Attached are the Scanned copies of Eagle Rock Concrete's corrected SW Permit Summaries. Please see the
Permit Billing Contact section for updated all of our billing contacts on the eight permits we have on our
facilities. That contact should be me. I would also like to ask if it would be possible to change all of our billing
months to the same month. This would aid us in making it easier to expect and pay them together. If not, we
will continue to pay as billed.

Good morning. While going through the eDMR Registration six step process step 1, verifying information on
the Stormwater Permit Summary for the above referenced COC, I see our invoice status is overdue. Where
was the invoice mailed? If to the Mount Herman Rd. address, as listed under the facility contact, it would have
been returned due to the fact that we have no mailbox at this address, All Eagle Rock Concrete invoices for all
eight locations should be mailed to our corporate address, Eagle Rock Concrete, 8311 Bandford Way, Ste 003,
Raleigh, NC 27615, attn; Dexter. I will make these corrections on each of our plants Permit Summary, scan
and forward each of them to you via email as requested in the first step.
The NCG 4's are 140425,140427,140443,140451,140366,140362,140293 & 140320.
Meanwhile, would you please send me a copy of the missing invoice for the COC 140425 and I will promptly
submit payment for it.
