Staff review of the plans and specifications has determined that the project, as proposed, will comply with the Stormwater Regulations set forth in Title 15A NCAC 2H.1000. We are forwarding Permit No. SW8 080744 dated August 21, 2008, for the construction of the subject project. This permit shall be effective from the date of issuance until August 21, 2018, and shall be subject to the conditions and limitations as specified therein. Please pay special attention to the Operation and Maintenance requirements in this permit. Failure to establish an adequate system for operation and maintenance of the stormwater management system will result in future compliance problems. If any parts, requirements, or limitations contained in this permit are unacceptable, you have the right to request an adjudicatory hearing upon written request within thirty (30) days following receipt of this permit. This request must be in the form of a written petition, conforming to Chapter 150E of the North Carolina General Statutes, and filed with the Office of Administrative Hearings, P.O. Drawer 27447, Raleigh, NC 27611-7447. Unless such demands are made this permit shall be final and binding. This permit shall be effective from the date of issuance until August 21, 2018, and shall be subject to the following specified conditions and limitations: I. DESIGN STANDARDS This permit is effective only with respect to the nature and volume of stormwater described in the application and other supporting data. 2. This stormwater system has been approved for the management of stormwater runoff as described in Section 1.6 on page 3 of this permit. The stormwater control has been designed to handle the runoff from 29,782 square feet of impervious area. 3. The tract will be limited to the amount of built -upon area indicated on page 3 of this permit, and per approved plans. 4. All stormwater collection and treatment systems must be located in either dedicated common areas or recorded easements. The final plats for the project will be recorded showing all such required easements, in accordance with the approved plans. 5. The runoff from all built -upon area within the permitted drainage area of this project must be directed into the permitted stormwater control system. The stormwater management system shall be constructed in its entirety, vegetated and operational for its intended use prior to the construction of any built -upon surface. 2. During construction, erosion shall be kept to a minimum and any eroded areas of the system will be repaired immediately. 3. The permittee shall at all times provide the operation and maintenance necessary to assure the permitted stormwater system functions at optimum efficiency. The approved Operation and Maintenance Plan must be followed in its entirety and maintenance must occur at the scheduled intervals including, but not limited to: a. Semiannual scheduled inspections (every 6 months). b. Sediment removal. C. Mowins and revegetation of slopes and the vegetated filter. d. Immediate repair of eroded areas. e. Maintenance of all slopes in accordance with approved plans and specifications. f. Debris removal and unclogging of bypass structure, filter media, flow spreader, catch basins, piping and vegetated filter. g. A clear access path to the bypass structure must be available at all times Page 3 of 7 State Stormwater Management Systems Permit No. SW8 080744 4. Records of maintenance activities must be kept and made available upon request to authorized personnel of DWQ. The records will indicate the date, activity, name of person performing the work and what actions were taken. 5. The facilities shall be constructed as shown on the approved plans. This permit shall become voidable unless the facilities are constructed in accordance with the conditions of this permit, the approved plans and specifications, and other supporting data. 6. Upon completion of construction, prior to issuance of a Certificate of Occupancy, and prior to operation of this permitted facility, a certification must be received from an appropriate designer for the system installed certifying that the permitted facility has been installed in accordance with this permit, the approved plans and specifications, and other supporting documentation. Any deviations from the approved plans and specifications must be noted on the Certification. A modification may be required for those deviations. 7. If the stormwater system was used as an Erosion Control device, it must be restored to design condition prior to operation as a stormwater treatment device, and prior to occupancy of the facility. 8. Access to the stormwater facilities shall be maintained via appropriate easements at all times. 9. The permittee shall submit to the Director and shall have received approval for revised plans, specifications, and calculations prior to construction, for any modification to the approved plans, including, but not limited to, those listed below: a. Any revision to any item shown on the approved plans, including the stormwater management measures, built -upon area, details, etc. b. Project name change. C. Transfer of ownership. d. Redesign or addition to the approved amount of built -upon area or to the drainage area. e. Further subdivision, acquisition, lease or sale of all or part of the project area. The project area is defined as all property owned by the permittee, for which Sedimentation and Erosion Control Plan approval or a CAMA Major permit was sought. f. Filling in, altering, or piping of any vegetative conveyance shown on the approved plan. 10. The permittee shall submit final site layout and grading plans for any permitted future areas shown on the approved plans, prior to construction. 11. A copy of the approved plans and specifications shall be maintained on file by the Permittee for a minimum of ten years from the date of the completion of construction. 