High Point, NC 27265 Subject: Request for Additional Information Stormwater Permit No. SW4190301 Mountain Park Elementary School Surry County Dear Mr. MacEldowney: The Division of Energy, Mineral and Land Resources (DEMLR), received a Stormwater Management Permit Application for the subject project on March 7, 2019. A review of that information has determined that the application is not complete. The following information is needed to continue the stormwater review: 1. The maximum project density for low density projects in Outstanding Water Resource watersheds is 12% (15A NCAC 02H .1021). For a project with existing built upon area, the project density may be calculated as the difference of total BUA minus the existing BUA divided by the difference of total project area minus existing built upon area (15A NCAC 02H .1003). 2. To qualify for low density, the project must use overland flow or vegetated conveyances. A minimal amount of piping is allowed only for driveways or culverts under a road when it cannot be avoided (15A NCAC 02H .1021). The proposed plans include too much piping to qualify for low density. 3. A Supplement EZ form is required for all projects, including low density developments. The form can be found here: hM2s:Hdeq.nc.gov/sw-bmp-manual. There is an option on the form for low density or high density. 4. The Applicant's Certification section of the application must be signed and notarized for all applications. The Property Owner Authorization was signed instead. This section is only for projects where the applicant is not the property owner. Please update the application with a signed Applicant's Certification. Provide pdfs of all revisions, 2 hardcopies of revised plan sheets, and 1 hardcopy of other documents. Pdfs must be uploaded using the form at: hqps:Hedocs.deg.nc.gov/Forins/SW ProjectSubmittal Please note that this request for additional information is in response to a preliminary review. The requested information should be received by this Office prior to July 4, 2019, or the application will be returned as incomplete. The return of a project will necessitate resubmittal of all required items, including the application fee. Please reference the State assigned project number SW4190301 on all correspondence. If you need additional time to submit the information, please submit your request for a time extension to the Division at the contact below. The request must indicate the date by which you expect to submit the required information. The Division is allowed 90 days from the receipt of a completed application to issue the permit. The construction of any impervious surfaces, other than a construction entrance under an approved Sedimentation Erosion Control Plan, is a violation of NCGS 143-215.1 and is subject to enforcement action pursuant to NCGS 143-215.6A.
