In accordance with your application for a discharge permit received on February 3, 2016, we are
forwarding herewith the subject certificate of coverage (COC) to discharge under the subject state —
NPDES general permit. This permit is issued pursuant to the requirements of North Carolina General
Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the US
Environmental Protection Agency dated October 15, 2007 (or as subsequently amended). We are
also hereby rescinding your current No -Exposure Certification NCGNE0814 for the subject facility.
Please take notice that this certificate of coverage is not transferable except upon approval of the
Division of Energy, Mineral, and Land Resources. The Division of Energy, Mineral, and Land
Resources may require modification or revocation and reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by
the Division of Energy, Mineral, and Land Resources, the Coastal Area Management Act, or any
other federal or local government permit that may be required.

in accordance with the effluent limitations, monitoring requirements, and other conditions set
forth in Parts I, 11, 111, and IV of General Permit No. NCG130000 as attached.
This certificate of coverage shall become effective March 9, 2016.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.

We've received an NOI for an existing a -waste recycling facility in your region:
NCG130076 Powerhouse Recycling in Rowan County. This site discharges stormwater at 3 points to UT to Grants Creek
(Class C) or possibly to the City of Salisbury storm sewer system. The NOI is attached.
Note that they currently have a No Exposure Certification (NCGNE0814) which will be rescinded simultaneously with the
issuance of this COC. The site was inspected by MRO in 2015 and at that time, was found to qualify for No
Exposure. Now, there are open containers exposed to stormwater which is why they are applying for permit coverage
now. Since it was inspected so recently, I don't know if you'll need to do another inspection, but please let me know
what you decide.
Please let me know if the Mooresville Regional Office has any concerns about issuing this COC.

The subject facility had a no exposure certification NC�NE0814 for its location at 175 Lane Pkwy, Salisbury, NC
28146. Recently they moved to a new location at � Ryan Patrick Dr., Salisbury, NC 28147 and they requested a new
no -exposure certification. I sent them an approval for no exposure for the new facility under the same number that we
received from Raleigh (NCGNE0814). My cover letter showed the new address. However, the old address was shown in
the inspection report. It looks like we need to create a new facility for the new address. It is now in your hands. Please
let me know how do we proceed from here.

The Division has reviewed your submittal of the No -Exposure Certification form for Exclusion
from NPDES Stormwater Permitting. Based on your submittal and signed certification of no
exposure at the above referenced facility, and the inspection dated April 7, 2015, the Division is
granting your conditional exclusion from permitting as provided for under 40 CFR 122.26(g), which
is incorporated by reference in North Carolina regulations.
Please note that by our acceptance of your no -exposure certification, you are obligated to maintain
no -exposure conditions at your facility. If conditions change such that your facility can no longer
qualify for a No -Exposure exclusion, you are obligated to immediately obtain NPDES permit
coverage for your stormwater discharge. Otherwise, the discharge becomes subject to enforcement
as an unpermitted discharge.
You must self re -certify your No -Exposure status annually using the included form or obtain
NPDES permit coverage for any stormwater discharges from your facility. The annual self re-
certification does not need to be submitted to DEMLR unless requested.
Your conditional exclusion from permitting does not affect your facility's legal requirements to
obtain environmental permits that may be required under other federal, state, or local regulations or
ordinances.

standards and regulations promulgated and adopted by the North Carolina Environmental
Management Commission, and the federal Water Pollution Control Act, as amended,

to receiving waters designated as UT to Grants Creek, a class C water in the Yadkin River Basin,
in accordance with the effluent limitations, monitoring requirements, and other conditions set
forth in Parts I, II, III, and IV of General Permit No. NCG130000 as attached.
This certificate of coverage shall become effective March 9, 2016.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.
