A Division of Energy, Mineral, and Land Resources file review has determined that
Stormwater Permit SW7060825 for a stormwater treatment system consisting of two
wet detention ponds serving the Albemarle Hospital located at 1144 N. Road Street,
Elizabeth City, NC expires on January 22, 2021. This is a reminder that permit renewal
applications are due 180 days prior to expiration. We do not have a record of receiving
a renewal application.
Please submit a completed permit renewal application along with a $505.00 fee for

Surgery and Emergency Department Expansion

Dear Mr. Hodge:
Please find enclosed two (2) sets of construction documents for the development of the
above referenced project, and a check for $420.00 (application processing fee). The
proposed development is the expansion of Albemarle Hospital's Surgery and Emergency
Departments which are located on the southeast corner of the existing building.
Albemarle Hospital is located on US47/N. Road St. approximately 0.7 miles north of the
intersection with US-158/N. Hughes Blvd and is zoned 0&1, Office & Institutional. The
site has frontage on its western boundary along N. Road St. of approximately 700 feet.
The proposed development is bounded on the south and north by existing developments
also zoned O&I. The east side of the site is bounded by wetlands that are adjacent to the
Pasquotank River.
The proposed development will comprise of additions to the first, second and third floors
of the southeast comer of the hospital. The additions to the first, second, and third floors
will total 28,260 sq ft, 29,418 sq ft, and 29,418 sq ft respectively. These additions will
require that the existing infrastructure in the affected area be re-routed or removed.
No new BMPs are proposed as part of this project. The drainage for the disturbed area
will maintain its existing drainage pattern. Drainage for the disturbed area is currently
conveyed to an existing BMP on the northeast corner of the property. Although
additional impervious area is being introduced to the BMP, water quality volume
calculations indicated that the pond does have capacity to support the additional
impervious area. These calculations are included with this submittal.

Please find enclosed 2 sets of construction documents for the above -referenced project
for your review and approval. The plans have been modified to address the continents
from your letter dated November 15, 2006. Below you will find our response to the
comments stipulated in your letter. Also, enclosed for your review are the following:
- 1 copy of the Project Narrative and Calculations Package
- 1 copy of your original comment letter
- Detention Basin Supplement form with O & M agreement
- Photos of the Pond Outfall 30' Filter Strip
ADDITIONAL INFORMATION NEEDED
1) The Owner has been notified of this suggestion but has not provided a
definitive answer at this time. We will advise the DWQ of the Owner's
decision at a later date.
2) See comment response #1.
3) The following are the responses to this multi -part question:
a. Pond information and calculations — information and calculations for
the existing pond are included in the attached Project Narrative and
Calculations Package.
b. Wet Detention Basin Supplement form with O & M agreement — this
form is attached to the re -submittal.
c. Forebay— according to the forebay calculations that were created
using the original design calculations, it appears that the forcbay was
sized to be approximately 20% of the pond volume. See Appendix E
of the attached Project Narrative and Calculations Package for
forebay calculations. Based on field observations, it appears that this

forebay was not only considered in the calculations but also
constructed.
d. SADA — this value is provided in Appendix D of the attached
calculations package. Please note that the 85% TSS removal table
was interpolated in calculating the new SADA value of 5.26%.
e. Storage volume for 1-inch storm — this value is also provided in
Appendix D of the attached design narrative. Please note that the
comparison between the required volume and actual volume has
been revised. The actual volume only includes the volume above the
permanent pool.
f. Orifice draw down time — please refer to the original draw down
time calculation excerpt provided in Appendix D of the attached
design narrative. The original calculation performed by Robey
Associates Engineering indicates -that a draw down time of 2.09 days
was calculated for the pond.
g. Vegetative perimeter shelf— based on field observations, it appears
that a vegetative perimeter shelf is provided.
4) Based on field observations, it appears that a 30' vegetated filter is provided
after the pond. As discussed with Brian Brewer of Kimley-l-lorn &
Associates, we have attached copies of recent photos of the vegetated filter
that confirm this observation. 'These photos have been labeled as
"Vegetative Filter Strip". -These observations confirm the ability for the
design calculations to utilize the 85% TSS Removal table. To ensure that the
velocities in the vegetative filter did not adversely affect the vegetation in
this channel, FlowMaster was utilized. FlowMaster calculated a velocity of
1.98 fps which is below the maximum permissible value of 2 fps per
NCDENR's Stonuwater Best Management Practices manual. The
FlowMaster calculations are now provided in Appendix G of the attached
design narrative.
5) Based on the revised required storage calculations that take into account the
additional impervious area now being conveyed to the pond, there is
sufficient storage above the permanent pool volume. As mentioned
previously, the comparison between the required volume and actual volume
has been revised. The actual volume only includes the volume above the
permanent pool.

