The Wilmington Regional Office received a Stormwater Management Name/Ownership Change Application for the subject
project on August 11 2015
Notice of Inspection
On September 4, 2015 the Wilmington Regional Office inspected the subject project and found it to be in compliance with
its stormwater permit The inspection report is attached
Request for Additional Information
A preliminary review of that information has determined that the application is not complete The following information is
needed to continue the stormwater review The transfer has been drafted and can be issued fairly quickly once we receive
these administrative items
1 Ownership The proposed permittee is Lumina Station 11 Retail Condo Owners Association Inc , but the deed
submitted (BK5878/PG2202 2206) shows that Lumina Station Commercial, LLC owns the property Please
submit documentation that the proposed permittee owns the property so that they can become the permittee
2 Signature Authority Mr Chad Carlson has signed the transfer form on behalf of the Condo Association We just
need some documentation for the file that he is the current Vice President (The Secretary of State s most recent
document is from 2000 since Condo Associations do not have to file a report each year)
Please keep in mind that changing one number may change other numbers and require the calculations, supplements and
other supporting documentation to be updated Verify all numbers are correct to ensure consistency in the application
documents
Please note that this request for additional information is in response to a preliminary review The requested information
should be received in this Office prior to October 5, 2015 or the application will be returned as incomplete The return of a
project will necessitate resubmittal of all required items, including the application fee
If you need additional time to submit the information please email or fax your request for a time extension to the Division at
the address and fax number at the bottom of this letter Please note that a second significant request for additional
information may result in the return of the project
The construction of any impervious surfaces, other than a construction entrance under an approved Sedimentation Erosion
Control Plan is a violation of NCGS 143 2151 and is subject to enforcement action pursuant to NCGS 143 215 6A

1 Due to the large public demand for file access, we request that you call at least a day in
advance to schedule an appointment for file review so you can be accommodated

2 You must specify files you want to review by facility name or incident number, as
appropriate The number of files that you may review at one appointment will be limited to
five
3 You may make copies of a file when the copier is not in use by the staff and if time permits
There is no charge for 25 or less copies , cost per copy after the initial 25 is 2 5 per copy
Payment is to be made by check, money order, or cash in the administrative offices
4 Files must be kept in the order you received them Files may not be taken from the office
No briefcases, large totes, etc are permitted in the file review area To remove, alter,
deface, mutilate, or destroy material in public files is a misdemeanor for which you can be
fined up to $500 00
5 In accordance with GS 25-3-512, a $25 00 processing fee will be charged and collected for
checks on which payment has been refused
6 The customer must present a photo ID, sign -in, and receive a visitor sticker prior to
reviewing files

The report summarizes the findings of a recent inspection of the project's stormwate ;nntrols conducted on
June 29 2000 to determine compliance with Stormwater Permit Number SW8 CR010') -ssued on June 28, 1999
A summary of the findings and comments are noted under Section 6 of the report Please inform this
Office in writing before July 30, 2000 of the specific actions that will be undertaken and the time frame required

3 Compliance Issues
The diversion boxes are missing the weir wall, which directs runoff to the infiltration trenches
Water had accumulated in DB 414, so it is possible that it has overtopped the weir and hidden it
from view
b The approved diversion box design indicates that the overflow side is under a manhole cover No
manholes were seen adjacent the grated inlet
c Standing water was seen in the diversion box
The pavement grading in the area of the rear entrance off Southland Avenue is questionable It
appears that some runoff will not make it into the system
Required Actions
a
Please inspect the diversion boxes again to make sure they comply with the design as shown on
the approved plans If there is a new design, please submit a permit modification
Please investigate the cause of the standing water
Please report on tn— pa mrt grading in the southeast corner anti venfy that all runoff wAl be
directed into the system
The approved calculations indicate that each of the trenches will drawdown in about 4 hours
