RLGAN sectelnr1 TRACY DAVIS Dlre0w The Division of Energy, Mineral and Land Resources (DEMLR), received a Stormwater Management Permit Application for the subject project on November 6, 2017. A preliminary review of that information has determined that the application is not complete. The following information is needed to continue the stormwater review: 1. Provide a soils report that includes the SHWT elevation within the footprint of each SCM [MDC 1]. 2. Provide a completed supplement form for the SCMs, using the Supplement -EZ form at: hgps:Hdeq.nc. gov/about/divisions/energy-mineral-land-resources/energy-mineral-land- permit-guidance/stormwater-bmp-manual 3. Provide a completed O&M Agreement for the SCMs, using the O&M -EZ form at: hgps:Hdeq.nc. gov/about/divisions/energy-mineral-land-resources/energy-mineral-land- permit-guidance/stormwater-bmp-manual 4. On sht. C501, the impervious areas for pre and post development in the project data table is not consistent with the narrative and calcs. 5. Show the existing and proposed contours on the pre and post DA maps; and delineate the DA for each SCM. 6. Provide the DA maps at a legible scale [02H.1042]. 7. Provide supporting calcs. for the 1" WQV and the underdrain sizing for each SCM [Gen MDC 1]. It must show the underdrains and cleanouts [MDC 5]. b. Spec. that #57 stone shall be washed. c. Spec. that the sand in the media be medium to coarse sand per MDC 6. d. Spec. that the sod not be grown in clay soils per MDC 10. e. Spec. that no mechanical compaction be used per MDC 8. 9. An IWS zone must be provided on the underdrains unless onsite soils tests demonstrate in-situ soils have an infiltration rate of 2 in/hr or greater [MDC 4 &5]. 10. The media depth must be a minimum of 30" with an IWS zone [MDC 5]. 11. The P -index no longer has a lower threshold of 10 [MDC 7]. 12. Demonstrate non-erosive flow for the 10 -yr storm at each discharge point into each SCM. Energy dissipaters are recommended. [Gen MDC 4] 13. Provide pdfs of all revisions, 2 hardcopies of revised plan sheets, and 1 hardcopy of other documents. Pdfs must be uploaded using the form at: https:Hedocs.deq.nc.gov/Fonns/SW ProjectSubmittal Please note that this request for additional information is in response to a preliminary review. The requested information should be received by this Office prior to December 29, 2017, or the application will be returned as incomplete. The return of a project will necessitate resubmittal of all required items, including the application fee. If you need additional time to submit the information, please submit your request for a time extension to the Division at the contact below. The request must indicate the date by which you expect to submit the required information. The Division is allowed 90 days from the receipt of a completed application to issue the permit. The construction of any impervious surfaces, other than a construction entrance under an approved Sedimentation Erosion Control Plan, is a violation of NCGS 143-215.1 and is subject to enforcement action pursuant to NCGS 143-215.6A.
