A preliminary review of that information has determined that the application is not complete. The following information is needed to continue the stormwater review: Sheet C201 of the Plans set states that no CAMA wetlands will be disturbed as part of this project. Please clarify if any other (404/401) wetlands are affected by this project. Provide two copies of signed and sealed revised sheet. The requested information should be received in this Office prior to February 23, 2011, or the application will be returned as incomplete. The return of a project will necessitate resubmittal of all required items, including the application fee. If you need additional time to submit the information, please mail, email or fax your request for a time extension to the Division at the address and fax number at the bottom of this letter. The request must indicate the date by which you expect to submit the required information. The construction of any impervious surfaces, other than a construction entrance under an approved Sedimentation Erosion Control Plan, is a violation of NCGS 143-215.1 and is subject to enforcement action pursuant to NCGS 143-215.6A. Holdings PO Box 120, Kitty Hawk, NC, 27949 C ®ENR February 2, 2011 Existing Conditions: G. Holdings, LLC owns a 2.06-acre site in Corolla consisting of 2 parcels located at this intersection of Ocean Trail and Malia Drive. The Natural Resource Conservation Service Soil Survey for Currituck County has mapped the soils on the portions of the site selected for development to consist primarily of Corolla Fine Sand (CoB), Newhan-Corolla Complex (NhC), and Osier Fine Sand (Os). Infiltration rates for these sandy soils typically exceed 20 inches per hour, with the exception of Osier Fine Sand, which often contains a limiting layer on the surface that is estimated to be between 6.0 — 20 inches per hour. The site varies in elevation, with the majority of the area between 4.0 and 9.0 feet above mean sea level (NAVD 1988). Observed water levels within the pond are ±1.5 msl. The property is within a flood zone of "AE-5" and "X". The site contains native grasses, scattered trees and a gravel parking area. Proposed Development: Stormwater Management permit SW7091003 was issued on 10/22/2009 to I.G. Holdings, LLC for a similar but larger project on three parcels, which includes the two within this application. The low -density permit included a pocket of high density and provided infiltration basins for stormwater management of those areas. The new proposal for a facility will include a 27-hole miniature golf course and "spin zone", but will not include an arcade or go-cart track. Infrastructure will also include a ticket booth, bathrooms, parking lot and walkways. Carolina Water Service will supply the project with water and sewer service. NCDENR Permitting Compliance: The site is being developed per the High Density Stormwater Management Option, meeting the requirements for the 20 coastal counties. Three infiltration basins have been provided for the site to manage runoff. A brief summary of these basins is as follows: Infiltration Basin #1: Runoff from the much of the parking lot as well as portions of the pavilion rooftop is directed to a flume located along the Malia Drive entrance. A low -point (elevation = 8.90) is located at the flume. When the basin fills to the top of basin elevation (9.00), runoff will by-pass the basin by remaining in the gutter until overtopping the adjacent high point elevation of 9.0 (located on the east end of the adjacent valley gutter). This excess runoff will then be directed to a vegetated filter / overflow swale along the Malia Drive right-of-way for additional storage & filtration before being entering the Malia Drive drainage system Land Planning — Engineering — Surveying — Environmental — Construction Management P.O. When the basin fills to the storage elevation (9.0), runoff will overflow into a drain basin set adjacent to the contributing flume. A pipe will then convey the runoff into a vegetated filter / overflow swale which will discharge into the adjacent low areas within the NCDOT unimproved right-of- way. Miniature Goff Course Drainage: The miniature golf course is mounded in the center and slopes gently to the perimeter. Due to the nature of the construction of a golf course, most of the runoff sheet flows from narrow impervious surfaces (golf holes and connecting pathways) and immediately infiltrates into adjacent sandy (highly permeable) surfaces or into adjacent water features that maintain and slowly evaporate the runoff. The surface area of the golf course is 36,368 sf, which excludes a ten foot perimeter around the site that will be left as open and vegetated. The holes and pathways account for 13,969 sf of coverage within the course, while the water features account for 3,253 sf. The remaining 19,146 sf is landscaped, pervious surfaces. When conservatively accounting for only 1/3 (6,387 sf) of the total pervious surface as receiving runoff & providing infiltration, this area is capable of infiltrating 21,290 cf