Also, I was able to locate a Wet Detention Basin Operation and Maintenance Agreement in the project folder for the Holiday Inn Express — Plymouth. Therefore, this particular form doesn't need to be completed. The only form that still needs to be completed for the renewal process is the Designer's Certification (enclosed) and it does need to be completed by a registered professional engineer in the State of North Carolina. The original consulting engineer for the permit was Jarvis Associates (here in Washington) in case your company would like to contact them to see if they ever completed a Designer's Certification for the Holiday Inn Express in Plymouth. They (Jarvis Associates) may have this form on file. If they don't, either Jarvis Associates or another engineer of your company's choice will need to complete this enclosed form for the renewal process to begin. The property is accessed from US Hwy 64. It is approximately 1.5 miles west of the intersection of Hwy 64 and NC32. Type of Development The project includes the construction of a hotel and parking lot. The proposed roadway is also to be developed at this time. At some future date the other four lots will be commercial developments. Existing Drainage Patterns: a. The property has ditches following the northern and southern boundaries. The property drains from the center or high point into both ditches. High Density Runoff Calculations Total Site Area Total Impervious Area Offsite Drainage Area Other Impervious Areas Total Project Area Proposed Drainage Patterns: = 6.97 acres = 5.52 acres = 0.00 acres = 0.00 acres = 6.97 acres a. The proposed hotel site is to sheet flow across the parking lots into the curbing which directs the water into the detention pond. b. A storm drainage system has been designed to collect all future site development as well as runoff from the roadway. The roadway has been designed to allow all runoff into this system. c. A portion of the site water will drain in to the catch basin located at station 1+68.40 and then be directed through the storm drainage system into the detention pond. d. Future lots are to be graded to drain into proposed drainage system and all areas have been used to determine sizing of the pond. H. Phone No. 974-0439 Monthly, or after every runoff producing rainfall event, whichever comes first: A. Inspect the trash rack; remove accumulated debris, repair/replace if it is not functioning. B. Inspect and clear the orifice of any obstructions. If a pump is used as the drawdown mechanism, pump operation will be checked. A log of test runs of the pump will be kept on site and made available to DWQ personnel upon request. C. Inspect the pond side slopes and grassed inlet swales; remove trash, and repair eroded areas before the next rainfall event. D. If the pond is operated with a vegetated filter, the filter will be checked for sediment accumulation, trash accumulation, erosion and proper operation of the flow spreader mechanism. Repairs/cleaning will be done as necessary. Quarterly (Four times a year): A. Inspect the collection system (ie. catch basins, piping, grassed swales) for proper functioning. Accumulated trash will be cleared from basin grates, basin bottoms, and piping will be checked for obstructions and cleared as required. B. Pond inlet pipes will be checked for undercutting. Riprap or other energy dissipation structures will be replaced, and broken pipes will be repaired. III. Semi-annually (twice a year): A. Accumulated sediment from the bottom of the outlet structure will be removed. B. ec at vanous po t removal is required when the depth is reduced to 75 % of the original forebay design depth of 3 ft. C. Grassed swales, including the vegetated filter if applicable, will be re -seeded twice a year as necessary. POND MAINTENANCE REQUIREMENTS :.PAGE 2 IV. Annually (Once a year): A. The pond depth will be checked at various points. Sediment removal is required when the depth is reduced to 75 % of the original design depth or 3', whichever is greater. Design depth 5.5' measured vertically from the orifice down to the pond's bottom. Sediment must be removed to at least the original design depth. V. General: A. Mowing of the side slopes will be accomplished according to the season. Maximum grass height will be 6". B. Cattails are encouraged along the pond perimeter, however they will be removed when they cover more than 1/2 the surface area of the pond. The best time to cut them is at the end of the growing season, in November. Environmentally sensitive chemicals for use in killing cattails can also be used. Contact your nearest Agricultural Extension Agent. C. The orifice/pump is designed to draw down the pond in 2-5 days. If drawdown is not accomplished in that time, the orifice will be replaced with a larger or smaller orifice. Slow drawdown may be attributed to a clogged system. The source of the clogging will be found and eliminated. D. All components of the detention pond system will be kept in good working order. Repair or replacement components will meet the original design specifications as per