Skvarla, III, Secretary Effective August 1, 2013 the State Stormwater program has been transferred from the Division of Water Quality (DWQ) to the Division of Energy, Mineral and Land Resources (DEMLR). All previous references to DWQ will remain in older stormwater permits issued prior to August 1, 2013 until they are modified. Please note that plan revisions and future correspondence regarding this permit will now reference DEMLR as the Division responsible for issuance of the permit On November 14, 2013, the Wilmington Regional Office received a request to revise the approved plans for Stormwater Management Permit Number SW8 080822 with additional information received on November 21, 2013. The revisions include the re -design of the proposed impervious area. This project was originally permitted as a Multipurpose Community Building and it will now be a Community Center. The change in the use of the property results in the reallocation of built upon area as follows: 8/9/2011 Permit 11/22/2013 Plan Revision Proposed BUA 36,134 37496.8 Future BUA 1,965 602.2 Existing BUA 0 0 Total BUA 38,099 38,099 It has been determined that a formal permit modification is not required for the proposed changes. We are forwarding you an approved copy of the revised plans for your files. Please replace the old approved plan sheets with the new ones. Please be aware that all terms and conditions of the permit issued on August 29, 2008, remain in full force and effect. Please also understand that the approval of this revision to the approved plans for the subject State Stormwater Permit is done on a case -by -case basis. Any other changes to this project must be approved through this Office prior to construction. The issuance of this plan revision does not preclude the permittee from complying with all other applicable statutes, rules, regulations or ordinances which may have jurisdiction over the proposed activity, and obtaining a permit or approval prior to construction. This maintenance record will be kept in a log in a known set location. Any deficient BMP elements noted in the inspection will be corrected, repaired or replaced immediately. These deficiencies can affect the integrity of structures, safety of the public, and the removal efficiency of the BMP. Important operation and maintenance procedures: RECFIVED — Stable groundcover will be maintained in the drainage area to reduce the se d tent load to the permeable pavement. AUG 19 kn — The area around the perimeter of the permeable pavement will be stabilized and mowed, with clippings removed. $Y: — Any weeds that grow in the permeable pavement will be sprayed with pesticide immediately. Weeds will not be pulled, since this could damage the fill media. — Once a year, the permeable pavement surface will be vacuum swept. — At no time shall wet sweeping (moistening followed by sweeping) be allowed as a means of maintenance. — There shall be no repair or treatment of Permeable Pavement surfaces with other types of pavement surfaces. All repairs to Permeable Pavement surfaces must be accomplished utilizing permeable pavement which meets the original pavement specifications. — Concentrated runoff from roof drains, piping, swales or other point sources, directly onto the permeable pavement surface shall not be allowed. These areas must be diverted away from the permeable pavement. Grass height will not exceed six inches at any time.. 2. Accumulated sediment and trash will be removed from the collection system as necessary. Svales and ditches will be reseeded or sodded following sediment removal. 3.. Eroded areas of swales and ditches will be repaired and reseeded. Swales and ditches will be revegetated as needed based on monthly inspections. 4. The collection system, including catch basins, curb cuts, velocity reduction devices, and piping, will be inspected monthly or after every significant runoff producing rainfall event. Trash and debris will be cleared away from grates, curb cuts, velocity reduction devices, and piping. The collection system may not be altered in any way without prior approval from NCDENR Division of Water Quality.
