McAdams Company, Inc. (McAdams) is pleased to present this Facility Decommissioning Report for the JC Express Auto Service facility. This report summarizes activities associated with the implementation of the Facility Decommissioning Plan that was prepared by McAdams, dated April 10, 2019. A copy of the Facility Decommissioning Plan is included in Attachment A. On May 31, 2019, Noble Oil Services (Noble) reportedly recovered approximately 400 gallons of used oil for disposal from the above -ground storage tank and drums (as described in the Plan) for off -site disposal to a permitted facility. On June 21, 2019, Oscar's Service Center purchased the aboveground hydraulic lift, brake lathe, and empty oil storage containers for off -site transport for use at another location. A copy of the waste oil disposal manifest and a copy of the equipment sales receipt is provided in Attachment B. On June 19, 2019, McAdams visited the former JC Auto facility to inspect the space and evaluate decommissioning activities. McAdams observed that items outlined in the Plan had been removed from the premises and that no petroleum and/or hazardous materials were present at the site. Our findings indicate that JC Express Auto did not conduct operations that likely caused or contributed to subsurface contamination afteryour purchase of the property. Photographs from the site visit are included in Attachment C. Based on the findings presented in this report, we conclude that decommissioning activities were conducted in general accordance with the Facility Decommissioning Plan. This plan shall be followed during decommissioning of the JC Express Auto Service located at 401 E Lakewood Avenue, Suite E2. B. JC Auto shall notify McAdams prior to initiating any phase of this plan. C. Execution of this plan will not be deemed complete until reviewed and approved in writing by McAdams. 1.2 WASTE OIL STORAGE VESSELS A. Two waste oil storage vessels are located in the JC Auto facility. This includes a 55-gallon polyethylene drum, and a steel above -ground storage tank (AST) approximately 250 gallons in size. The following steps shall be followed to decommission these containers: 1. Removal and disposal of contents: a. Removal of the contents by a contractor qualified to handle and transport the material. b. Disposal at a facility permitted to accept contents. C. Provide copies of waste manifests from all disposal facilities. 2. Disposal of storage containers. a. With permission from McAdams, the 55-gallon drum may be transported with its contents if placed in an approved overpack. b. The containers shall be damaged in such a way as to ensure that they cannot be reused (i.e. crushing, drilling holes, etc.) C. Provide copies of waste manifests from all disposal facilities. Any spills that occur during this process shall be handled in accordance with applicable laws. a. Recover all released chemicals (as applicable) for disposal at a permitted facility. 1.3 HYDRAULIC LIFT REMOVAL A. One aboveground hydraulic lift was observed in the JC Auto facility. This lift shall be removed prior to surrender of the property. 1. Any spills that occur during this process shall be handled in accordance with state law. a. Recover all released chemicals for disposal at a permitted facility. 1.4 MISCELLANEOUS HAZARDOUS MATERIALS A. All containers of other petroleum substances, hazardous materials, and other items identified by McAdams personnel shall be disposed of at a permitted facility. 1. Products such as lubricants, degreasing agents, paints, solvents, antifreeze, oils, gasoline and diesel fuels shall be disposed of at a permitted facility. a. Removal of the contents by a contractor qualified to handle and transport hazardous chemicals. b. Disposal at a facility authorized to accept contents. C. Provide copies of waste manifests from all disposal facilities. 2. Any spills that occur during this process shall be handled in accordance with state law. a.
