The letter states that a hard copy needs to be submitted but what does this look like? Would this be a letter resolving the inconsistencies and clarifying those issues? Does the letter need to be notarized or would her signature suffice? Also the date on the letter says the response is due by 3/11/22. Did you mean 4/11/22? A preliminary in-depth review of that information has determined that the application is not complete and that additional information is needed. To continue the review, please provide the following additional information in a single, hard copy submittal package: 15A NCAC 02H.1045(3)(a)(ii): Please confirm the following: a. Section B.1 of the application lists the applicant as Carolina Power and Light Co. DBA Progress Energy Carolinas, Inc. However, a review of the DEQ database found Duke Energy Progress, LLC based on a name/ownership name change application that was accepted in June 2016. Please confirm that the applicant listed on the application is consistent with the DEQ database. b. The mailing address for the property owner listed in Section BA of the application is not consistent with the office mailing address on file with the Secretary of State. Please confirm the mailing address for property owner listed in Section BA of the application is consistent with the information on file with the Secretary of State. Please remember to confirm that any revised information is presented consistently throughout the application documents. All Title V Permit required reports or correspondence are to be signed by a RO. The specific responsibilities of the RO include: Signing and certifying all Title V submittals (including permit applications, reports, and annual compliance certifications); Assuring the necessary resources are committed to the facility to provide for operation in compliance with all regulatory and permit requirements; and, Verification of on -going compliance or resolving any non-compliance. Other Related information The NPDES Compliance or Water Programs Managers, their respective designees, or when unavailable and as last resort to avoid noncompliance, the Environmental Coordinators, may submit other reports, information, or correspondence to the Permit Issuing Authority where certification Is not requested by the agency, or is unnecessary per normal practice and/or precedent. Events and associated correspondence that qualify in this category may be variable and specific to the State regulatory agency with oversight and thus, specific examples cannot be given that would be accurate in all situations. Reporting and signatory practices as outlined by the direct permitting authority are to be followed. The following responsibilities have been assigned and authorized on behalf of the facilities' owners and operators. This was facilitated several years ago when Stormwater was still a part of Water Quality. Duke Energy sent a letter directly to Chuck Wakild and it came down to all the regional offices through Matt Matthews. The name change was done up in Raleigh correcting the name in BIMS. I was given a copy of the letter as Matt Matthews asked for my help in looking at some other Water Quality permits for proper facility naming. I'm attaching a copy of the letter for each of you. In order for this to just be a name change they would have actually had to just change their name on the SOS. They did provide the signature authority document, which is very important. This might be an actual new transfer to a new entity. Also, there is only "no fee" for the first transfer. We will transfer of the property and permit. I will look into it.
