Attached is a Rescission Request Form for Quality Carriers, Inc. (QCI) located at 3823 Hawkins Avenue, Sanford,
NC 27330. QCI is requesting to terminate their stormwater permit since the facility has ceased operations at this
location on or about February 1, 2019. Please let me know if you have any questions. A copy has also been
mailed to NCDEQ.
