Dear Ms. Blalock:

ATC Associates of North Carolina, P.C. (ATC) has completed well abandonment activities for the
former Haithcock Grocery. The well abandonment was performed based on a December 20, 2019
Task Authorization (TA-05) by the Trust Fund Branch of the Underground Storage Tank (UST)
Section, Division of Waste Management, North Carolina Department of Environmental Quality
(NCDEQ). The work is performed as part of State Lead Contract #N17004 between NCDEQ and
ATC.

The former Haithcock Grocery is located at the southeast corner of the intersection of US Highway
158 and Tulip Lane in Macon, Warren County, North Carolina. The site consists of a two story
building with concrete and gravel parking occupied by Macon Auto Sales and is currently owned by
Tammy Parker. A total of seven USTs (one 2,000-gallon diesel, three 6,000-gallon gasoline, one
1,000-gallon gasoline, one 550-gallon kerosene, and one 2,000-gallon gasoline) were closed by
removal in 1992. Four monitoring wells (MW-1, MW-2, MW-3, and MW-4) are located at the
site. A site topographic map is included as Figure 1. The approximate locations of the former
USTs and monitoring wells are shown on Figure 2.

ATC prepared a Fate & Transport Modeling Report dated October 31, 2019. Based on the results
of the fate & transport modeling, the nearest active water supply well (P-8), which is located
cross-gradient of the site and used for drinking, will not be impacted by the contaminants at the
site. Therefore, ATC recommended closure of the Haithcock Grocery incident and that
groundwater usage at the site be restricted.

On February 5, 2020, Regional Probing Services mobilized to the site to abandon monitoring
wells MW-1, MW-2, MW-3, and MW-4. Monitoring well construction details are included in
Table 1. The monitoring wells were filled with hydrated bentonite and capped with concrete.
The well abandonment records are included in Appendix A.
