forwarding herewith the subject certificate of coverage to discharge under the subject state — NPDES
general permit. This permit is issued pursuant to the requirements of North Carolina General Statute 143-
215.1 and the Memorandum of Agreement between North Carolina and the US Environmental Protection
Agency dated October 15, 2007 (or as subsequently amended).
Please take notice that this certificate of coverage is not transferable except after notice to the
Division of Water Quality. The Division of Water Quality may require modification or revocation and
reissuance of the certificate of coverage.
Per the requirements of the Neuse Riparian Buffer Rule, all stormwater drainage to stream buffers,
from portions of this site that have been constructed after July 22, 1997, must be discharged through a
correctly designed level spreader or another device that meets diffuse flow requirements per 15A NCAC
213 .0233. Diffuse flow requirements are described in Chapter 8 of the North Carolina Stormwater BMP
Manual, available at: http://h2o.enr.state.nc.us/su/bmp forms.htm.
This permit does not affect the legal requirements to obtain other permits which may be required
by the Division of Water Quality or permits required by the Division of Land Resources, Coastal Area
Management Act or any other federal or local governmental permit that may be required.

Dear Mr. Winterholler:
Staff of the North Carolina Division of Water Quality, Surface Water Protection Section, Raleigh
Regional Office, conducted a stormwater compliance evaluation inspection of K-Flex USA, 100
Namco Drive, Youngsville, on November 16, 2012. Your assistance was very helpful and appreciated
during the inspection process.
The following observations were made during the stormwater inspection:
K-FLEX USA is a leading manufacturer of synthetic rubber insulation materials, including
sheets and rolls for piping. Also produces elastomeric, closed cell insulation for plumbing,
HVAC, commercial/industrial, marine, oil & gas and solar applications.
2. The Stormwater Pollution Prevention Plan for this facility contained all components
required by NCG050000. The SPPP is currently implemented by the facility (Please update
the responsible party(s) list).
This facility does not perform any vehicle maintenance and does not utilize more than 55
gallons of new motor oil per month when averaged over a calendar year. Therefore, no
analytical test are required.
4. Qualitative monitoring was performed as required by General Permit NCG050000.
This facility has two BMPs in place. The BMPs are bio-retention ponds. All BMPs were
well maintained at the time of the inspection.

NCG05. The site discharges stormwater to an unnamed tributary to Hattles Branch (C; NSW).
The NOI is attached.
Please let me know if the Raleigh Regional Office has any concerns about issuing this
facility a COC for this general permit, and are there any potential impacts to wetlands? If
we don't receive any objections, we'll issue the COC in 30 days.