of runoff in a two-hour period, or more than eleven times the required 1,825 cf of runoff resulting from 1-1/2" of rainfall. Three basins have also been provided along the perimeter of the basin to provide capture of any runoff that may occur beyond the infiltrative abilities of the pervious surfaces. The perimeter swale provides additional opportunity for infiltration of runoff. Additionally, a combination of graded basins and swales along the southwest edge of the course can intercept any runoff that may leave the course. This has been designed to hold an additional 2,110 cf of runoff. In the unlikely event runoff would fill this basin, the southern berm is designed to an elevation = 5.5, proving overflow into an adjacent vegetated filter. Holdings, LLC December 21, 2010 Existing Conditions: I.G. Holdings, LLC intends to construct a new recreational park to include a 36-hole miniature golf course and spin zone and arcade on two parcels in Corolla. The total site area is approximately 2.06-acres. The site includes 2 of the 3 parcels included within the previously approved "Corolla Fun Park" (SW7091003, issued 10/22/2009). The "Corolla Fun Park" was a low density permit that included a pocket of high density. The third parcel was not obtained and the project was not constructed. This soils report includes information from the previously approved project. The Natural Resource Conservation Service Soil Survey for Currituck County has mapped the soils on the portions of the site selected for development to consist primarily of Corolla Fine Sand (CoB), Newhan-Corolla Complex (NhC), and Osier Fine Sand (Os). Infiltration rates for these sandy soils typically exceed 20 inches per hour, with the exception of Osier Fine Sand, which often contains a limiting layer on the surface that is estimated to be between 6.0 — 20 inches per hour. The site varies in elevation, with the majority of the area between 4.0 and 9.0 feet above mean sea level (NAVD 1988). An existing non -jurisdictional manmade pond (t 4.68-acre) is located adjacent to the site and observed water levels within the pond are t1.5' msl. The property is within a flood zone of "AE-5" and "X". The site contains native grasses, scattered trees and a gravel parking area. Soils investigations were performed by Albemarle & Associates. These borings were performed using a hand auger to a depth or four to five feet below the existing surface. The borings confirmed that the soils matched the Soil Conservation Service description for the soil types noted above. Seasonal high water table appears to vary with proximity to the pond as expected. The maximum ESHWT in the vicinity of the infiltration basin is 4.3' above msl, with lower ESHWT elevations anticipated closer to the pond. The results of these borings are attached to this report. Infiltration Basin A series of infiltration basins have been chosen for this site due to the excellent suitability of the soils, available space and contributing drainage area. In order to meet a two -foot separation to the ESHWT, the bottom of the basins serving the parking lot and buildings are being limited to 6.5' above MSL. A series of smaller basins have also been provided to accommodate runoff from the golf course, although it is anticipated that this runoff will infiltrate directly into the pervious areas immediately adjacent to the walkways and golf course. The basins are closer to the pond were the water table is lower due to the low pond levels. The minimum basin elevation in this area is set at 4.5, allowing 2'seperation from a ESHWT of 2.5 previously determined in the low areas adjacent to the pond. The runoff from all built -upon area within the drainage area of the high density portion of the project, must be directed into the permitted stormwater control system. 6. Within the low density portions of the permitted project, all roof drains shall terminate at least 50 feet from the mean high water mark of surface waters. The permittee shall submit to the Director and shall have received approval for revised plans, specifications, and calculations prior to construction, for any modification to the approved plans, including, but not limited to, those listed below: a. Any revision to the approved plans, regardless of size. b. Project name change. C. Transfer of ownership. d. Redesign or addition to the approved amount of built -upon area. e. Further subdivision, acquisition, or sale of all or part of the project area. During construction, erosion shall be kept to a minimum and any eroded areas of the system will be repaired immediately. 11. Upon completion of construction, prior to issuance of a Certificate of Occupancy, and prior to operation of this permitted facility, a certification must be received from an appropriate designer for the system installed certifying that the permitted facility has been installed in accordance with this permit, the approved plans and specifications, and other supporting documentation. Any deviations from the approved plans and specifications must be noted on the Certification. 