Plastek Industries, Inc, Certificate of Coverage NCGO50002 Richmond County Dear Mr. Anderson: On September 14, 2023, Bailey Taylor and Denise Bruce from the Fayetteville Regional Office of the Division of Energy, Mineral and Land Resources (DEMLR), conducted a site inspection for the Plastek Industries, Inc. facility located at 1015 County Home Road, Richmond County, North Carolina. A.copy of the Compliance Inspection Report is enclosed for your review. Bobby Smith, Maintenance Technician Lead, was also present during the inspection and his time and assistance is greatly appreciated. The site visit and file review revealed that the subject facility is covered by NPDES Stormwater General Permit NCG050000 under Certificate of Coverage NCG050002. Permit coverage authorizes the discharge of stormwater from the facility to receiving water designated as Falling Creek (McDonalds Pond), class WS-III waters in the Yadkin-Pee Dee River Basin. As a result of the site inspection, the following deficiencies are noted: -Per Part B Section 3 of NCGO50000...The site map did not include the latitude and longitude of the outfalls and did not include the drainage areas for the site. - Per Part B Section 1 of NCG050000...The list of Responsible Parties did not have appropriate personnel, who still worked at the location, listed. - Per Part B Section 14 of NCGO50000...The Stormwater Pollution Prevention Plan had not been updated annually, as shown in the inaccurate information in the SWPPP. - For the above reasons, the Stormwater Pollution Prevention Plan has not been implemented, because these are required components of the SPPP. The site met all other requirements of the SPPP and the NCG050000 permit. Other Observations Please refer to the enclosed Compliance Inspection Report for additional comments and observations made during the inspection. Action Items -Immediately review and amend the facility's SPPP as needed and begin incorporating documentation pertaining to the annual update requirements of the permit. -Update the detailed site map to include all required components per the permit. Thank you for your attention to this matter. This office requires that the deficiencies, as noted above and detailed in the enclosed inspection report, be properly resolved. Failure to address the deficiencies could result in the issuance of a Notice of Violation, which is subject to a civil penalty assessment of up to $25,000 per day for each violation. Permit: NCGO50002 Owner-Facility:Plastek Industries Inc Inspection Date: 0911412023 Inspection Type:Compliance Evaluation Reason for Visit: Routine Inspection Summary: DEMLR staff met with Brian Anderson, Facility Engineering Manager and Bobby Smith, Maintenance Technician Lead on site on 9/14/2023 to conduct the compliance evaluation. During the evaluation,the SWPPP was reviewed and found to be missing accurate facility contact information. It was also discovered that the site map required revisions to include necessary components, such as;drainage delineation, and the latitude and longitude information. For these reasons, it is apparent the facility has not been reviewing and updating the SWPPP annually, and therefore the SWPPP has not been implemented appropriately.The facility has been conducting qualitative monitoring per the permit requirements and had all other necessary components of the SWPPP in place.This facility does not conduct any vehicle maintenance on site and therefore is not required to comply with analytical monitoring requirements until the first quarter of 2024 per the permit requirement. During the site inspection, all storage areas were observed, in addition to both outfalls and all loading and unloading areas.The site appeared to be clean and well maintained at the time of inspection.
