Its official. I am literally too stupid to even fill out a simple permit renewal form, despite the fact I have literally filled out
hundreds over the past 25 years. I guess my age has finally caught up with me, and perhaps I suffer from early
dementia. Considering my rapid decline, you may want to retain someone younger and smarter in the future to assist
you with what used to be a simple straightforward process.

The "principal office mailing address' is what needs to match the mailing address on the application. The mailing
address is Section BA is the mailing address for the company that holds the permit, not necessarily the address for the
managing members.

You are confusing me .... which admittedly is not hard to do.
Please see the attached Annual Report for DWNR, LLC
The NC Secretary of State has BOTH addresses I have used on the renewal forms on record....
That report list the "Principal Office Address: as 310 Western Blvd.
That report also list the address for Company Officials (Don Williamson) as 2445 N. Marine Blvd.

I attempted to fill out the Renewal Form to match exactly the addressed on record with the Sec. of State to avoid this
very confusion.

It is not necessary for the mailing address and the project address to be the same. However, the official mailing address
that is on file on the North Carolina Secretary of State webpage for DWNR LLC is 310 Western Blvd. I must have
matching information to go forward and process the renewal. I must have an updated page one of the application
reflecting this information in Section B.4 of the application. You may send it as an email attachment if that works best. I
can carbon copy the N Marine Blvd on the bottom of the renewal when it is issued if we need the permit to go to that
address a well

The project street address is 310 Western Blvd. as shown on the Permit Renewal form in Section A3.

The Permittee signing official's (Mr. Williamson) address is 2335 N. Marine Blvd. as shown on the Permit Renewal form
in Section B4.
If you read the entire Annual Report, I believe you will note that the addresses included in the permit renewal form
exactly match that of the latest Annual Report.
I do not believe it necessary for a Permittee to have the same mailing address as the project address. In fact, I believe it
is usually the case that a permittee have an address different from the project. For the purpose of the Stormwater
Permit, I believe you need both (i.e. you need to know where the project is AND know where and how to contact the
permittee).

The Wilmington Regional Office received and accepted a State Stormwater Management Permit Renewal
Application for the subject project on March 9, 2022. A preliminary in-depth review of that information has
determined that the application is not complete and that additional information is needed. To continue the review,
please provide the following additional information in a single, hard copy submittal package:
1. 15A'NCAC 02H.1045(3)(ii): Please confirm the following:
a. The mailing address for the property owner listed in Section B.4 of the application is not
consistent with the office mailing address on file with the Secretary of State. The application lists
2445 N Marine Blvd while the Secretary of State lists 310 Western Blvd. Please confirm the
mailing address for property owner listed in Section BA of the application is consistent with the
information on file with the Secretary of State.
Please remember to confirm that any revised information is presented consistently throughout the application
documents. Please also remember to provide a hard copy of any updated documents per 15A NCAC
02H.1045(3).
The requested information should be received in this Office by March 28, 2022, or the application will be
returned as incomplete. If additional time is needed to submit the requested information, please email your
extension request prior to this due date to the email address provided below with a justification and a date of when
the requested information will be submitted, which shall be no later than 15 days past the original due date. Please
note that only two letters requesting additional information is allowed in the State Stormwater Program.for a
total of 30 days to submit the requested additional information. If the information is not satisfactorily provided
after either the second request or 30 days, the project will be returned. If the project is returned and you wish to
re -submit later, you will need to resubmit all required items at that time, including the application fee.

North Carolina General Statutes and the Coastal Stormwater rules require that this property be covered under a