12. If the stormwater system was used as an Erosion Control device, it must be restored to design condition prior to operation as a stormwater treatment device, and prior to occupancy of the facility. 13. Permanent seeding requirements for the stormwater control must follow the guidelines established in the North Carolina Erosion and Sediment Control Planning and Design Manual. 14. This permit shall become void unless the facilities are constructed in accordance with the conditions of this permit, the approved plans and specifications, and other supporting data. 15. The permittee shall at all times provide the operation and maintenance necessary to assure that all components of the permitted stormwater system function at optimum efficiency. The approved Operation and Maintenance Plan must be followed in its entirety and maintenance must occur at the scheduled intervals including, but not limited to: a. Semiannual scheduled inspections (every 6 months). b. Sediment removal. C. Mowing and re -vegetation of side slopes. d. Immediate repair of eroded areas. e. Maintenance of side slopes in accordance with approved plans and specifications. f. Debris removal and unclogging of structures, orifice, catch basins and piping. g. Access to all components of the system must be available at all times. 16. Records of maintenance activities must be kept by the permittee. The records will indicate the date, activity, name of person performing the work and what actions were taken. 17. The permittee shall submit to the Division of Water Quality an annual summary report of the maintenance and inspection records for each BMP. The report shall summarize the inspection dates, results of the inspections, and the maintenance work performed at each inspection. The facility must be in compliance with all permit conditions. Any items not in compliance must be repaired or replaced to design condition prior to the transfer. Records of maintenance activities performed to date will be required. 19. Prior to the subdivision and/or sale of this project, in whole or in part, the following deed restrictions must be recorded with the Office of the Register of Deeds: a. The following covenants are intended to ensure ongoing compliance with State Stormwater Management Permit Number SW7091003, as issued by the Division of Water Quality under NCAC 2H.1000. b. The State of North Carolina is made a beneficiary of these covenants to the extent necessary to maintain compliance with the Stormwater Management Permit. C. These covenants are to run with the land and be binding on all persons and parties claiming under them. d. The covenants pertaining to stormwater may not be altered or rescinded without the express written consent of the State of North Carolina, Division of Water Quality. e. Alteration of the drainage as shown on the approved plans may not take place without the concurrence of the Division of Water Quality. f. An engineered system will be required to collect and treat the runoff from all built -upon area associated with the project, including that area permitted under the low density option. i. Filling in or piping of any vegetative conveyances (ditches, swales, etc.) associated with this development, except for average driveway crossings, is strictly prohibited by any persons. j. The built -upon areas shall be located a minimum of 50 feet landward of all perennial and intermittent surface waters. 20. A copy of the recorded deed restrictions must be submitted to the Division within 30 days of the date of recording the plat, and prior to selling lots. The recorded copy must contain all of the statements above, the signature of the Permittee, the deed book number and page, and the stamp/signature of the Register of Deeds. III. GENERAL CONDITIONS This permit is not transferable to any person or entity except after notice to and approval by the Director. In the event there is either a desire for the facilities to change ownership, or there is a name change of the Permittee, a completed "Name/Ownership Change Form" must be submitted to the Division of Water Quality, signed by the parties involved, along with the applicable documents as listed on page 2 of the form. The project must be in good standing with DWQ. The approval of this request will be considered on its merits and may or may not be approved. The permittee is responsible for compliance with all of the terms and conditions of this permit until such time as the Director approves the transfer request. The issuance of this permit does not prohibit the Director from reopening and modifying the permit, revoking and reissuing the permit, or terminating the permit as allowed by the laws, rules and regulations contained in Session Law 2008- 211, Title 15A NCAC 21-1.1000, and NCGS 143-215.1 et.al. 12. Holdings, LLC September 24, 2009 Existing Conditions: I.G. Holdings, LLC intends to construct a new recreational park to include a 36-hole miniature golf course, go cart track, spin zon"nd arcade on three parcels in Corolla. The total site area is approximately i;- acres. The