SCOPE OF SERVICES 3.1 Contract Information The scope of services for this report was performed in general accordance with S&ME’s proposal number 43-1500107, dated January 27, 2015, and 43-1500228, dated February 20, 2015. The scope of services was authorized by the North Carolina Department of Environment & Natural Resources (NCDENR’s) Task Authorizations 17513-01, dated February 5, 2015, and 17513-02, dated February 19, 2015, and within contract number N14005i. 3.2 Approved Scope of Services S&ME’s approved Scope of Services for these Task Authorizations included collecting groundwater samples from monitoring wells MW-1R, MW-3, MW-4, MW-5, and TW-1, and water supply wells WSW-1, WSW-2, WSW-3, WSW-4, and WSW-6, and completion of a federal/state groundwater monitoring report (TA-01), and performing a receptor survey update (TA-02). These services were completed as proposed, with the exception that the property serviced by water supply well WSW-4 remains vacant and the well is not currently active. A sample was collected from water supply well WSW-5 in its place. The updated receptor information is incorporated into this report. A separate receptor survey update letter report was not generated. Groundwater Monitoring Report, C&H Market, Inc. #17513 S&ME Project No. 4305-15-045 730 NC Highway 87, Elon, NC August 31, 2015 4 4. SITE HISTORY AND CHARACTERIZATION The C&H Market site, identified as NCDENR Incident Number #17513, is located at 730 NC Highway 87, Elon, Alamance County, North Carolina (Figures 1 and 2). The site is currently owned by Payne Oil Company, Inc. and contains four above-ground propane storage tanks; however, the site previously contained four USTs. In August 1991, three 1,000-gallon gasoline USTs and one 550-gallon kerosene UST were removed from the site. An assessment and observation of the tank removal was not performed. On January 29, 1996 a Closure Report issued by Cedar Rock Environmental Services, Inc. confirmed soil contamination near both the former gasoline USTs and the former kerosene UST. No contaminated soil has been removed from the site. In 2004, Geological Resources, Inc. (GRI) installed five shallow monitoring wells (MW-1 through MW-5) and one deep monitoring well (TW-1). Sampling results showed concentrations of benzene, toluene, ethylbenzene, total xylenes, and naphthalene above their respective 15A NCAC 2L .0202 Groundwater Quality Standards (2L Standards) in the sample collected from monitoring well MW-1. Water supply wells WSW-2, WSW-3, WSW-4, WSW-5, and WSW-7 were also sampled in March and April 2004. On March 11, 2004, benzo(a)anthracene was detected in the sample collected from water supply well WSW-4 and anthracene was detected in the sample collected from water supply well WSW-5 at concentrations greater than the laboratory method reporting limits. However water supply wells WSW-4 and WSW-5 were resampled on April 26, 2004, and the compounds were not detected at concentrations greater than the laboratory method reporting limits. GRI also conducted soil sampling in the area of the former USTs. Soil samples collected on March 11, 2004, showed petroleum constituents above their corresponding Soil-to-Groundwater Maximum Soil Contaminant Concentrations (MSCCs). However, the concentrations did not exceed the Residential MSCCs. On January 4, 2006, ATC Associates of North Carolina, P.C. (ATC) collected groundwater samples from the site monitoring wells and water supply wells WSW-1 through WSW-5. Laboratory analytical results indicted benzene, toluene, ethylbenzene, total xylenes, naphthalene, and 2-methylnaphthalene present at concentrations greater than their respective 2L Standards in the sample collected from monitoring well MW-1. In addition, chloroethane was detected in the sample collected from water supply well WSW-4 and chloroform was detected in the sample collected from water supply well WSW-5 at concentrations greater than the laboratory reporting limits, but less than their respective 2L Standards. On September 29, 2009, S&ME was on-site to gauge depth-to-water and to collect groundwater samples from monitoring wells MW-1, MW-3, MW-4, and MW-5. The laboratory analytical results of the groundwater samples indicated several petroleum contaminants in the sample collected from monitoring well MW-1 at concentrations greater than their respective 2L Standards. Analysis of groundwater samples collected from monitoring wells MW-3, MW-4 and MW-5 did not detect target petroleum compounds. Three water supply wells (WSW-2, WSW-3 and WSW-4) were sampled. The laboratory analytical results for each of the water supply well samples reported no concentrations exceeding the detection limits for the targeted petroleum constituents. Groundwater Monitoring Report, C&H Market, Inc. #17513 S&ME Project No. 4305-15-045 730 NC Highway 87, Elon, NC August 31, 2015 5 Water supply well samples were collected from water supply wells WSW-2, WSW-3 and WSW- 4 on January 14 and April 13, 2010. Lead was detected in samples collected from both water supply wells WSW-3 and WSW-4 exceeding its 2L Standard. Water supply wells WSW-3 and WSW-4 were sampled again for total