Please provide the information requested below. Distances are measured from the site property boundary unless otherwise indicated by the DEQ Brownfield's Project Manager (PM). Provide occupancy and use information. A residential structure is present on the 3612 Davis Drive parcel which is currently vacant. The structure will be razed during redevelopment. b. Describe the construction of the builidng including materials (e.g. wood frame, block), type and size of openings (e.g. windows, bay doors), and height (number of stories). Structure is two-story, wood frame construction, tile roof, second -story deck, with brick exterior. Two single entrance doors on first floor & one on second floor. One bay window, two double windows & six single windows. Single doorways inside. c. Describe the foundation construction. Include details on type, floor construction, and depth below grade. Structure is slab -on -grade foundation. Floor materials include linoleum, hardwood, tile, and carpet. d. Describe the HVAC system in the building. Include available details on type, equipment location, source of air return, and design considerations (e.g. positive pressure?). No HVAC unit is currently present in structure; air return intake not visible. Likely used oil from former above ground storage tank previously located behind structure. e. Are any subslab ventilation systems or moisture barriers in place? If so, please provide details. Given the age of the structure (approximately 1957) it is very doubtful that a ventilation system is in place and no evidence of a moisture barrier was observed during prior site reconnaissance. If public, please include the water providers name. A well house is located at the northwest corner of the vacant residential structure - 3612 Davis Drive. No well construction details are available. The well is to be abandoned as part of the redevelopment. Please provide the following information regarding water supply wells in the vicinity of the Property. At a minimum, a windshield survey within 1,500 ft of the property boundaries should be completed to determine if water supply or irrigation wells may be present. Information from applicable databases can and should be utilized; however, should not be utilized in lieu of the windshield survey. Rev. 09/2015 Page 3 of 3 ATTACHMENT TO RECEPTOR SURVEY FORM The Overture at Twin Lakes - Cary site is located at 3612 and 3624 Davis Drive in Morrisville/Cary, Wake County, North Carolina (as outlined on the Topographic Map). The site consists of two parcels which are mainly wooded, with a single vacant residential structure on the smaller parcel (3612 Davis Drive). The property totals approximately 12 acres. Contaminants of Concern Environmental assessments conducted at the subject site by Mid -Atlantic have identified petroleum impacted soils around a former heating oil AST. The most recent assessment work from November 2017 included collection of three hand auger samples from stained soils around the supports that formerly held the AST. Laboratory testing results from soil samples indicated total petroleum hydrocarbon gasoline range organics and diesel range organics (TPH GRO and DRO) were present at concentrations above North Carolina Department of Environmental Quality (NCDEQ) Action Limits. The Action Limits for GRO and DRO are 50 parts per million (ppm) and 100 ppm, respectively. Receptor Survey Notes Representatives of Mid -Atlantic visited the site on January 29, 2019 to complete the receptor survey. Mid -Atlantic completed the information requested on the Brownfields Property Receptor Survey form, Revised September 2015 form. As shown on the attached Topographic Map, the subject site is situated on the east side of Davis Drive, approximately 0.1 miles to the north of its intersection with Lake Grove Boulevard. The elevation decreases to the west and to the east of Davis Drive, with ponds present both to the east and west. The elevation decrease on the subject site from Davis Drive to the three ponds on the Cary Town Park Property to the east is approximately 30 feet. Map A shows the locations of residences, housing developments, daycares, and surface water features within a 1,000-foot radius. The addresses of the residences and the daycare are presented on a separate list that accompanies Map A. The Discovery Child Development Center is built as a slab -on -grade and is connected to municipal water and sewer from the Town of Cary. Single-family residences on large parcels are located to the northwest, west, and southwest of the subject site across Davis Drive. Single-family residences and townhomes in housing developments are located to the south, southeast, east, and northeast in the Grace Park, Addison Park, Downing Village, and Twin Lakes communities. Mid -Atlantic was unable to determine the presence of basements in the surrounding residential communities. Given the location and topography of the surrounding land and the presence of the ponds in the area, the presence of basements is not likely. Mid Atlantic Attachment to Brownfields Property Receptor Survey Form February 11, 2019 Overture at Twin Lakes - Cary Page 2 Morrisville/Cary, North Carolina Map B shows the relative locations and direction of the water, sewer, reclaimed water, and sewer lines as obtained from the Town of Cary. Information could not be obtained on the projected depths of the lines, but the diameters of the lines and locations of "manholes", "blowoffs," and other features were available. Prior to the site visit, the client provided Mid -Atlantic with a survey map showing current and planned site utilities prepared by The John R. McAdams Company, Inc. (McAdams) dated May 10, 2018. This map is included for reference. The locations for these onsite and adjacent utilities (both present and planned) as mapped by McAdams and the Town of Cary (as referenced on their website) were field -verified. We walked along public roads and public facilities within 1,000 feet of the site. Between 1,000 and 1,500 feet, properties were viewed by a vehicle travelling slowly and frequently stopping on public roads. Map C presents the locations of properties within a 1,500 radius with the potential for private water supply wells or septic systems. Mid -Atlantic personnel did observe some evidence of water supply wells in the area. However, municipal water meters were not commonly observed on the parcels. Some of the residences across Davis Drive to the west of the subject site appeared to be vacant. Mid -Atlantic submitted a request to the Town of Cary (which provides water and sewer services to both the Cary and Morrisville parcels) for information regarding the presence or absence of a utility account for the residences within 1,500 feet of the subject site. This information is included on the attached Map C. According to the information obtained during the survey, there are potentially four inhabited residences with potential water supply wells in use west of the subject site. Two additional residences along Morrisville Carpenter Road appeared to be abandoned.
