Thank you for taking my call earlier today. I apologize for the delayed response. I have been asked to complete the
attached questionnaire on behalf of Dale Recycling. The delay has been due to my schedule (and I am out of the office
this week). We respectfully request a brief extension to complete the attached paperwork. We can have all submitted
by Wednesday, April 24, 2019. 1 will submit the response directly to DENR's Raleigh office via US Mail and will scan you
a copy.
Please confirm receipt and whether the extension is granted.
