B. EXECUTIVE SUMMARY
1. Summary of Activities Associated with Current TA
WithersRavenel, Inc. (WR), in association with Task Authorization‐02 under NCDEQ Contract # N17005,
performed assessment activities including completion of a GPR survey to locate potential USTs, the
collection of soil samples for laboratory analysis, the installation and sampling of one groundwater
monitoring well, and completion of a site reconnaissance and receptor survey at the facility.
2. GPR Survey, Soil, Groundwater, and Free Product Assessment Information
Ground‐Penetrating Radar Survey
WR contracted with GEL Engineering to conduct a ground penetrating radar (GPR) survey of the subject site
on December 12, 2019. The goal of the GPR survey was to identify relic underground storage tanks (USTs) that
may remain on the subject property. The GPR survey identified two possible USTs located on the subject
property (Figure 2), and a subsurface water line, also mapped. No additional areas of interest or anomalies
resembling relic USTs were identified on the subject property. Field notes and site photos are located in Appendix B.
Soils
WR collected four soil samples (SS‐1 through SS‐4) from the vicinity of the USTs identified during the GPR
survey to be analyzed for total petroleum hydrocarbons (TPH) gasoline range organics (GRO) and diesel
range organics (DRO).
Laboratory analysis of soil samples SS‐1 through SS‐4 showed both TPH‐GRO and TPH‐DRO above their
respective North Carolina State Action Levels. TPH-GRO results ranged from 260 mg/kg to 3,300 mg/kg and
TPH-DRO results ranged from 100 mg/kg to 3,800 mg/kg.
Groundwater
Groundwater monitoring well MW‐1 was installed and sampled on December 12, 2019 for volatile organic
compounds (VOCs) by SM 6200B and semi‐volatile organic compounds (SVOCs) by EPA Method 625 plus
tentatively identified compounds (TICs). Multiple petroleum compounds were detected in the groundwater
sample collected from monitoring well MW‐1 at concentrations above their respective 2L Standards;
benzene and 1,2‐dichloroethane were detected at concentrations exceeding their respective GCLs.
LNAPL
No LNAPL was observed in on‐site groundwater monitoring wells during the December 12, 2019
assessment at the facility.
3. Receptor Information
WR performed a receptor survey in association with the current TA. A total of three water supply
wells were identified within a 1,000‐foot radius of the site. One additional water supply well is located
between 1,000 and 1,500 feet of the source area. Municipal water is available to the area through the Town of
Canton.
The NC Source Water Assessment Program, managed by the NCDEQ Water Resources Drinking Water
Protection Program, does not identify any community water supply wells within a one‐mile radius of the site.
The nearest identified wellhead protection area is located 1.34‐miles northeast of the site
(Harmony Grove Baptist Church, Public Water Supply ID NC0144459, EPA ID 15688).
An unnamed tributary to North Hominy Creek is located approximately 125 feet west‐southwest of the
subject property.

5. Recommendations
Based on the results of assessment activities conducted at the site in association with the current TA, WR
recommends the following:
•Remove the two USTs that were identified by the GPR survey and any contaminated soils identified
during closures.
•Install additional monitoring wells to delineate the hydrocarbon plume and determine groundwater
flow direction.
•Connect the residence located at 110 Jordan Lane to municipal water and abandon their impacted
water supply well.
C.BACKGROUND
The Wood’s Grocery is located at 1370 Newfound Road in Canton, Haywood, North Carolina (see Figure
1). The subject property currently consists of a mobile home, drive way, and grass yard. Figure 2 is provided
as a site map showing the current conditions of the subject property. The subject parcel encompasses a
total of 0.80 acre of property. A mobile home and storage sheds are located on the southern portion of the
parcel. The current property owners plan to build a single‐family residence on the central portion of the
parcel and demolish the mobile home.
In July 2018, a groundwater sample was collected from the water supply well located at 1370 Newfound
Road. The laboratory analysis of the groundwater sample detected di‐isopropyl ether, benzene, and 1,2‐
dichloroethane at concentrations that exceeded applicable standards. It was determined that the water was
not suitable for residential use. The residence located on the subject property was connected to the
municipal water line along Newfound Road in October 2018. In January 2019, the water supply well located
on the subject property was abandoned by a licensed well driller.
In August 2019, two adjacent water supply wells were sampled. The water supply well located at 110 Jordan
Lane contained petroleum compounds that exceeded the applicable standards for potable use. The property
has been receiving bottled water delivery since September 2019. The resident of 110 Jordan Lane, Mr. Joey
Smith, informed the NCDEQ that there was a gasoline station formerly located at 1370 Newfound Road
known as the Wood’s Grocery Store. The dates of operation of the gasoline station are unknown, however
the store building eventually burned down.
The following paragraphs describe characterization of risk as well as the GPR survey and the soil and
groundwater assessment activities that occurred on December 12, 2019.
4.Remediation Activities
No remedial activities have been undertaken at the site, to date.