12. The Director may notify the permittee when the permitted site does not meet one or more of the minimum requirements of the permit. Within the time frame specified in the notice, the permittee shall submit a written time schedule to the Director for modifying the site to meet minimum requirements. The permittee shall provide copies of revised plans and certification in writing to the Director that the changes have been made. Page 4 of 7 State Stormwater Management Systems Permit No. SW8 080744 III. GENERAL CONDITIONS This permit is not transferable except after notice to and approval by the Director. In the event of a change of ownership, or a name change, the permittee must submit a formal permit transfer request to the Division of Water Quality, accompanied by a completed name/ownership change form, documentation from the parties involved, and other supporting materials as may be appropriate. The approval of this request will be considered on its merits and may or may not be approved. The permittee is responsible for compliance with all permit conditions until such time as the Division approves the transfer request. 2. Failure to abide by the conditions and limitations contained in this permit may subject the Permittee to enforcement action by the Division of Water Quality, in accordance with North Carolina General Statute 143-215.6A to 143-215.6C. 3. The issuance of this permit does not preclude the Permittee from complying with any and all statutes, rules, regulations, or ordinances, which may be imposed by other government agencies (local, state, and federal) having jurisdiction. 4. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions, the Permittee shall take immediate corrective action, including those as may be required by this Division, such as the construction of additional or replacement stormwater management systems. 5. The permittee grants DENR Staff permission to enter the property during normal business hours for the purpose of inspecting all components of the permitted stormwater management facility. 6. The permit may be modified, revoked and reissued or terminated for cause. The filing of a request for a permit modification, revocation and reissuance or termination does not stay any permit condition. 7. Unless specified elsewhere, permanent seeding requirements for the stormwater control must follow the guidelines established in the North Carolina Erosion and Sediment Control Planning and Design Manual. 8. Approved plans and specifications for this project are incorporated by reference and are enforceable parts of the permit. 9. The permittee shall notify the Division any name, ownership or mailing address changes within 30 days. Permit =TH a 21st day of August, 2008. Required Items Checklist, Page 1 of 1 OVERVIEW Background This report contains the approach and results of a Stormwater Analysis conducted for the proposed First Citizens Bank within the Town of Oak Island, Brunswick County, North Carolina. The project site is located at the intersection of East Oak Island Drive and McGlamery Street. The subject parcel is approximately 1.32 acres and contains an existing First Citizens Bank on the site. The site currently drains to existing drop inlets on the site or to the existing storm drains in the street which tic into the existing road drainage system. Sec pre -development drainage map in the appendix of this report Ior existing drainage areas and composite curve numbers. The site currently has a one story First Citizens Bank building and asphalt pavement for parking in the southwest corner of the site. These existing features will need to be demolished. There is open space to the east and north east of the existing building consisting of trees and grass which extends to Price Street. The existing impervious area on the site is 0.38 acres. The soils for this parcel are entirely comprised of Kureb fine sand (KrB). This soil is classified as hydrologic soil group A. CN values used for this report have been calculated based on the land cover and hydrologic soil group. A pre -development time of concentration of 5.0 minutes was utilized based on the size of the site and amount of existing impervious surface. Proposed Conditions The subject site is approximately 1.32 acres and proposes a 2,700 SF First Citizens Bank with a drive thru, entry walks and 17 associated parking spaces. The site will have two access drives off of McGlamery Street and one access drive off of Price Street, A total of 0.68 Acres of impervious area is proposed within the site limits which include all of the above mentioned items. A Stonntech Underground Detention System is proposed for Water Quantity and Water Quality to meet the Stormwater Management requirements as recommended by the Town of Oak Island. The time of concentration for all post -development drainage is 5.0-minutes. See post development drainage map in the Appendix of this report for drainage areas and composite curve numbers. Water Quality The proposed Stormtech System was sized to accommodate the volume associated with the first 1.5" of rainfall from all impervious surfaces on site. Based on a site area of 1.32 acres and a proposed impervious area of 0.68 acres, a storage volume of 3,741 cf was required. Based on hydraulic conductivity tests performed by Soil & Environmental Consultants (S&EC) at the location of the proposed Stonntech System and an onsite meeting with NCDWQ Soil Scientist Vincent Lewis, it was determined that an infiltration rate of 10 in/hr would be accepted by NCDWQ for the site. Given the agreed upon infiltration rate of 10 in/hr, the water quality volume of 3,741 cf is absorbed into the soil in approximately 0.1 days (well below the required 5 day maximum draw down time). In addition, as demonstrated in the routing calculations (Appendix D: Water Quantity Underground Storage