, 7.9ug/m3, and 6.8ug/m3, respectively. As stated above none of the 14 indoor air samples did not indicate the presence of compounds at concentrations above the IHSB Non -Residential Screening Values. As such an indoor air risk calculation was not performed. MINERAL SPRINGS ENVIRONMENTAL PC Indoor Air Sampling and Receptor Survey Report March 8, 2022 Former AMI Site Page 5 3 RECEPTOR SURVEY A Brownfields Receptor Survey was completed and is included as Appendix D. During several receptor survey events numerous drinking water wells were identified adjacent to the subject site. These wells were primarily along Hahn Road and were northeast of the site. On June 14, 15 and 18 2001 a total of 11 drinking water wells were abandoned. Prior to the well abandonment activities, all residences along Hahn Road and Tucker Street were connected to the City of Burlington's municipal water supply system. The abandoned well locations are summarized on Table 4 and are shown on Drawing 4. A well previously identified at 939 Hahn Road was located under a concrete driveway and the owner of a well located at 936 Hahn Road refused to allow abandonment of the well. It is our understanding the well is used for lawn irrigation. During a 2005 receptor survey, a drinking water well was identified at 900 Hahn Road. The well is to the north of the subject site. In order to obtain current water well user information, MSE mailed a drinking water survey form to 17 property owners within 1,500 feet of the site. Survey forms were not mailed to those property owners with abandoned wells above. Of the 17 survey forms, a total of nine were mailed back. The returned receptor survey forms and initial letters are contained in Appendix E. No other properties appeared to contain drinking or industrial water wells. Based on the returned survey forms and field survey performed on October 31, 2021, the following can be concluded: • A drinking water well is located at address 939 Hahn Road and is covered by a concrete driveway. As such the well could not be properly abandoned. A current survey was not returned for this address as such this well is considered to be present and not properly abandoned. Of note, the residence is connected to city water. The well is designated Number 10 on Drawing 5 and summarized in Table 5. • According to reports the property owner with address 936 Hahn Road refused to allow abandoned of the site drinking water well in order to use the well for irrigation purposes. As such the well has not been properly abandoned. A current survey was not returned for this address as such this well is considered to be present and not properly abandoned. Of note, the residence is connected to city water. The well is designated Number 11 on Drawing 5 and summarized in Table 5. • A drinking water well is present at 900 Hahn Road. According to the survey, the well "caved -in" approximately 10 years ago (2012) and can not be used. The residence is vacant and currently city water is provided along Hahn Road. The well is designated Number 12 on Drawing 5 and summarized in Table 5. • A large tract of land is located at 1931 Hatchery Road and was previously occupied with a residence that utilized an on site drinking water well. It appears that the residence has been demolished. A current survey was not returned for this address as such this well is considered to be present and not properly abandoned. City water is present along Hatchery Road. The well is designated Number 13 on Drawing 5 and summarized in Table 5. MINERAL SPRINGS ENVIRONMENTAL PC Indoor Air Sampling and Receptor Survey Report March 8, 2022 Former AMI Site Page 6 A drinking water well is still in use at address 2523 Tucker Street, Burlington and is used for bathroom use and not for public consumption. The site is not connected to city water at this time. The well is designated Number 14 on Drawing 5 and summarized in Table 5. Workorder# 2111112 Fifteen 6 Liter Summa Canister (100% Cert Ambient) samples were received on November 03, 2021. The laboratory performed analysis via modified EPA Method TO-15 using GUMS in the full scan mode. Method modifications taken to run these samples are summarized in the table below. Specific project requirements may over -ride the EATL modifications. Requirement TO-15 ATL Modifications Initial Calibration </=30% RSD with 2 </=30% RSD with 4 compounds allowed out to < 40% compounds allowed out RSD to < 40% RSD Blank and standards Zero Air UHP Nitrogen provides a higher purity gas matrix than zero air Receiving Notes The Chain of Custody was missing method assignment in the 'Requested Analyses' checkboxes for the associated samples. EATL proceeded with the analysis as per the original contract or verbal agreement. The Chain of Custody (COC) information for samples IA2-2 and IA2-2D did not match the information on the canister with regard to canister barcode. The sample labeled 6L1902 and 6L2103 on the COC are labeled as 6L2103 and 6L1902 on the canister. The client was notified of the discrepancy and the information on the canister was used to process and report the samples. Analytical Notes All Quality Control Limit exceedances and affected sample results are noted by flags. Each flag is defined at the bottom of this Case Narrative and on each Sample Result Summary page. Target compound non -detects in the samples that are associated with high bias in QC analyses have not been flagged. Definition of Data Oualifyim Flags Eight qualifiers may have been used on the data analysis sheets and indicates as follows: B - Compound present in laboratory blank greater than reporting limit (background subtraction not performed). J - Estimated value. E - Exceeds instrument calibration range. S - Saturated peak. Q - Exceeds quality control limits. U - Compound analyzed for but not detected above the reporting limit, LOD, or MDL value. See data page for project specific U-flag definition. UJ- Non -detected compound associated with low bias in the CCV Page 3 of 55 e u rof i n s Air Toxics N - The identification is based on presumptive evidence. File extensions may have been used on the data analysis sheets and indicates as follows: a -File was requantified b-File was quantified by a second column and detector rl-File was requantified for the purpose of reissue Page 4 of 55 tiff e u rof i n s Air ToxicS, Summary of Detected Compounds MODIFIED EPA METHOD TO-15 GC/MS FULL SCAN Client Sample ID: IA2-1 Lab ID#: 2111112-OIA Compound Rpt.
