If you already sent an application, please keep in mind we are receiving hundreds of them and are limited on manpower.

How to Submit Paper DMRs Before eDMR is Implemented
The Stormwater Program is planning for NPDES Stormwater Industrial permitted facilities to use
electronic Data Monitoring Reports by January 1, 2022. Until your facility begins using eDMR,
follow this NEW procedure for submitting eDMRs:
• Download the applicable DMR from the NPDES Stormwater Industrial Individual website or the NPDES
Stormwater General Permit website.
• Complete, sign, scan and upload the DMR to the NPDES Stormwater Permit Data Monitoring Report
(DMR) Upload form within 30 days of receiving sampling results.
• Mail the original, signed hard copy of the DMR to the appropriate DEMLR Regional Office.
• Do not send DMRs to Central Files in Raleigh! (This is a change from the previous procedure.)
• Note that DMR forms have recently been updated.

You will need to contact Suzanne McCoy concerning getting signed up on eDMR (I have copied her on this email).
As for the spill sample results, please submit the results from the lab, along with a cover letter summarizing what
happened, what was tested for, and the test results. This will be included as part of the spill response.

I have completed the DMR for September Storm water discharge analysis. (Please see attached). It looks like I need to
be set up in the system to upload it to the eDMR system. I didn't see a "new user" sign up. I will get the original copy in
the mail today to you.
The spill samples that the Ken and Rose asked for were not the same as what we test for in our SWPP. Do I need to
report them the same way or differently? Also, I find it very odd that the Formaldehyde results are below recordable
limits. They are sending this test to be done off site now so I am wondering if there is a different test method. I have
attached the actual results for you to review as well.
I know I am asking a lot of questions, I apologize and I will learn quickly.

Yes, that is the correct form. Make sure you label the parameters that were tested for in the headers in the table for
Part A. Please also include the benchmark limits as specified in Table 3 of Section II of the Individual Permit for this
facility.

Good afternoon all,
I just received the results from the 2 pond samples that were checked after our glue spill. The first sample is where the
drains enter into the main pond. The second sample is where the 3rd pond drains into the spillway to our outfall. lam
attaching these results for you to review. The only recordable amount was Barium. I spoke to our Chemist and nothing
in our products produce or involve Barium, so I hope this is something that occurs naturally. I am sure you guys have
more insight on that than I do.