Calculations), all runoff volume from the 1-yr/24-hour storm percolates into the soil and does not leave the Stonntech System. S&EC also evaluated the condition of the soil at the proposed bottom elevation of the Stormtech System (13.5 fmsl). The soil texture was determined to meet the media requirements for a 4" clean sand layer as required at the base of an infiltration trench. Water Quantity For on -site water quantity, a Stormtech Underground Storage System is proposed for reducing the post development peak discharge to pre development conditions for the 1-year 24-hour and the 10-year 24- hour storm events. Stormtech utilizes a system of perforated molded plastic chambers and a stone foundation for retention. ( The stornlwater enters the system via a manhole or inlet pipe and is stored within the retention system utilizing a control structure. See attached technical specifications in the Appendix of this report for Stornrtech details and specifications. The outlet control structure proposed for the proposed Stormtech System contains an intermediate orifice, overflow weir and outfall culvert. The intermediate orifice (3.5"xl9") is located approximately 1.2' above the bottom of the structure to control the 10-year, 24-hour storm event. An overflow weir is also proposed approximately 2.6' above the bottom of the riser to pass larger storm events. The outlet culvert is a 15" RCP. See the Outlet Control Structure Diagram located in Appendix F of this report for orifice locations, weir elevations, and inlet/outlet pipe configuration. The outlet control structure and the Storrtech System are proposed at an elevation of 13.5. Proposing the system at this elevation meets the 2' separation requirement from the bottom of the infiltration system to the seasonal high water table as described in the Geotechnical Report prepared by S&EC on June 11, 2008. For the pond routing, the NRCS Method with type III distribution was used via computer software called "PondPack" by I laestad Methods. Subsurface conditions at the site were investigated by performing three soil test borings at the approximate locations shown on the attached site plan. The test borings were located in the field by measuring distances and estimating right angles from existing roadways and property boundaries. As such, the indicated boring locations should be considered as approximate. The test borings extended to depths of 15 feet below existing ground surface, and all borings were sampled at selected intervals using standard penetration test procedures designated in ASTM D-1586. This report presents the findings of the investigation and our recommendations for site grading and foundation support based on these findings. PROJECT AND SITE DESCRIPTION The proposed project site is located at the intersection of McGlamery Street, Price Street and Yaupon Drive, in Yaupon Beach, North Carolina. The site is currently occupied by a mobile bank unit which we understand will be removed and a new bank structure constructed. Portions of the property are currently undeveloped and consist of grass covered areas. No detailed topographic information was available; however, based on our visual observations, the site appears to be relatively flat. We understand that the proposed project will involve construction of a single story branch bank building. The bank building will be constructed in the central portion of the site and will include three drive-thru teller windows. No detailed information concerning structural loads was available at this time; however, based on our experience with similar type projects, we anticipate that the structure will be supported on a combination of load bearing walls and interior columns. Wall loads are expected to be on Geotechnical and Construction Materials Testing Services First Citizens Bank June 23, 1998 Page: 2 the order of 2 to 3 kips per linear foot with column loads in the order of 25 to 40 kips. Additionally, we also anticipate that a vault area will be constructed which will likely have floor loads of approximately 1,000 psf. In addition to the proposed structure, parking and driveway areas will comprise the majority of the remainder of the lot. Access drives will be constructed on Price Street, McGlamery Street and Yaupon Drive. SUBSURFACE CONDITIONS A generalized subsurface profile prepared from the test boring data is attached to this report as Figure 2 to graphically illustrate subsurface conditions encountered at the different locations across the site. More detailed descriptions of the conditions encountered at the individual test borings are then presented on the attached test boring records. The subsurface profile encountered at this site consists of a very thin layer of topsoil and root mat extending no more than l to 2 inches below existing grade. Underlying the near surface topsoil, the borings encountered very loose to medium dense fine to medium sands. Penetration resistances within the sands ranged from 3 to 12 blows per foot (bpf). Typically, the sands were somewhat looser within the upper 2 to 3 feet with penetration resistances typically increasing below that depth. Groundwater was encountered at approximately 5.5 feet below existing ground surface at the time of boring completion. However, it should be understood that regional groundwater levels will fluctuate with seasonal and climatic changes and may be different at other times. RECOMMENDATIONS The following recommendations are made based upon a review of the attached test boring data, our understanding of the proposed construction, and past experience with similar projects and subsurface conditions. If structural loads will be significantly heavier than those anticipated, we would appreciate an opportunity to review and comment on the recommendations of this report so that they may be confirmed, extended, or modified as necessary. Additionally, should