Inspection Summary:
Inspection of Brown sand mine was conducted on 08/24/2023 (9 am to 12:30 pm). Met with on-site person (Franklin Clark,
[e-mail], P: 910-379-5427) and explained to him the NCG02 permit requirements that were
missing in the NOI (notice of intent) received.Tony Harris, Operations Manager(P:910-990-7928,
[e-mail]) couldn't make it to the site on that day. Dewatering of this mine will be discharge
into Faith lake Inc. routed through a vegetative buffer from a proposed discharge point as indicated in the site map.
Following is the list of things that need to be submitted to NC-DEMLR for NCG02 permit issuance:
1. SWPPP(STORMWATER POLLUTION AND PREVENTION PLAN)was missing on the site and in the NCG02 NOI
application.
2. SCM (Stormwater Control Measure) clean out measures
3. Secondary Containment Plan was missing
4. Spill Prevention and Response Procedures was missing
5. Narrative Description of Stormwater BMPs was missing.
6. Employee Training, for facility personnel responsible for operations that have the potential to contaminate stormwater
runoff, was missing.
Though some of the constituent parts of SWPPP were available in the application or on site(e.g. site plan, mining permit, a
Pumping Operation and Monitoring (POM) Plan, Mine Safety and Health Administration (MSHA)documentation, etc. were
available). Refer to Part B of NCG02 permit requirements for constituent parts of a SWPPP at this link:

In addition, following was observed on the Brown sand mine site:
1. Silt-fencing was missing on the outer side of the birm-wall.
2. Secondary containment(interior lining)of the diesel tank on the site (picture attached)to be confirmed. Dyke wall around
diesel storage not present. If interior lining is missing, dykewall needs to constructed for the volume capacity of the diesel
stored plus sufficient freeboard to contain the 25-year, 24-hour storm event around the diesel tank.
It is recommended that the responsible part should review the overall NCG02 permit requirements as it applies to this site.
Issuance of permit shall be reviewed after completion of above tasks.
