A preliminary in-depth review of that information has determined that the application is not complete and that additional information is needed. To continue the review, please provide the following additional information in a single, hard cony submittal Package: 1. 15A NCAC 02H.1045(3)(a): Please include the following: (i) perm ittee name and contact information (ii) owner name, title, and contact information in Section B. 1, B.2, and B.3 of the application 2. 15A NCAC 02H.1045(3)(a)(ii) or (iii): Please confirm the following: a. There is no contact information for the applicant listed in Section B.6 of the application. Please provide complete contact information for the applicant, including phone number and e-mail address, in Section B.6 of the application. b.