D.RISK CHARACTERIZATION
Part I – Groundwater/Surface Water/Vapor Impacts
High Risk
1.Has the release contaminated any water supply well including any well used for non‐drinking
purposes? YES – The water supply well located on site that is now abandoned and the water supply
well located at 110 Jordan Lane.
2.Is a water supply well used for drinking water located within 1,000 feet of the source area of the
release? YES – A total of three water supply wells are located within 1,000 ft of the source area.
3.Is a water supply well not used for drinking water (e.g. irrigation, washing cars, industrial cooling
water, and filling swimming pools) located within 250 feet of the source of the release? NO – All
supply wells identified at locations surrounding the source area are used as potable supplies,
although these same wells may be used for non‐potable purposes.
4.Does groundwater within 500 feet of the source area of the release have the potential for future
use (there is no source of water supply other than groundwater)? NO – municipal water is available
to the area by the Town of Canton.
5.Do vapors from the release pose a threat of explosion because of accumulation of the vapors in a
confined space or pose a serious threat to public health, public safety, or the environment? NO –
WR is not aware of any subsurface utilities or basements near the source area.
6.Are there any other factors that would cause the release to pose an imminent danger to public
health, public safety, or the environment? NO.
Intermediate Risk
7.Is a surface water body located within 500 feet of the source area of the release? YES – an unnamed
tributary to North Hominy Creek.
If YES, does the maximum groundwater contaminant concentration exceed the surface water
quality standards and criteria found in 15A NCAC 2B .0200 by a factor of 10? YES – benzene was
detected in the groundwater sample from monitoring well MW‐1 at a concentration greater than
10 times the 2B Standard.
8.Is the source area of the release located within an approved or planned wellhead protection area as
defined in 42 USC 300h‐7(e)? NO ‐ According to the NCDEQ Wellhead Protection Program online
map viewer, the source area is not located within an approved or planned wellhead protection
area.
9.Is the release located in the Coastal Plain physiographic region as designated on a map entitled
“Geology of North Carolina” published by the department in 1985? NO.
10.Do the levels of groundwater contamination exceed the gross contaminant levels established by the
Department? YES – benzene and 1,2‐dichloroethane were detected at concentrations above their
GCLs.

4.Do children visit the property? YES – children live on the property.
5.Is access to the property reliably restricted consistent with its use (e.g., by fences, security
personnel, or both)? NO.
6.Do paved areas, buildings, or other structures cap the contaminated soil? NO – the contaminated
soil is covered with grass.
7.What is the zoning status of the property? “Residential” (Haywood County Zoning designation).
8.Is the use of the property likely to change in the next 20 years? NO.
Property Surrounding Source Area of Release
The questions below pertain to the area within 1,500 feet of the source area of the release (excludes
property containing source area of the release):
1.What is the distance from the source area of the release to the nearest primary or secondary
residence (permanent or temporary)? The nearest permanent residence from the source area is
located at 1333 Newfound Road, approximately 90 ft northwest of the source area.
2.What is the distance from the source area of the release to the nearest school, daycare center,
hospital, playground, park, recreation area, church, nursing home, or other place of public assembly?
The nearest church is the Calvary Baptist Church located at 23 Phillips Street, approximately 600
feet west‐southwest of the source area.
3.What is the zoning status of properties in the surrounding area? “General Residential”
4.Briefly characterize the use and activities of the land in the surrounding area. The surrounding
properties consist of private rural residences and agricultural lands. Table B‐5B provides a
summary of the adjacent properties.
D.1. RECEPTOR INFORMATION
1.Water Supply Wells
A water supply well survey for the area within 1,500 feet of the source area was completed by WR
for the purpose of risk classification. In the course of completing the survey, WR reviewed the
surrounding properties, and confirmed that publicly available water supply is located within 1‐mile
of the site.
A total of three water supply wells were identified within a 1,000‐ft radius of the site. One
additional water supply well is located between 1,000 and 1,500 ft of the source area. Municipal
water is available to the area through the Town of Canton. Locations of water supply wells within
1,500 ft of the site are shown on Figure 6. Supply well ownership for wells are included in Table
B‐5A.
2.Public Water Supplies
Are public water supplies available within 1,500 feet of the source area of the release? YES ‐ The
Town of Canton provides municipal water to properties within 1,500 feet of the source area.
3.Surface Water
Identify all surface water bodies (e.g., ditch, pond, stream, lake, and river) within 1,500 feet of the
source area of the release.
An unnamed tributary to North Hominy Creek is located approximately 125 feet west‐southwest
of the subject property.