Natural Resource Conservation Service Soil Survey for Currituck County has mapped the soils on the portions of the site selected for development to consist primarily of Corolla Fine Sand (CoB), Newhan-Corolla Complex (NhC), and Osier Fine Sand (Os). Infiltration rates for these sandy soils typically exceed 20 inches per hour, with the exception of Osier Fine Sand, which often contains a limiting layer on the surface that is estimated to be between 6.0 — 20 inches per hour. The site varies in elevation, with the majority of the area between 4.0 and 9.0 feet above mean sea level (NAVD 1988). An existing non -jurisdictional manmade pond (± 4.68-acre) is located on the site and observed water levels within the pond are ±1.5' msl. The property is within a flood zone of "AE-5" and "X". The northwest portion of the site is partially wooded while the remainder of the site contains native grasses, scattered trees and a gravel parking area. Soils investigations were performed by Albemarle & Associates. These borings were performed using a hand auger to a depth or four to five feet below the existing surface. The borings confirmed that the soils matched the Soil Conservation Service description for the soil types noted above. seasonal high water table appears to vary with proximity to the pond as expected. The ESHWT in the vicinity of the infiltration basin is 4.3' above msl. The results of these borings are attached to this report. Infiltration Basin An infiltration basin was chosen for this site due to the excellent suitability of the soils, available space and contributing drainage area. Holdings PO Box 120, Kitty Hawk, NC, 27949 September 24, 2009 Existing Conditions: The 38.253 acre site in Corolla is adjacent to the Currituck Sound and contains Coastal Wetlands, 404 wetlands and a non -jurisdictional manmade pond (± 4.68-acre). The Natural Resource Conservation Service Soil Survey for Currituck County has mapped the soils on the portions of the site selected for development to consist primarily of Corolla Fine Sand (CoB), Newhan- Corolla Complex (NhC), and Osier Fine Sand (Os). Infiltration rates for these sandy soils typically exceed 20 inches per hour, with the exception of Osier Fine Sand, which often contains a limiting layer on the surface that is estimated to be between 6.0 — 20 inches per hour. The site varies in elevation, with the majority of the area between 4.0 and 9.0 feet above mean sea level (NAVD 1988). Observed water levels within the pond are ±1.5' msl. The property is within a flood zone of "AE-5" and "X". The northwest portion of the site is partially wooded while the remainder of the site contains native grasses, scattered trees and a gravel parking area. Proposed Development., I.G. Holdings intends to purchase three parcels in Currituck County for the purpose of developing the "Corolla Fun Park". The facility will include two 18-hole miniature golf courses, a go cart track, spin zone and arcade as well as a parking lot and walkways. Carolina Water Service will supply the project with water and sewer service. NCDENR Permitting Compliance: The site is being developed per the Low Density Stormwater Management Option, meeting the requirements for the 20 coastal counties. The total site area is 38.253 acres, which includes 8.073 acres of coastal wetlands as well as the 4.68 acre pond, leaving a "NCDENR-DWQ project area" of 25.50 acres. The proposed coverage for the site is 7.1% based upon the built upon area of 79,354 sf. Stormwater runoff from the site is being collected via sheet flow to swales and directed to adjacent open space and the existing pond. Runoff from the parking lot as well as a portion of the building is being collected by a closed collection system and directed into an infiltration basin on the north end of the site. The basin provide volume in excess of the 4,050 cf produced by a 1.5" rainfall event and manages a total of 16,950 cf considering infiltration during a two hour period. Runoff in excess of this amount bypasses the infiltration basin and is directed into an existing drainage system that conveys the runoff to the existing on -site pond. Collect all runoff from all BUA (proposed and/or existing and/or offsite) as the case may be, by any means including piping or swales, and direct it to the infiltration system. B. Check the proposed basin/ trench design to make sure it meets or exceeds the minimum design criteria for design storm, volume and drawdown. II. What makes up a complete infiltration basin / trench application package? A. Two sets of sealed, signed & dated layout & grading plans with appropriate details. B. Completed application with supplement(s), SWU-101, SW401-Infiltration Trench, SW401- Infiltration Basin, and inspection and maintenance agreements. C. Deed restriction document, if applicable. (subdivisions & projects with out parcels) D. Sealed, signed & dated calculations. E. A soils report containing information in regard to the soil type, boring locations, seasonal high water table elevation and expected infiltration rate. `.