Please find enclosed a report summarizing the environmental assessment activities in the
area of the former heating oil UST at the above referenced residence. The site activities
consisted of the identification of the tank location, the collection of a soil sample from
immediately adjacent to the UST, a receptor survey completed by a Licensed Geologist
from Paragon, and the supervision of the UST Removal by a Licensed Geologist from
Paragon. The results of our findings are summarized herein.
Paragon Environmental Consultants, Inc. performed the site inspection at 502 S. Main
Street on June 21, 2021. A heating oil tank, estimated to be approximately 1,000 gallons
in capacity, is located on the south side of the house structure. A geologist from Paragon
obtained a soil sample immediately adjacent to the east end of the tank at the subject site.
A hand auger boring was conducted on the east end of the heating oil tank to a depth of
approximately 6 feet below the land surface. The hand auger boring was advanced to
approximately the depth of the bottom of the UST as determined from measuring the depth
to the bottom of the tank through the fill pipe. Suspected soil contamination was observed
from approximately 5 1/2 feet to the termination depth of the hand auger boring.
The soil sample from the hand auger boring, labeled as T 1-6 to represent the tank and the
its depth beneath the land surface, was submitted to Meritech, Inc. for analysis by EPA
Method 3550. This analytical method tests for high boiling -point hydrocarbons such as
diesel fuel, kerosene, and heating oil. The soil sample indicated a total petroleum
hydrocarbon (TPH) concentration of 22,600 milligrams per kilogram (mg/kg), which is in
excess of the NCDEQ action level of 100 mg/kg. A copy of the laboratory analytical report
and chain -of -custody record for this soil sample obtained at the subject site is contained as
Attachment A.

A supply well survey has been conducted within a radius of 150 feet from the release area.
During this reconnaissance, no supply wells were found to be located within this radius.
Public water supplies are available to all of the properties within this radius.
On July 16, 2021 a licensed geologist from Paragon was on -site to supervise the removal
of the 1,000 gallon heating oil UST performed by KBS Earthworks, Inc. at 502 S. Main
Street. A Tank Disposal Manifest documenting the UST removal performed by KBS
Earthworks in contained as Attachment B.
Paragon certifies that the 1,000 gallon heating oil UST has been removed from the subject
site and that no water supply wells exist within a 150-foot radius from the source area.

back on a limited basis in August. UST staff will be in 1-2 days per week on a rotating schedule then. Schedules will