4.Wellhead Protection Areas
Identify all planned or approved wellhead protection areas (e.g., ditch, pond, stream, lake, and river)
within 1,500 feet of the source area of the release. This information must be shown on the USGS
topographic map. Wellhead protection areas are defined in 42 USC 300h‐7(e). The NC Source
Water Assessment Program, managed by the NCDEQ Water Resources Drinking Water Protection
Program, does not identify any community water supply wells within a 1‐mile radius of the site.
The nearest identified wellhead protection area is located approximately 1.34‐miles northeast of
the site (Harmony Grove Baptist Church, Public Water Supply ID NC0144459, EPA ID 15688).
5.Describe Deep Aquifers in the Coastal Plain Physiographic Region
The site is not located within the Coastal Plain Physiographic Province of North Carolina.
Therefore, Deep Aquifers of the Coastal Plain are not addressed herein.
6.Describe Subsurface Structures (refer to page 19 of the guidelines):
None Identified.
7.Property Owners and Occupants
The attached Table B‐5B lists the names and addresses of property owners and occupants adjacent
to the subject property.

E.SITE GEOLOGY AND HYDROGEOLOGY
The Geologic Map of North Carolina (Department of Natural Resources, Division of Land Resources, North
Carolina Geologic Survey, 1985) shows materials underlying the site to consist of “biotite gneiss” of
Proterozoic age, within Sedimentary and Metamorphic rocks of the Blue Ridge Belt. Base materials at the
site location are indexed as “migmatitic; interlayered and gradational with biotite‐garnet gneiss and
amphibolite; locally abundant quartz and alumino‐silicates.”
Evaluation of the Canton, NC Topographic Quadrangle (US Geological Survey, 7.5 Minute Series, 2019) and
2‐ft elevation contours provided by the Haywood County GIS Service indicate that the subject property is
at an approximate elevation of 2,815 feet above mean sea level (AMSL) at the location of the former UST
basin. A scaled portion of the USGS Quadrangle is included as Figure 1. Topography at the site slopes
primarily to the west‐southwest. Depth to groundwater at the site was measured at 58.60 feet beneath
the Top of Casing (TOC) of monitoring well MW‐1 installed on the parcel. No LNAPL was measured or
observed in monitoring well MW‐1.
The general lithology of the site soils consisted of micaceous silts and clayey silts.

