This permit expires on March 31. 2003, The Division staff is currently in the process of rewriting this permit and is scheduled to have the permit reissued by early spring of 2003. Once the permit is reissued, your facility would be eligible for continued coverage under the reissued permit. 4 In order to assure your continued coverage under the general permit, you must apply to the Division of Water Quality (DWQ) for renewal of your permit coverage. To make this renewal process easier. we are informing you in advance that your permit coverage will be expiring. Enclosed you will find a Permit Coverage Renewal Y Application Form. The application must be completed and returned by October 2. 2002 in order to assure continued coverage under the general permit. On or about 5:30 am on 17 April 2012, we received a report of a discharge of PVC resin from our silo 42. We immediately shut down the silo which stopped the discharge. Approximately five hundred pounds of resin was released onto our property and the adjacent storm water out flow area. Of the approximate five hundred pounds that was released from the silo we estimate that approximately two hundred pounds made it to the storm water area. Our storm water out fall is dry with no flow unless we have a rain event. There are no aquatic organisms present in this area. We conducted a visual inspection of the storm water area and adjacent areas on April 17 th, April 18'h and April 19'h and did not observe any substantial harm to the environment. Our investigation found that the over fill protection system which is a redundant safety system on the silo failed causing the silo to over fill. We will not bring the silo back on line until the over fill system is repaired. We are also going to upgrade the over fill system and install manual controls to insure the silo does not over flow again. We have contracted with Clean Harbors, our emergency response contractor, who have initiated the removal and clean up of the resin from the affected areas. No additional discharge to the storm water system has occurred. If you have further questions about this report please contact our EHS Manager, David Dougherty at 828-459-3302.