subsurface conditions adverse to those indicated by this report be encountered during construction, those differences should be reported to us for review and comment. Site Grading Considerations. Site grading should initially begin with the removal of all root mat and topsoil from those areas designated for construction of the new building and adjoining parking facilities. Once stripping is completed, we recommend the site be densified utilizing a heavy vibratory roller. The vibratory roller should have a minimum static weight of 10 tons and should make at least 6 overlapping passing in alternating perpendicular directions across the site. The equipment should be operated at First Citizens Bank June 23,1998 Page: 3 a walking speed with the vibrator engaged during the densification operations. Any areas which yield or pump during the densification operation should be repaired in the field based on recommendations provided by a geotechnical engineer. However, based on our experience with similar type soils, we anticipate that these soils will readily density during vibratory rolling. Time on -site soils with the exception of topsoil will be suitable for reuse as structural fill beneath both building and pavement areas. If any off -site materials are needed to achieve the desired grades, those materials should consist of granular soils. All fill placed on the site should be compacted to a minimum of 95% of the standard Proctor maximum dry density except in the final foot beneath pavements and floor slabs where this requirement should be increased to 98% of the standard Proctor maximum. We recommend that field density testing be performed during the compaction operations to verify that the required degree of compaction is being achieved. Foundation Support Considerations. The proposed structure may be supported on shallow spread footings or continuous wall footing foundations designed for a maximum allowable bearing pressure of 2,000 psf for bearing in existing soils or on properly and wall footings should have minimum dimensions compacted structural fill. Column of 24 and 16 inches, respectively, and all footings should bear at least 12 inches below finished grade for frost protection. We recommend that all footing excavations be inspected by a geotechnical engineer to verify that subgrade soils have adequate strength to support the anticipated loads. Any areas with soft soils can be evaluated and repaired as recommended at the time of field inspection. Based on the results of the soil test borings, we do not anticipate that repairs will be required provided that the site is properly densified during the initial grading operations. Based on our experience with we anticipate this design will limit total and similar projects and subsurface conditions, differential settlements to approximately 1 inch and 0.5 inches, respectively. It should be understood that somewhat higher settlements could occur at the vault structure depending on specific vault loads. As such, we recommend that some flexibility be constructed in joints where the vaults will be located to accommodate any additional minor differential settlement. We anticipate that some uplift loading will also occur for foundations supporting the proposed drive-thru tellers. At these locations, the foundation should be embedded to sufficient depth and have adequate size to provide the necessary resistance to uplift loading. We recommend utilizing a unit weight of 110 pounds per cubic foot for granular materials properly compacted above the foundations. As groundwater exists at a depth of 5.5 feet below existing grade which will complicate foundation construction should any of the canopy footings need to bear at deeper depths. We recommend that consideration be given to keeping the foundation bearing level above 5.5 feet and enlarging the dimensions if additional deadweight is needed to resist uplift loads. if excavations must extend into the groundwater more than about one foot, well points likely will be needed to control groundwater due to the permeable character of the granular materials which underlie this site. No detailed information concerning traffic volumes was available at this time, nor was detailed laboratory testing completed to enable us to perform a final pavement design. However, based on our experience with similar type soils, we anticipate that a design CBR value of 10 percent can be utilized for calculating a pavement section. Additionally, similar type projects in the area have utilized a pavement section consisting of 2 inches of asphalt overlying 6 inches of CABC base course stone in areas such as the car parking stalls and a section consisting of 2 to 3 inches of asphalt over 8 inches of base course stone in areas subject to channelized car traffic and occasional truck traffic. We recommend that the subgrade of all pavement areas be proofrolled and any yielding areas be identified and repaired during the initial grading operations. Additionally, all subgrade soils should be recompacted to a minimum of 98% of the standard Proctor maximum dry density immediately prior to base course stone placement. We recommend all pavement areas be properly graded to promote run-off of water and prevent ponding of water on the pavement surface which can lead to eventual saturation of the subgrade soils and a loss of pavement support. SUMMARY In summary, subsurface conditions at this site appear favorable for the proposed construction. Because the near surface sands are very loose to loose, we recommend that the soils be densified utilizing a vibratory compactor during the initial grading operations. Following proper densification of the near surface sands, the structure can be supported on shallow spread footings and wall footing foundations designed for a maximum allowable bearing pressure of 2,000 pounds per square foot.