F.SOIL AND GROUNDWATER INVESTIGATION – DECEMBER
2019
WithersRavenel mobilized to the site on December 12, 2019 to conduct a GPR survey, install one Type II
groundwater monitoring well, and collect soil samples from the vicinity of the USTs. Monitoring well MW‐1
was installed approximately 45 feet south‐southeast of the UST basin, between the existing UST basin and the
on‐site contaminated water supply well which has been abandoned (Figure 2).
F.1. SOIL INVESTIGATION
Prior to the collection of soil samples, a GPR survey was conducted to identify possible USTs on the subject
property. Two possible USTs were identified on the northwestern portion of the subject property. Four soil
samples were collected in 5‐foot intervals to a depth of 10 feet below ground surface (bgs) using a
GeoProbe™ drill rig and direct push techniques (DPT). Soil samples were removed from the borings via
Macro‐Core® Sample Tubes with a dedicated Macro‐Core PETG Liner inside, classified, and collected for
screening into individual ZipLoc® bags. The retained soil samples were then field screened using a RAE
Systems™ Mini RAE® 2000 Photo‐ionization Detector (PID) for the presence of VOCs typical of potential
petroleum hydrocarbons. The PID is an industry-accepted scanning device used to detect the presence of
organic vapors, but is not entirely relied upon to determine specific levels of contamination. Samples were also
assessed through visual observations and assessment for odors reminiscent of petroleum products. PID readings
of the soil samples were >15,000 ppm at 10 feet bgs in each sample.
Sampling activities were performed by properly trained environmental professionals. Sampling
personnel utilized new disposable gloves to prevent contact with skin and prevent cross -contamination of
samples. Sampling equipment was decontaminated prior to advancement. All samples slated for laboratory
analysis were transferred directly into laboratory‐prepared containers, immediately placed on ice,
and transported under chain‐of‐custody to Con-Test Analytical Laboratories (Con-Test) in East Long-
meadow, MA under standard turnaround times to be analyzed for TPH‐DRO and TPH‐GRO by EPAMethod 8015C.
Laboratory Results
Laboratory analysis of soil samples SS‐1 through SS‐4 detected both TPH‐GRO and TPH‐DRO above their
respective North Carolina State Action Levels. TPH-GRO results ranged from 260 mg/kg to 3,300 mg/kg and
TPH-DRO results ranged from 100 mg/kg to 3,800 mg/kg.
A summary of the soil analytical results is presented in Table B-3 and shown on Figure 5. A copy of the
laboratory analytical report and chain-of-custody is included in Appendix A. Field notes and a photographic
record are provided in Appendix B.
F.2. GROUNDWATER INVESTIGATION
Following soil sampling activities, 4.25‐inch inner‐diameter (ID) hollow stem augers were utilized to advance a
boring t o a depth of 65 feet bgs where t he boring was t erminated in saturated conditions. A Type II
monitoring well (MW‐1) was constructed with 15 feet of 2‐inch diameter 0.01‐slotted PVC well screen
and 40 feet of 2‐inch diameter PVC casing. Clean‐filter sand was placed into the annular space surrounding
the well screen to approximately 2 feet above the screened section of the well. A bentonite seal was placed
over the sand pack extending from 5 feet bgs to the upper surface of the sand pack and hydrated.
The remaining annular space was then filled with neat cement grout to surface grade, encasing an 8‐
in diameter steel manhole cover. The monitoring well was developed to remove fine
sediments following completion. See Table B‐7 for well construction details & groundwater elevation data.

Laboratory Results
Groundwater monitoring well MW‐1 was installed and sampled on December 12, 2019 for volatile organic
compounds (VOCs) by SM6200B, and SVOCs by EPA Method 625. Multiple petroleum compounds were
detected in the groundwater sample collected from monitoring well MW‐1 at concentrations above their
respective 2L Standards; benzene and 1,2‐dichloroethane were detected at concentrations exceeding their
respective GCLs.
A summary of the groundwater analytical results is presented in Table B-4. A copy of the laboratory
analytical report and chain‐of‐custody is included in Appendix A.
Following well development, groundwater samples were collected using dedicated bailers and placed
directly into laboratory-provided containers, placed in a cooler of ice, and under proper chain-of-custody
transported to Contest to be analyzed for VOCs using SM6200B and SVOCs + TICs using EPA
Method 625+TICs.

G. CONCLUSIONS
Based on data obtained in the completion of this assessment at the property, two potential USTs were
identified during the GPR survey, soil contamination was identified around the USTs above the North
Carolina State Action Levels, and groundwater contamination was identified in monitoring well MW‐1. Both
TPH‐GRO and TPH‐DRO were identified in the four soil samples collected around the UST basin above the
North Carolina State Action Levels. Multiple petroleum hydrocarbons were detected in the groundwater
sample collected from monitoring well MW‐1 above their respective 2L Standards; benzene and 1,2‐
dichloroethane were reported at levels above their respective GCLs.
Municipal water is available to the site and surrounding properties located within 1,500 feet of the source
area through the Town of Canton. Three active water supply wells were identified within a 1,000‐foot
radius of the source area. One of these water supply wells, located at 110 Jordan Lane, is known to be
contaminated. One additional inactive water supply well was identified between 1,000 feet and 1,500 feet
of the source area.
Accounting for the presence of water supply wells in proximity to the release, one of the water supply wells
known to be contaminated, and the use of the property as a single‐family residence, WR believes the site to
meet the criteria of “high” risk, and recommends the following:
• Remove the two USTs that were identified by the GPR survey and any contaminated soils
identified during closures.
• Install additional monitoring wells to delineate the hydrocarbon plume and determine groundwater
flow direction.
• Connect the residence located at 110 Jordan Lane to municipal water and abandon their impacted
water supply well.
