HomeMy WebLinkAbout05016_Southern Mfg Site Visit Notes Phone Summaries 20080528Southern Manufacturing-NC Music Factory Asbestos removal 8/29 Call from Richard Nelson call about illegal run off to storm water on to city streets from soil excavation at the site. Micheal Burkhart was the contact person. I tracked down Micheal Burkhart who is the team leader for inspectors in the area. His plumbing inspector called in the potential infraction. The site in the City jurisdiction and Saba from City Erosion Control visited the site. During his visit the streets were clean and he did not see any infractions. He was later brought to the site for the MH erosion control permit inspection the following week. 9/5 9:45 am. I visited the site unannounced. East Coast was off the site, no erosion control fencing, the site was wide open. I meet Jim Rodarte of Morlando-Holden. He explained they were the general contractors laying the storm water and finish work on the site. They were connecting some broken pipes and found potentially asbestos containing materials. I arrived within the first half hour of them working. We called Mac of Ark to come and discuss the situation. Mac and I called Scott Cornell of East Coast Wrecking and spoke on the phone. I asked about sediment fencing and erosion control. He stated his company rolled off the job a week ago and removed all silt fencing since it was in contact with asbestos soils. It was his understanding the general contractor MH was to install his own fencing and erosion controls. He agreed to send the waste manifest certificate. I asked about final inspections whether visual or analytical. I stated that MH was to be working in the soil, testing would have be done to show ACM was no longer present. All soil work on the site must be done by an asbestos accredited personnel. We agreed to meet back on Friday morning at 10 am. 9/7 10 meeting at the site with ARK, MH, East Coast, City Erosion inspector Saba, and DENR BF. • Discussed erosion controls, the disturbed area is less than 1 acre, therefore did not need grading permits. However, inspected the site and gave an approval. He will make random inspections. • Discussed the additional work that needs to be 9/4 Friday Andy F&R 704-998-8844, asked questions about UST requirements 9/12 Wednesday, 3:12 pm Brady Tub, NC HHCU Raleigh He received a call from Chuck of Mecklenburg County air permitting. ACM was on the ground, not ropes off taped off . Wanted to let me and follow-up as needed. 919-707-59 9/12, Wednesday, 3:56 pm Pat Wiley left message around called to stated an anomis caller stated that ACM soil were being improperly handled and stored at the site. 9/13 Notes from discussion with Pat Wiley, Industrial Hygiene Consultant, NC DHHS - Health Hazards Control Unit, 919-707-5971 Pat review the 8/5/07 Site Safety and Health Plan prepared by East coast Wrecking. She made the following comments: 3.2.1 Asbestos Abatement, references state of Alabama. 3.3.2 Excavation of Equipment Oil Spils, reference Base Facility Fire Department and Base Facility Response Plan, what are these plans and are they for the NC Music Factory. 4.3.12 Inspections: What environmental consultant has been hired to conided the air monitoring? It must be an accredited air monitor for NC. Ask for the results during the initial excavation work. 5.1 Respiratory Protection: None visible in site photos, nor have I see any during early morning site visits. Review this section with Scott during meeting on 9/14. Asbestos marking tape to caution personal that ACM in the area. Likely to need half face masks 7.4.2 Truck Wash, has the truck wash station been rebuilt? 8.1 Air Monitoring: air monitor work for employees can be done by supervisor, but along property boundaries for site information, must be conducted by accredited air monitor person. General comments: • An asbestos designer should have written he HASP, the only person at East Coast Wrecking with a license is Scott Cornell. • All persons on the site working in the soil excavation process should have accreditation. It is a picture ID similar to driver’s license. • OSHA 29 CFR 1929.1101 is training for asbestos work. Heavy construction equipment workers must have this certification. This would be an OSHA violation. The equipment must have an enclosed cab, otherwise it must be ACM accreditation. • OSHA Consultants to ask questions: o Ed Geddie 919/807-2880 o Susan Hritos 919/807-2879 • Post excavation, refered to NESHAP 40 CFR 61.151 closure of manufacturing site. Pat stated this level of closure would take approval from EPA Region IV and could not be done at state level. I will follow-up with Bruce on how if relates to BF Program. Based on my understanding of BF Statute, we should have the authority to close since it is not NPL Caliber or petroleum UST Site. Follow-up: • 9/14 site meeting : Mac from Ark Entertainment, Scott Cornell of East Coast; Jim Rodarte MH; Carolyn Minnich, DENR BF • Send copies of the disposal manifests to Pat Wylie 9/14 9:30 am Site Meeting with Mac, Scott, Brandon, Jim, Joe, and another person from MH. Passed out approval letter for based on change in work plan request. All soil excavation is complete Storm pipes need to be installed, during laying for storm pips, no soil excavation will be needed, however small portions o dirt will be moved to fit pipes to existing pipes. The side walls of the hole will be lined with poly and areas sprayed down. Materials are being ordered and work will begin on Monday. Soil excavation was done on Wednesday. Scott was on site directing MH excavator how to handle sol. 9/14 Site visit: During visit, the area was contained, the piles were covered and orange fencing was around the excavation area. East Coast will hang signs that ACM and tape off areas. The meeting courteous, effective and done by 10 am. I agreed to visit the site next week either M, W, or F.
