Summary: Able to access western apartment building rooftop where active VIMS fan was
operating as designed (Photo 2). Roof top fan/riser on eastern building (Photo 3) will
require a ladder to access and Mr. Marquez had limited time to look for VIMS
components before an appointment. Unable to verify locations of western VIMS ground
level riser although maintenance and electrical rooms below the rooftop riser were
thoroughly checked. I believe eastern building riser was located although there was no
labeling, low vacuum alarm or sample port shown in design drawings (Photos 4 and 5).
This will be confirmed on follow-up site inspection in addition to accessing eastern roof-
top fan and making an additional attempt to locate western ground floor VIMS riser.

Photo 1: Western apartment building, intersection of Foster Ave. and New Bern St.
Following the site visit, Summit Engineering was contacted by email on 2/8/2022 to
request a site meeting and to provide as-built drawings of VIMS. Summit Engineering has
not been involved with the project since 2013, there are no As-Built drawings available,
and the reports they sent in their email response on 2/8/2022 were already in DEQ’s files.
Patrick Lowery with Summit contacted me by phone, and I let him know I was going to
perform a follow-up inspection at the property when Mr. Marquez had an opening, and I
would put together a list of any deficiencies with the system that Mr. Lowery agreed
would be addressed. The follow-up site visit, list of items to be addressed, proposed
timeline, etc. will be contained in a Follow-up Site Inspection Report.

no labeling, sampling port, or low vacuum alarm installed. Design drawings also show active
fan near floor slab. However, active fans are located above the roof on both VIMS risers.