Dear Mr. Balagurchik:
This office received a Coastal Stormwater permit application and plans for the subject expansion project
on August 31, 2006. A preliminary review of your project indicates that the wet detention pond serving the area
you propose to renovate was originally authorized under stormwater certification number SW7940703 issued
November 1, 1994. The Hospital has also been issued a stormwater permit number SW7021021 on November
15, 2002 for the front portion of the property. I would suggest that we combine both permits at this time so that all
stormwater management units could be covered under one permit. This is not a requirement, only a suggestion.
Before a State Stormwater permit can be issued the following additional information is needed:
1. Please advise if you would like to combine all permits on this property.
2. In what official name should the permit be issued?
3. Since the amount of impervious area in the drainage basin tributary to the wet detention pond on the
rear of the property is increasing this pond needs to be brought up to current design standards.
Please provide information, calculations, wet pond supplement form with O & M agreement and plan
details that show that this pond meets current design standards including current forebay design,
SADA, storage volume for 1 inch storm, orifice to draw down the 1 inch storm in 2-5 days and current
design for the vegetated perimeter shelf.
4. The project narrative indicates that the wet pond current conditions were approximated based on
original design criteria. The current pond conditions must be field verified including whether there is
a vegetated filter after the pond. If there is no vegetated filter a 90% TSS removal design should be
used.
5. Only the storage above the permanent pool should be considered when calculating the storage of
the 1-inch rainfall in the pond. It appears that entire volume of the pond was considered for storage.
The above requested information must be received in this office prior to December 15, 2006 or your
applications will be returned as incomplete. The return of these projects will necessitate resubmittal of all required
items including the application fees. If you need additional time to submit the required information, please mail or
fax your request for time extension to this office at the Letterhead address.

You should also be aware that the Stormwater Rules require that the permit be issued prior to any
development activity. Construction without a permit is a violation of 15A NCAC 21-1.1000 and North Carolina
General Statute 143-215.1 and may result in civil penalties of up to $25,000 per day.

Dear Mr. Balagurchik:
The Washington Regional Office received a completed Stormwater Application for the subject project on
December 15, 2006. Staff review of the plans and specifications has determined that the project, as proposed, will
comply with the Stormwater Regulations set forth in Title 15A NCAC 2H.1000. We are forwarding Permit No.
SW7060825 dated January 22, 2007 to the Albemarle Hospital Authority for wet detention ponds to serve the
Albemarle Hospital site located at 1144 N. Road Street in Elizabeth City, NC.
This permit replaces permits SW7021021 issued November 15, 2002 and SW7940703 issued November 1,
1994 and shall be effective from the date of issuance until January 22, 2017 and shall be subject to the conditions and
limitations as specified therein. Please pay special attention to the Operation and Maintenance requirements in this
permit. Failure to establish an adequate system for operation and maintenance of the stormwater management
system will result in future compliance problems. The issuance of this permit does not resolve any previous violations
of the stormwater rules for construction without a permit.
If any parts, requirements, or limitations contained in this permit are unacceptable, you have the right to
request an adjudicatory hearing upon written request within thirty (30) days following receipt of this permit. This
request must be in the form of a written petition, conforming to Chapter 150E of the North Carolina General Statutes,
and filed with the Office of Administrative Hearings, 6714 Mail Service Center, Raleigh, NC 27699-6714. Unless such
demands are made this permit shall be final and binding.