* • F. Chlorides test results must be provided if the project is within ''/z mile of SR waters (Phase II). (This is only required to test out of SR water treatment requirements) III. BIMS ent4 (for DWQ use only) Enter & track application acknowledged date, review date, add info requested/received dates, permit issue dates, and drainage area info. Best done after the add info letter is written and before sending permit up for signature. IV. Review Procedure- look for consistency between the various elements of the package- application; plan and calculations. A. APP CATION 1. riginal sign tures are required. A signed, dated and notarized Inspection and Maintenance Agreement for each proposed system with an original signature. 3. V The elevations, areas and volumes reported on the supplement match up to the numbers used in the calculations and shown on the plan details. 4. t , Built -upon areas are reported in square feet in Section 111.6. 5. Receiving stream name and classification. 6. ✓ Section 111.6 is filled in -cannot be left blank. One column must be filled in for each proposed infiltration basin or trench. 7. ✓ If the applicant is a corporation, partnership or LLC, look it up on the Secretary of State Corporations Database. Make sure corporation is spelled correctly (capitalization and punctuation matter) and that the person signing the application is at least a vice- president in the corporation, a General Partner in the partnership, a member in a member -managed LLC, or the manager of a manager -managed LLC. Need documentation to support if the Articles of Incorporation do not list the members or managers of the LLC. If an agent signs, then a letter of authorization is needed from the ✓president, vice president, general partner, member or manager. S. For subdivided projects, a signed and notarized deed restriction statement must be 11 vided. 9. Soils report with the SHWT, Soil type and expected infiltration rate. Infiltration Basin & Trench, cont. B. CALCULATIONS 1. t/The basin or trench must draw down within 5 days. Spray irrigation is not acceptable as the sole means of drawdown. 2. ✓The design storm is 1.5" if the project is subject to Phase II or is sited within'/z mile of and draining to SA waters. The design storm for projects subject to Phase II sited within '/z mile of and draining to SR waters must treat the difference between the pre and post development conditions for the 1-yr 24-hr storm. Otherwise, use V design /storm. 3. ✓/Report the bottom surface area. 4. /Report the surface area at the overflow elevation. ✓ 5. how the calculation for the minimum required volume. '171 6. ( Provide a table of elevations, areas, incremental volumes and accumulated volumes to demonstrate that the minimum required volume has been provided. 7.Vcalculations are signed, sealed & dated. V. 8. Report the Drainage Area and Built -upon area for each basin or trench. 9.�The seasonal high water tablemust be at least 24" below the proposed bottom elevation of the system. For Phase II projects, at least 12" of this must be in naturally occurring soils. 10✓All runoff in excess of the design storm must bypass to a vegetated filter prior to entering the basin or trench. If the system is designed to store a larger design storm (generally 2-3 times the minimum), the requirement for an offline bypass can be waived. Please contact the Division to determine what design storm i appropriate for the project. Infiltration Basin & Trench, cont. C. PLANS - Plan sheets should be kept to a minimum. For small commercial single basin projects, the plan set could consist of only 2 or 3 sheets, layout, grading and details. For larger projects, show as much information as possible on as few sheets as possible, without clutt ring them up. 1.piDevelopment/Project Name 2. Drainage easement locations shown on the plans along with their width. 14. Drainage delineated, or a note on the plans that none exist. Provide a copy of the wetlands delineation map signed by the Corps of Engineers, or have the applicant include a copy of the unsigned delineation map submitted to the Corps. Infiltration systems may not be located in wetlands unless a permit to fill those wetlands has been obtained and the soils meet the SHWT and hydraulic conductivity requirements. 15'VDetails for the roads, parking, cul-de-sacs, including sidewalk width, radii, dimensions & lopes. 16 �/ Apartment / Condo development- provide a typical building footprint with dimensions, and note all concrete and wood deck areas. 17 The drainage area for each basin or trench is clearly delineated and numbered to match up,to the calculations and supplement. Drainage area delineation is best done as a separate plan sheet. 18.,A basin section detail showing bottom and storage elevations, 3:1 side slopes. The side slopes of an infiltration basin are not restricted to vegetation. Vegetation is specified for basin slopes not being hardened. Weeping Love Grass is not 'suitable as a permanent vegetated cover for side slopes. 26.All roof drainage must be directed to the system.
