Upon resubmittal, please upload Erosion Control Plan Close-out Documentation that is issued by same entity who issued
the E&SC plan approval - NCDEQ-DEMLR-Raleigh Regional Office (919)-791-4200. This is different from the NCG01
inspection report. In addition, Richard Kelly is the legally responsible person and his name goes on legally responsible
person line on the NOT Cert Form (as was done). If Brennon Fuqua is to esign eNOT, then Brennon Fuqua must wet sign
NOT Cert Form and prints name and title on the bottom line of the form.This also allows Brennon Fuqua to esign eNOT.