lead by EPA Method 3030C with a duplicate sample lab filtered prior to analysis. Both the lab filtered and non-lab filtered samples from water supply wells WSW-3 and WSW-4 did not exhibit detections of lead above laboratory detection limits. On July 19, 2011, S&ME was on-site to gauge depth-to-water from monitoring wells MW-1, MW-2, MW-3, MW-4, MW-5, and TW-1. Following gauging, samples were collected from monitoring wells MW-1, MW-3, MW-4, MW-5, and TW-1. Samples were also collected from water supply wells WSW-2 and WSW-3. Six petroleum related compounds were detected at concentrations greater than their respective 2L Standards in the sample collected from monitoring well MW-1. Samples collected from monitoring wells MW-3, MW-4, MW-5, and TW-1 and water supply wells WSW-2 and WSW-3 did not exhibit concentrations greater than the laboratory method reporting limits. 4.1 UST Owner and Operator Information. The former owner of the tanks was Mildred M. Garrison. According to the NCDENR Registered UST Database the address for Mildred M. Garrison was 4259 NC Highway 87, Elon, North Carolina 27244. Information regarding the former UST system at the site is summarized in Table 1 and the locations of the former USTs are depicted on Figure 2. 4.2 UST Information The former UST system at the site included three 1,000-gallon gasoline USTs and one 550- gallon kerosene UST. According to the Closure reported submitted by Cedar Rock on January 29, 1996, the USTs were permanently closed by removal on August 22, 1991. However, according to the NCDENR Registered UST Database, the kerosene UST was removed in January 1978 and the gasoline USTs were removed on August 22, 1991. Information regarding these USTs is summarized in Table 1 and the locations of the former USTs are depicted on Figure 2. 4.3 Description of the UST Release  Date reported: August 23, 2010  Cause and Source (including tank identification number and contents): USTs 1, 2, and 3 (1,000-gallon gasoline UST) and UST 4 (550-gallon kerosene UST) 4.4 Site Characteristics  Land Use of Site: The site is currently occupied by Payne Oil Company. Historically the site was retail automotive fueling facility.  Land Use of Surrounding Area: The land use of the surrounding area includes a mixture of residential and agricultural properties. Groundwater Monitoring Report, C&H Market, Inc. #17513 S&ME Project No. 4305-15-045 730 NC Highway 87, Elon, NC August 31, 2015 6  Topography: The site is approximately 750 feet above the Nation Geodetic Vertical Datum of 1929. Based upon the topographic map reviewed, the area of the subject property slopes predominantly to the east. Refer to Figure 1 for a USGS Topographic Map.  Vegetation/Surface Cover: The site is predominantly covered by grass; howeve, a A few landscaped grass areas exist at the site; however, and asphalt and gravel driving area also exists at the site.  Surface Water: An unnamed pond is located approximately 820 feet northwest of the source area.  Wells: S&ME updated the receptor survey for this report. A public water supply is not available to the area of the site. Eleven water supply wells used as a primary drinking water source were identified within 1,000 feet of the site.  Buildings: No buildings are located at the site.  Soil Type: Soils underlying the site are characterized as Cecil fine sandy loam, with two to six percent slopes and is eroded, in the majority of the site; Cecil fine sandy loam, with six to 10 percent slopes and is eroded in the northwest corner of the site; and Starr loam, with two to six percent slopes and is occasionally flooded, in the southeast in the southeast corner.  Depth to and Nature of Bedrock: According to the 1985 USGS Geologic Map of North Carolina, the site is underlain by the Carolina Slate Belt of the Piedmont Physiographic Province. Bedrock in this area is light gray to greenish gray metamorphosed dacitic to rhyolitic flows and tuffs. They are interbedded with mafic and intermediate metavolcanic rock, meta-argillite, and metamudstone.  Depth to Groundwater in Source Area: Based upon data collected on April 29, 2015, depth to groundwater measurements at the site ranged from 14.59 feet below top of casing (ft-BTOC) in monitoring well MW-5 to 15.76 ft- BTOC in monitoring well TW-1. Groundwater depth and elevation measurements are summarized in Table 2.  Direction of Groundwater Flow: Based upon measurements collected on April 29, 2015, groundwater at the site flows predominantly west and southwest. Figure 3 graphically depicts the groundwater elevations at the site based on the April 29, 2015 measurements. 4.5 Owners and Occupants of Adjoining/Nearby Properties and Land Use Details regarding the adjacent properties are summarized in Table 3. The land use of the surrounding area includes a mixture of residential and agricultural properties. 