Box 480 Granite Quarry, North Carolina 28072 Subject: Compliance Evaluation Inspection McKenzie Taxidermy' COC Number NCG050019 Rowan County, NC Dear Mr. McKenzie: Enclosed is a copy of the Compliance Evaluation Inspection (CEI) report for the inspection conducted at the subject facility on January 19, 2011, by Ms. Barbara Sifford of this Office. The site drains into an unnamed - tributary of Church Creek in the Yadkin River Basin. The site visit and file review revealed that the subject facility is covered by NPDES Stormwater General Permit NCG050000. The assistance of Mrs. Wanda Mills was greatly appreciated during the inspection. Accordingly, the following observations were noted during the inspection: 1) Stormwater Pollution Prevention Plan (SPPP) Yes X No A SPPP has been developed the plan needs to be updated to reflect the additional buildings that have been added to the complex. This plan is to be reviewed and updated on an annual basis. The update is to include a recertification that the stormwater outfalls have been evaluated for the presence of non- stormwater discharges. 2) Qualitative Monitoring Yes X No Qualitative monitoring should be conducted on a semi annual basis. All outfalls should be evaluated during representative rain events and document the discharge characteristics. A qualitative form -is included with this report. To retain the capacity of the pond it will need sediment to be removed when the capacity is reduced by 50 %. Otter outfalls noted during the inspection could use some rock to stop erosion. The facility will have 90 days from the date of this letter to come into compliance wish the rioted observations. A follow --up evaluation will be conducted after July 1.2011 to ensure compliance with the facility's Stormwater Permit NCO050019 Certificate of Coverage_ The report should be self-explanatory. Please be advised that violations of the NPDES General Permit are subject to a civil penalty assessment of up to $25,000.00 per day for each violation.