the approved stormwater plan. If previously approved components are determined to be ineffective, the component must be redesigned and/or replaced. VI. DESCRIPTION OF PROJECT Location of Project This site is located within the city limits of Plymouth, NC, and the county of Washington. The property is accessed from US Hwy 64. It is approximately L5 miles west of the intersection of Hwy 64 and NC32. The proposed hotel site is to sheet flow across the parking lots into the curbing which directs the water into the detention pond. b. A stone drainage system has been designed to collect all future site development as well as runoff from the roadway. The roadway has been designed to allow all runoff into this system. c. A portion of the site water will drain in to the catch basin located at station 1+68.40 and then be directed through the stonn drainage system into the detention pond. d. Future lots are to be graded to drain into proposed drainage system and all areas have been used to determine sizing of the pond. A:: i'- -- -- EROSION AND SEDIMENTATION CONTROL A. DESCRIPTION OF PROJECT Location of Project This site is located within the city limits of Plymouth, NC, and the county of Washington. The property is accessed from US Hwy 64. It is approximately 1.5 miles west of the intersection of Hwy 64 and NC 32. Type of Development The project includes the construction of a hotel and parking lot as well as site grading of the out parcels. The proposed roadway is also to be developed at this time. At some future date the other four lots will be commercial developments. Earth Disturbing Activities: At this time the site has field grass growing. For site improvements the project will be stripped of the top soil for placement of fill to the proposed grades. A drainage pond is to be constructed as well as placement of storm drainage pipe. Also utilities will be extended onto the site including water and sewer. After construction of the utilities the roadway will be graded, stoned, and paved. The additional sites are to be graded to drain toward the proposed roadway. Existing Drainage Patterns: a. The property has ditches following the northern and southern boundaries. The property drains from the center or high point into both ditches. Proposed Drainage Patterns: a. The proposed hotel site is to sheet flow across the parking lots into the curbing which directs the water into the detention pond. b. A storm drainage system has been designed to collect all future site development as well as runoff from the roadway. The roadway has been designed to allow all runoff into this system. c. A portion of the site water will drain in to the catch basin located at station 1 +68.40 and then be directed through the storm drainage system into the detention pond. d. Future lots are to be graded to drain into proposed drainage system and all areas have been used to determine sizing of the pond. B. EROSION AND SEDIMENTATION CONTROL MEASURES 1. A copy of the latest approved soil erosion and control plan must be on file at the job site. Timely and proper attention shall be required for prevention of offsite erosion and sedimentation. Various levels of erosion control are proposed in the plan. If it becomes evident that additional erosion and sedimentation control measures are needed they shall be installed immediately. A permanent ground cover, sufficient to restrain erosion, must be provided within the shorter of 15 working or 30 calendar days after completion of the construction or development. 2. A buffer zone, sufficient to restrain sedimentation, must be maintained between the land - disturbing activity any adjacent property or water courses. Maintain / Do Not Disturb existing vegetative cover in areas of no construction. 3. Install a gravel construction pad at the egress of US Hwy 64, as noted on the plans. The pad will be a minimum of 50 It long, 30 ft. wide and 6" deep of washed 1-2" stone. 4. All disturbed areas will be seeded within 30 days following construction in accordance with the seeding schedule as shown on the plans. Rye grass is not to be used as ground cover. 5. Temporary sediment control primarily will consist of a silt fence along the northemly property line to prevent silt from leaving the site at the exiting ditch. Also inlet protection around all storm drainage will be used. The proposed site sheet drains to the detetion facility which will act as a temporary sediment basin such to detain all sediments leaving the site until a substantial ground cover is obtained. This pond is to be cleaned as needed after every major land disturbing rainfall event. C. TIME SEQUENCE OF OPERATIONS Construction sequence is as noted on the sedimentation and erosion control plan sheet. Construction is planned to begin October 1998. . if) FINANCIAL RESPONSIBILITY/ OWNERSHIP FORM SEDIMINTATION POLLUTION CONTROL ACT No person may initiate a land -disturbing activity on one or more acres an covered by the Act before this form and an acceptable erosion and sedimentation control plan have been completed and approved by the Land Quality Section, N.C.
