A preliminary review of that information has determined that the application is not complete. The following information is needed to continue the stormwater review: 1. Provide a soils report that includes the SHWT elevation within the footprint of each SCM [sand filter MDC 1]. The soils report provided does not state a SHWT elevation. 2. Provide a completed supplement form for that sand filter, using the Supplement-EZ form at: https://deq.nc.gov/about/divisions/energy-mineral-land-resources/energy-mineral- land-permit-guidance/stormwater-bmp-manual 3. The WQv in the calcs is 603.47, but enough accumulated volume is not provided at that elevation for the WQv. The plans show it at 604.5. 4. Revise sht. 10 to specify that the berm rip rap, filter sand, and underdrain stone shall be washed. 5. Provide an easement around the SCM to a public right-of-way for access and maintenance. 6. Provide pdfs of the initial modification application package using the link in item 7 below. 7. Provide pdfs of all revisions, 2 hardcopies of revised plan sheets, and 1 hardcopy of other documents. If you need additional time to submit the information, please submit your request for a time extension to the Division at the contact below. The request must indicate the date by which you expect to submit the required information. The Division is allowed 90 days from the receipt of a completed application to issue the permit.