4.6 Receptors/Potential Receptor Information S&ME completed a receptor survey update to identify water supply wells within 1,000 feet of the source area. The update was completed by performing a site reconnaissance to observe Groundwater Monitoring Report, C&H Market, Inc. #17513 S&ME Project No. 4305-15-045 730 NC Highway 87, Elon, NC August 31, 2015 7 potential well structures on properties within 1,000 feet. Well survey questionnaires were also mailed to property owners with observed well and/or residential structures on properties within 1,000 feet of the source area. S&ME referenced the Alamance County, North Carolina GIS system to identify property owners. The collected data was compiled into Table 3 and the location of the identified water supply wells are shown on Figure 4. 5. CURRENT SITE ASSESSMENT INFORMATION 5.1 Current Liquid Levels On April 29, 2015, S&ME was onsite to gauge liquid levels in monitoring wells MW-1R, MW- 3, MW-4, MW-5, and TW-1. The depth-to-water ranged from 14.59 feet ft-BTOC in monitoring well MW-5 to 15.76 ft- BTOC in monitoring well TW-1. A summary of current and historic groundwater levels is included in Table 2 and the groundwater elevations based on the April 29, 2015 measurements are depicted in Figure 3. 5.2 Groundwater Monitoring Events After measuring the depth-to-water, S&ME personnel purged monitoring wells MW-1R, MW-3, MW-4, MW-5, and TW-1 with disposable polyethylene bailers by evacuating at least three well volumes. The monitoring wells were allowed to recharge and suspended sediments allowed to settle. After completing the well purging, S&ME personnel collected a groundwater sample from the monitoring wells. The groundwater samples were placed in laboratory-supplied containers, stored in a cooler with ice, and shipped under chain-of-custody procedures to Pace Analytical Services, Inc., a North Carolina certified laboratory for laboratory analysis. The groundwater samples were analyzed for volatile organic compounds (VOCs) by Standard Method 6200B. S&ME also collected samples from water supply wells WSW-1, WSW-2, WSW-3, WSW-5, and WSW-6 on April 29, 2015. Each well that was sampled was purged for approximately 15 minutes prior to sampling. The groundwater samples were placed in laboratory-supplied containers, stored in a cooler with ice, and shipped under chain-of-custody procedures to Pace for laboratory analysis. The water supply well samples were analyzed for VOCs by Standard Method 6200B. The locations of the water supply wells are depicted on Figure 4. A copy of the monitoring well sampling form is included in Appendix I. 5.3 Summary of Groundwater Assessment Information The monitoring well sampling results are summarized in Table 4. No compounds were detected in the samples collected from monitoring wells MW-4, MW-5, and TW-1 at concentrations greater than the laboratory method reporting limits. Figure 5 graphically depicts the contaminants detected at concentrations greater than their respective 2L Standards in the groundwater samples collected on April 29, 2015. Because target analytes were only reported in monitor well MW-1R at concentrations greater than 2L Standards, isoconcentrations maps were not prepared to depict the estimated extent of dissolved petroleum compounds. No compounds were detected at concentrations greater than the laboratory method reporting limits in the samples collected from water supply wells WSW-1, WSW-2, WSW-3, WSW-5, and WSW-6. A copy of the laboratory report for the samples collected on April 29, 2015, is included in Appendix II. 5.4 Regional and Site Geology and Hydrogeology 5.4.1 Soil and Bedrock See Section 4.4. 5.4.2 Hydrogeology Based upon measurements collected on April 29, 2015, groundwater at the site flows predominantly west and southwest with an approximate gradient of 0.022 feet per foot. Figure 3 graphically depicts the approximate groundwater flow at the site based upon the April 29, 2015 measurements. 5.5 Evaluation of Groundwater Assessment Information 5.5.1 Current Extent and Severity of Contamination Concentrations of eight petroleum related compounds were detected in the samples collected from monitoring well MW-1R greater than their respective 2L Standards. As compared to the sample collected in July 2011, compounds concentrations detected in the sample collected from monitoring well MW-1R collected during this sampling event increased approximately 67% Groundwater Monitoring Report, C&H Market, Inc. #17513 S&ME Project No. 4305-15-045 730 NC Highway 87, Elon, NC August 31, 2015 9 overall. Compound concentrations detected above the laboratory method reporting limit in the sample collected from monitoring well MW-3 decreased approximately 67% as compared to the sample collected in July 2011. CONCLUSIONS AND RECOMMENDATIONS Based on the current and historical assessment information for the site, the following conclusions are presented: 1. The UST Incident meets the following conditions for a High Risk classification: a. Eleven water supply wells used as a primary source of drinking water are located within 1,000 feet of the source area. b. Contaminant levels in the groundwater exceed the 2L Standards. 2. The extent of groundwater impacts at the site appears to be limited. Based on the above, the following recommendations are presented: 1. Consider source area treatment to reduce groundwater contaminant concentrations. 2. Conduct annual monitoring of monitoring wells and water supply wells.
