(Terraine) has prepared this letter report to summarize the results of groundwater monitoring activities at the Ferrell Residence located at 5379 Wiggins Mill Road, Lucama, Wilson County, North Carolina. On August 31, 2010, Terraine personnel mobilized to the site to collect groundwater samples from monitoring wells MW-1, MW-3, and MW-5R using new disposable bailers, and from water supply wells WSW-2 and WSW-3 to evaluate current groundwater quality conditions. Groundwater elevation data is presented in Table 1. The groundwater samples were submitted under proper chain of custody control to the Pace Analytical laboratory in Huntersville, North Carolina, for chemical analysis. Groundwater analytical data are summarized in Table 2. A complete laboratory report and chain of custody form for the groundwater samples obtained are attached. Immediately following sampling, a Universal Remediation, Inc. PRP® Well Boom was installed in monitoring well MW-1. Well Booms contain powdered beeswax and stimulate rapid biodegradation of dissolved petroleum contaminants. The incident manager was notified of contamination in water supply well WSW-3 immediately following receipt of the analytical results. Should you have any questions regarding any of this information, please contact me at 1-800-531-1242 ext. 710 or jbean@terraine.com. Sincerely, Jad Bean Project Manager Terraine, Inc.
